Very nice video. But this is the good scenario. The bad scenario is to have to deal with an obese patient where the blade of the scalpel might not get into the trachea,and so will not enter the bougie or the ett 6.0

How would you dress the tube so the hole in the neck doesn't get infected. Sterile bandaging is essential to line insertion, this hole seems a lot bigger and would need to be protected with some sort of sterile wrap.

Essentially once CICO has been activated and this technique applied the need for sterility goes down in the priority because definitive airway takes presidence over infection which can be treated as a secondary measure later. Those who require CICO will need ENT referral for further management and assistance as well.
