Dr Imtiaz Hussain is a Consultant General and Laparoscopic Surgeon. After graduating from Nishtar Medical University, Pakistan, he visited Australia for Australian Medical Council exam. He completed his FCPS degree which is the highest degree in Pakistan in the field of General Surgery. During his postgraduate residency program, he had also trained himself in the field of Urology, Plastic Surgery, Paediatric Surgery and Orthopaedic Surgery.
Since 2012, he, as a volunteer, is working as an instructor for the Basic Life Support (BLS) Program in Fatima Jinnah Medical University Lahore, Pakistan. In this course, he is the Master Trainer and teaches CPR and other lifesaving techniques to Doctors , Medical Students, N

Thanks Imtiaz sb. I am a fistula patient and had met with four revisions of seton placement. My last procedure is done on 28oct2023. I have 7 cm high and complex fistula (single track). My treatment is still on way. But my surgeon has used a ruber band as seton instead of thread, is it good technic?. Nicely defined everything in this video. Jaza'ak Allah.

Agar aap ne operation karwaya hai aur jahan seton laga tha, wahan abhi bhi bohat dard ho raha hai, to yeh zaroori hai ke aap apne doctor se consult karein. Seton lagne ke baad dard hona aam baat hai, lekin agar dard zyada hai ya lamba arsa guzar gaya hai aur dard kam nahi ho raha, to yeh check karna zaroori hai ke koi infection ya complication to nahi ho gayi. ### Kuch Aam Wajuhat Jo Dard Ka Sabab Ho Sakti Hain: - **Seton ka Tight Hona:** Kabhi kabhi seton zyada tight hota hai, jiski wajah se dard ho sakta hai. - **Infection:** Agar wahan infection ho jaye, to dard zyada ho sakta hai. - **Healing Process:** Jahan seton laga hota hai, wahan healing mein waqt lag sakta hai, aur is doran dard mehsoos ho sakta hai. ### Kia Karna Chahiye: 1. **Doctor se Consult Karein:** Apne surgeon se mil kar unko apni condition ke bare mein batayein. Thank you very much. I had my second surgery 20 days ago. During the first surgery, my doctor placed a seton and advised occasional pulling during sitz baths or bathroom visits. It began to loosen, as mentioned in your video. When I visited my doctor, they mentioned they would examine under anesthesia to decide whether to replace the seton or remove it. During the examination, they cut and left the wound open, mentioning that it had healed from the inside, thus avoiding the placement of a new seton. They explained that it would naturally heal over 1-2 months. It's been 20 days, and I've noticed a sticky discharge in the mornings and experience gas and farting before sleeping. I have two questions: 1. My doctor mentioned the wound would heal naturally. Does this indicate a fistulotomy? Is it advisable to start with seton placement and then proceed with the fistulotomy in a second surgery? 2. Are farting and gas normal after fistula surgery? I've been using Caremaffin syrup as prescribed by the doctor for stool softening over the next two months. Could this syrup be causing the gas and farting?

আসসালামুয়ালাইকুম স্যার। বাংলাদেশ থেকে বলছি।স্যার আমার fistula in Ano(complex) অপারেশন করছি একমাস হয়েছে এবং সেটন লাগিয়ে দিছে। এখন ওটা ধরে আস্তে আস্তে টানতে বলতেছে। কিন্তু স্যার টানার কয়েকদিন পর লক্ষ্য করছি আমার হাওয়া বা বায়ু বের হচ্ছে।কোন ব্যাথা বা রক্তপাত নেই। সামান্য ঘাম ঘাম পানি আসে। স্যার এখন বায়ু ছাড়লে সেটনের পাশ দিয়ে বের হয়ে যায়। এতে কোন সমস্যা আছে কি????

Sir, very good information I got fistulactomy and Seton placement simultaneously... Fistulactomy wound healed timely ....but Seton had been very painful and tightening was done 4 times....after one and half month Seton got out....but still Seton pathway or wound pains ....and it seems that healing is not proper.... I met my surgeon who is very competent... but 2 and half month still I am not fully recovered... If you could suggest me... What should I do. I shall be obliged.... Apko apny operating surgeon say rabta karna chahiay. Wo apko behtre guide kar sakty hain. Hum baghair check up kiay koi medicines ya ilaaj nahin bta sakty aur na apko baghair examination kay koi medicines leni chahiay. Seton care ke baad anal fistula ka ilaaj bohat aham hai. Yahan kuch zaroori baatain hain: 1. **Safai**: Seton wali jagah ko naram garam pani se din mein 2-3 dafa saf karo, khas tor pe pakhane ke baad. 2. **Dard ka ilaaj**: Dr ke diye gaye painkillers ka istamal jari rakho. 3. **Dressing**: Zakham ko dry rakho aur dressing badalti raho. 4. **Discharge ka khayal**: Thora discharge normal hai, lekin agar ziada pus ya badboo aaye to doctor se raabta karo. 5. **Regular check-ups**: Apne doctor ke paas bar bar jao.

Jab Seton fistula ke ilaj mein istemal hota hai, to jab yeh seton final stage tak rakha jata hai aur nikala jata hai, toh amooman saath mein fistula ka tract khatam ho jata hai. Seton ka istemal fistula ke tract ko open rakhne aur infection ko control karne mein madad karta hai. Ismein Seton slowly-slowly fistula ke tract ko heal hone mein madad karta hai. Lekin, har mareez ka haal mukhtalif hota hai, aur ismein kuch variations bhi hosakti hain. Aapko is masle ke bare mein apne doctor se mukammal malumat leni chahiye.

Seton lagne ke baad 5 din tak dard hona aam baat hai, kyunki seton ka main maksad fistula ko khol kar usay theek karna hota hai. Is process mein irritation aur pressure ki wajah se dard mehsoos hota hai. Jaisay jaisay healing hoti hai, dard kam hota chala jata hai. Agar dard zyada ho raha hai ya barqarar hai, to aap apne doctor se mashwara karein takay wo aapko behtar treatment ya pain management ke liye kuch de sakain.
