The only times I have to deal ( or choose to deal with) 360p is on Tek Syndicate's content, and that is rarely.

some videos are pre-uploaded and released on a schedule. (I.E. uploaded @ 7am, released to public @ 9am). when this is the case, youtube has the time between 7am-9am to process the video & as such when the video is released to the public at 9am - it is released at full quality. However if a video is uploaded for immediate release (no release schedule) then sticking to the same situation - the video would be uploaded at 7am and live to public at 7am aswell. This means that at 7am, the public can watch the video but youtube hasn't yet had the chance to fully finish processing - hence why higher quality variant's are not available (the video has not yet finished processing).
