The safety first philosophy is ultimately responsible for the stupid accidents that occur everyday in the oil patch. The oil industry has inadvertently created a workforce dependent on upper management and their "checklists". It has taken the modern day workers responsibilities and accountability away. When workers learn responsibility and accountability they can learn modify their risk taking behaviors. Once thats all sorted then the accidents will go away.
