Always enjoy listening to your discussions! As a general surgeon I also do colonoscopies and flexible sigmoidoscopies. I find that patients are more compliant with oral bowel prep going the typical direction than they are with enemas going the opposite direction. Medicare pays $191 for a screening colonoscopy and $59 for flexible sigmoidoscopy (CA), so not quite that $2000 in the cecum😂. The fear of being blamed for leaving a right sided lesion behind is a stronger force than any financial incentive, despite not having a statistical basis for that feeling. Really appreciate the work you do!

Since US insurance is largely only covering colonoscopy and not covering sigmoidoscopy, this whole discussion is just pushing people to skip all the screening which seems like a net harm to me. It's true that best current evidence shows sigmoidoscopy is just as good as colonoscopy, but some screening is better than no screening even if it's not as fantastic as marketing might make it seem. (Bretthauer et al 2022 "Effect of Colonoscopy Screening on Risks of Colorectal Cancer and Related Death") Additionally this discussion is skipping over separate advice for people with family history. Prep is not that bad. You take a vacation day and play with your phone on the toilet all day

Colonoscopy is not that bad, you get sedated but a little, you can watch the screen with the doc. It is just uncomfortable afterwards as you’ll have a lot of air in your intestines. Prep is indeed a bit difficult as you will be sitting on the toilet a lot, but you prepare at home so not a big deal. If you are concerned and have some symptoms go and have it. Scripps in San Diego does a great job, not sure how it is done elsewhere.

Had the colonoscapy at 52. The worst thing was drinking the prep "go lightly" everything else was easy. Drink it all the day before. I drank 12 oz every 90 minutes until it was gone. About 8 glasses. I mixed 3 or 4 packets of crystal light lemonade flavoring into the gallon jug. It helped a lot. Dont mix anything with red or blue or green. Clear as possible. No issues. They found 1 polyp. Told me not to come back for 7 years. I will wait until 60. Pro tips : Eat light the day before the prep. Mix in crystal light lemonade to make it taste better. Drink it all the night before. 8 large glasses with ice works good. Do not eat anything once you start prepping. Do not eat anything until you get the proceedure other than water.
