Everything is finally starting to come together. As a new family doctor I've finally been able to start getting the office set up how I like and am officially seeing new patients. Busy is an understatement right now but at the same time, it feels great to finally be seeing some real progress. My medical residency graduation is this coming Sunday and the day before I'll be working in rural emergency medicine. Never a dull moment but I hope that this office tour was interesting for long time viewers, medical students and even potentially a few future patients :)
If any future family doctors have questions, feel free to ask and I will do my best to share any tips that I have about setting up a new practice.

Hi everyone, it's nice to meet you 🤙 My name Is Gianluca and I'm a second year family medicine resident physician at McMaster University in Ontario, Canada. I can tell you're enthusiastic and an eager beaver to start practicing. You have a plan and what order you have to complete them in. A ultrasound machine would be helpful for sure! Congrats on being set free to help. Please don't shy away from patients who you think are too complicated.

Congratulations Gianluca!! Awesome video as always! Thanks for sharing. Wishing you all the best in your new practice. How were you able to decide on a particular FHO or FHT? Would you recommend one joining a multispecialty team? i.e a practice with pediatricians, Rheumatology, peds and Int med as opposed to a FHO or FHT? Thanks!!

Thank you very much :) I’m with a FHO which tends to be a very good earning model when compared to FFS. When deciding between FHO and FHT I suppose the real question is how much control do you want to have over your own office? Because FHTs are multidisciplinary, these are often relatively large offices with > 5 practitioners (NP, PA, MD, psychiatrist, dibabetes educator, etc) working in the same place. With the increased staff comes increased admin costs and the need for a building manager in many cases… it’s not a bad way to do things, but it can get complicated quickly and depending on where you go, “overhead” can be on the higher end - shop around and see what you’re being quoted. In my opinion, the FHO model has been my favorite but this is made even better if you have good access to specialists in the area that can help with referrals when you need them. Unfortunately, specialist referral is hard to come by in my area in many cases. Final recommendation - keep your overhead low, see a lot of people, do good medicine and go where you feel most comfortable working. Best of luck :)

For a hopeful IMG who wishes to one Day practice in Canada, your vids have been helpful. Now really interested in this family practice of yours. Got many questions though - Shall u be hiring Nurses, lab guys and other assistants to help u? If so how are they compensated...and if not, I'd be intrigued about how you'll handle the work load alone. Plus how did u manage to finance the place..like the setup and all the necessary equipment

Hi :) we have 2 nurses working with us and 1 full time front desk staff. I pay their salaries directly along with the other doctors working - the government does not help us fund their salary so it comes out of our own salary. I pay my overhead costs on a monthly basis - in Ontario, doctors are usually not be paid for the first 3 months of working as OHIP billings are held for 3 months. So you cover the costs of your office in the beginning with loans from the bank or incentives paid to you by the city you choose to work in if they have a program like that in place. Hope this helps

I'm also starting my internship year here in my country in August This year...an intense working period of 12 months.. rotating in all the 4 major clinical rotations before we can be licensed to be medical officers, the would be Family Drs according to your system. Hello doctor I'm so motivated by your videos and I love your patience and hardworking. I really need your advice I'm from Egypt and I'm at my fourth year in medical school. I suffer from domestic violence and my mom used to punish me by locking the door and not letting me go to exams that really lowered my GPA and I have 2 years gap because of that and PTSD. I left my parents house because that really destroyed me and my schooling, I hope you don't judge me for that as I feel like if you don't go through this you won't understand stand . My question is would the 2 years gap affects the match ? And another thing I work and I pay for my own things but that really barely enough for my rent and food , do you know any organisation or people that would help me pay for step1 , they can pay it themselves because it's okay to be suspicious and don't believe me , and I will work more to be able to pay for step2 after I graduate because my current job is giving me 3000 Egypt pound and I use them all to pay rent , food . Hi Doc, it's so nice to hear your thoughts. I am also a Canadian and an IMG but I completed my MRCGP training from the UK, now I am planning to move to USA. Can you please advise which US states are easier to get in? I have LMCC from Canada, plus ECFMG I also have USMLE 1,2, Step CK & CS currently I am working as GP in family medicine in UK and I would really like to hear your thoughts on this? Which US states are easier to get in family medicine? Please advise

Hi :) I don’t know what your experience has been like so far working in the US, but instead of making your decision based off of which state is easiest to work in, I would suggest going through the licensure process in the state that you can see yourself living in long term. Each of the states is different in terms of people and their experience with the healthcare system and working in Florida is much different than working in New York. It’ll depend on what you’d like for yourself. I would recommend looking at the process on their official websites and deciding if it’s worth it for you. As a personal example, I will at some point soon look into getting licensed in Florida - I have no plans of moving there right now but think it will be beneficial for me to have personally. @@nxtgenmd Hello once again Doc, and thanks for your response. Actually since I am not trained in Canada I am facing a lot of hurdles in transitioning from the UK to the USA physician pathway as there are only few states that accept UK trained MRCGP. But I am still hopeful and exploring, trying to find new information from people who are going through similar experiences. So far it looks like I will have to do a fellowship in the USA to get the flexibility to choose the states which I desire 😞. Anyway the UK job market isn't that great but I am still thankful since they gave me a chance. Anyway it's really nice to hear back from you. If you get any information regarding transitioning from the UK to the USA please do share it.
