Details of a crash investigation two decades ago that claimed the lives of 7 in a 6 seat Beechcraft Baron B58P. The tragic plane crash occurred due to an engine failure and the inability of the pilot to retract the landing gear. The aircraft burst into flames in a wooded area two miles from the airport in Keene, New Hampshire.

That’s the case anywhere. Flying driving working . Even sport shooting. Don’t take care of your equipment, you’re going to be in trouble. I worked at an FBO in college and some owners thought their planes were toys, they had issues. The guts that treated it like a profession didn’t. Also GA guts need to act like airline guys an operate with a minimum maneuvering speed with built in cushions so the don’t stall spin

So, is it safe to say it was his fault? I know that sounds harsh..... or was it engine failure and having the landing gear down. Wow, I never thought that having landing gear down could cause such a problem. Very sad story.

@@lorimart747 you don’t intentionally take off in a plane with a known discrepancy like “the gear won’t retract”. That’s kind of a big deal. Most light twin engine planes like this cannot experience an engine failure on takeoff and continue to climb.. it WILL crash. Twins also have higher stall speeds, and off airport landings aren’t as survivable like a single engine plane would be.

I will throw in my two cents and say "thank you" for an informative, well-written, and extremely listenable narration of this incident. So many factors involved that any one of them should have kept them grounded. In addition to the unexpected engine failure! A very sad ending to what was likely a nice family weekend trip. Likely over gross weight, possibly out of cg limits, departed with inoperative landing gear, pretty much guaranteed the outcome in the event of an engine failure. Exceptionally poor judgement and his family paid the price. Tragic, but completely avoidable.

Just want to mention, it is actually allowed to have 2 small children on one seat, in small aircrafts. I was surprised by the fact, but this is actually written in FAA rules.

The NTSB doesn’t clarify the ages of the occupants. They only mention adults and teenagers. It is possible that young kids were on board as well

As a former military aircraft mechanic I'm all to familiar with situations just like this. As technicians we have a system of expertise levels that would allow us to ground the aircraft, military or civilian on a overseas military base. That Experience and testing starts with a level of 7, called the " Seven Level." This gave us the authority to ground aircraft based on the problem. When I reached this level of experience I had absolutely no compunction grounding a aircraft, not caring who would scream at me for doing that. I've seen too many near mishaps, one's I can't mention to let myself get complacent. With aircraft mishaps it's almost always a series of small problems coupled together that lead up to a crash. I feel for that family's and can't imagine the horror the passengers felt, even though it was only a few minutes at best.

Indeed, it’s almost always a series of small things that cause catastrophes, not a single big thing. Aeronautical decision making requires PIC’s to looks for the small things and add them up before their cumulative weight negates the physics of controlled flight. If ADM is contaminated by “get-there-it is” the calculation fails all too often. Yes, as pilots we have all bent the rules. But as we gain experience (not necessarily reflected in logbook hour totals, unfortunately!) our personal minimums should and often do shift toward being more cautious. Such was not the case here, apparently. As PP owner-operators, we don’t have dedicated ground crew who vet our aircraft before every flight. We have to do our ADM alone. Flight service and ATC won’t do it for us. In my experience, this means as owner and PIC I have to regard “the rules” not as something fluid that I can play around with, but firm boundary markers for the limits of safety. This is especially true when there are other souls whose lives depend on the PIC making good decisions. The possibility that my flight decisions could be responsible for the deaths of my wife, children, and grandchildren, as was the case here, is one I take very seriously, and even moreso after viewing this excellent video presentation.

Having six or four seats in a GA aircraft doesn't mean you can fill them with obese passengers while ignoring density altitude and weight and balance limitations. These aren't cars we're stepping into.

Its so sad that this pilot’s bad decisions, caused the death of his loved ones. RIP🙏🏻. To add context, in 1976, I checked out in a baron 55 at age 23 in Norfolk. I don’t remember how the quality of my checkout was. But, in flying it for a year (500 hours) into major airports, in cold and hot temperatures, over inhospitable terrain and into small GA airports, I never lost an engine. But, I also never put 7 people in it,. I also understood and complied with all that was a in the POH, and I never dreamed of a long flight, with the gear down.

Engine failure on early climb-out in a twin is a handful when everything else is perfect- you must keep on task, know your numbers, and execute the procedures. Most twins have two power sources for a reason... it's because they don't fly worth a crap on one. Throw in other inoperative components, questionable weight and balance numbers- you're likely destined for an unplanned intersection with terrain. Sad but so avoidable. Never, never depart with known deficiencies- the aircraft's, which includes it being outside of it's performance and/or weight and balance envelopes, or your own. Why do so many find themselves in this situation? Hyper-extended no-go thresholds is all I can think of. RIP to all, and condolences to surviving loved ones.

@@lorimart747 the engine was defective but the added strain put on the engines by flying with the gear down with high RPM settings to try to gain altitude because of weight could have caused the marginal engine to fail.

Reminds me a bit of Rod Machado describing 'seeing 7 people climb out of a Cherokee 6'. I don't recall hearing of this tragic accident before, but I think I'll look it up on the NTSB. I'm curious for a more-detailed breakdown on this cylinder failure. I like the style of the video. When I started flying, there weren't many sources for animated or real video of plane crashes. Seeing a crash and lead-up to it is completely different than just reading some dusty old report. These are a great resource for newer pilots, but somehow, preventable accidents continue to happen, despite a better overall awareness of the factors contributing to them.

Assuming the weight and CG were within limits, a Baron can carry more than 6 legally as long as each additional person is "held by an adult who is occupying an approved seat or berth, provided that the person being held has not reached his or her second birthday and does not occupy or use any restraining device."

Fantastic excellent summary. Thank you for creating this presentation. I have flown a twin Comanche over a long distance with the gear in the down position because of failed component. I was not at maximum weight, but certainly put myself and additional risk for the engine and field. There, but for the Grace of God go I.

Sounds like the pilot was an accident waiting to happen. Cramming too many people into the plane may not have caused the crash, but it says something about his reckless attitude to safety. I simply would not have got into such a plane. The teenage kids probably didn’t know enough to consider themselves anything other than immortal and it’s sad he was willing to take advantage of kids that age with their whole lives in front of them.

Piston engines fail about every 3000 hours per the FAA. So, on a twin piston, if you fly 1500 hours you are due for a single engine emergency. Flying overloaded with stuck gear was about as stupid as it gets...even with the gear retracted he might not have made it.

NTSB listed flying with a known mechanical discrepancy as the cause, but poor judgment probably fits it better. I’ve ferried aircraft with the gear down, but always with the aircraft as light as possible (and a ferry permit).

I know I'm only a Private Pilot with an instrument rating but I think a huge Factor here is poor ADM. We all know an accident is a series of events leading to the final crash. That's said it started with his decision to carry more people than seatbelts. He topped off the tanks. The biggest bad decision was to take off with defective landing gear. Even a 1,400 our pilot should know that the increase parasite drag would affect all the performance characteristics on that aircraft. I blame this accident on poor decision-making and get home itis

We have all taken calculated risks, but when the risks that we knowingly took are complicated by a sudden factor we didn’t expect.. we are lucky if we survive and learn. I survived many unexpected factors by learning not to start out with any defects to “deal with”. Start out with an airworthy plane. The thought “ this is a short trip” is a killer.

You know the IO 550 Continentals are touchy and very seldom make it to TBO. I understand how pilot error is a factor yet the IO 550s vs IO 520s have not been as reliable.

Was there a toddler among the víctims? The rules allow for a toddler to ride on the lap of an adult and not to be counted as a pax. So literally, up to eleven persons could legally ride in a six-seater.

The NTSB did not provide a list of passenger ages, but the witness reports they included only mentioned adults and the teenage girls

Multiple errors of judgement are not additive, they were exponential in this case. Absolutely no respect for the potential disastrous events that could occur or the consequences. When you take your own life in your hands, you are the one who suffers the consequences. When you put the lives of innocent passengers in high danger, the consequences compound. How many lives were irreparably changed here?

In certain parts of the world, motor vehicles have to go through vigorous inspections just to be allowed onto the roads. They are called MOT testing. If this type of testing was introduced into aviation, half of the still flying relics of the past would be gone. And possibly these people might still be around. Perhaps not the pilot, quite a few mistakes and poor judgment are evident here. So tragic. One moment all is merry, then the culmination of every small mistake and circumstance results in sheer calamity. Thanks for the upload.

All aircraft in the United States must undergo a annual inspection in order to receive their airworthiness certificate from the FAA for that year. Aircraft used for commercial purposes are inspected every 100 hours. It is up to the pilot to determine the airworthiness of the aircraft before *EVERY* flight. In this case, the pilot unfortunately decided that it would be a good idea to essentially become a "test pilot", and conduct the flight just outside of the safe operating envelope of the aircraft.

Yours is a good answer. But a great deal of the accidents on RU-vid aviation are airframe or engine maintenance issues. If it wasn’t for the specific media sites most of these accidents would go unnoticed. Because of the sheer cost of aviation, a lot of aircraft are flying in some comprising condition. Just waiting to pounce on the right set of circumstances that will catch out the crew or ground crews. We can learn from these situations but never stop it. Thanks for the rebuttal.
