Sacroiliac joint pain can often be mistaken or inappropriately treated for other causes of low back pain.
Piriformis syndrome:
The sciatic nerve is compressed by the piriformis muscle in the buttock. Piriformis syndrome may be associated with lower lumbar radiculopathy similar to spine pathology. It occasionally develops due to blunt trauma to the buttock. Localized buttock pain increased by sitting or driving, tenderness in the sciatic notch.
Lumbosacral disc herniation or bulge:
The disc may move out of place (herniate) or break open (rupture) from injury or strain. Disc herniation of the lumbosacral region could involve the nerve roots causing low back pain. The pain is usually midline and can go down the leg.
Lumbosacral facet syndrome:
The facet provides stability for the spine. These facets contain a joint. That joint may be affected with inflammation or degeneration causing pain that can be mistaken for SI joint pain.
Lumbar radiculopathy:
Major source of back pain. Occurs from inflammation, irritation or impingement of the nerve root. Condition can be confused with SI joint pain.

The ultimate explanation of my case 🙏🏻 after dealing with sciatica for 4 months doing wrong exercises and physical therapy that aggravated my pain. And recently after a week of exercises for SI joint and muscles strengthening I finally feel the improvement. I wish no one went through this terrible pain as I had, this is the most miserable time I’ve ever had in my life…

First couple weeks after I fell down on my right buttock, it was just sore but then it started to increase and I fell the pain in my piriformis muscle, it was always in spasm, what caused sciatica symptoms as well, the pain was so bad from my buttock straight down to the leg and toes. Any painkillers hardly helped me these months. But as soon as I haven’t received the help from my health providers, I did a research on my own. And found out about SI joint and tried every exercise that was suggested by those physical therapists on RU-vid. Now my pain decreased from 10 to 1-2, I have no longer sciatic pain, I just feel instability in my SI joint, it’s a little bit out of place, because I feel these “clicks” sometimes, but as I’m strengthening more the muscles around my lower back and buttock, it’s slowly getting better. I have a couple advices for those who struggle (only from my own experience): 1) less painful activity, more muscles strengthening, be gentle, don’t over exercise, less stretching, don’t stretch your hamstring, no nerve flossing please 2) sleeping position is better on your good side with a pillow between your legs, firm mattress highly recommended 3) watch your steps, watch your posture, stay in balance, don’t stand on one leg only, no couches, sit on a firm seat 4) vitamins: B6, Magnesium Glycinate, Turmeric highly recommend 5) drink water 6) rest 7) and be patient Good luck!

@@shashigoyal6677 the main focus is on strengthening the glutes:) my routine starts from sitting in lotus position, but not crossing my legs too much, just to feel a nice stretch of hamstrings and glutes. Then I lay down on my back, I bend my knees and pull them a little towards my chest, then I slowly swing my hips, pointing my bent knees to the right and to the left for about 3-5 minutes. Hands on the floor maintain balance. Then I do the “bicycle” exercise, as if I were pedaling. Then I do about 10 bridge exercises, slowly without sudden movements, I don’t overdo it, the main thing is to feel that the gluteal muscles are working. Another good exercise is to stand on your knees and hands, like a table position, the back is straight, and you lift one leg still bent at the knee (8-10 reps), then the same with the other leg. And I finish with a downward facing dog pose stretching, or a child pose. I do this set everyday 2-3 times. I also could share a few physical therapists channels, who recommend the approach I like and that worked for me:)

My opinion is that doctors should have been warning that pounding pavement/ hard surfaces is not good for a body, no matter what kind of shoes or form they have. People get addicted to the chemical surge from running and sure your heart is benefiting from activity but the body is being abused and broken- guaranteeing you will need orthopedic and spinal care for that last 1/2-1/4 of your life .

DR., a fabulous video, very descriptive and well illustrated. I have numerous spinal maladies including severe DDD and facet Joint Arthritis and Spondy of he spine. My Rheumatologist who after seeing numerouis doctors seeking help with Chronic Pain and nerve conditions, told me that I SI joint inflammation. She told me that the SI joints are the eopicenter to many Spinal Problems - going up and down. All the surgeons that i have consulted alltold me - No Surgery , your whole spine would have to be rebulit. With the conditions, I experience on and off Piriformis / Glute pain and terrible stiffness into the Thigh and into the leg (s). I have leg Length Discrepancy (1 inch shorter right leg) sustained form a fractured right femur in 1966 - at age 15 playing HS Football. Now at 71, that LLD has wreaked havoc with my well being. The LLD was left Unaddressed until the last 12 years or so with corrected shoes. it is Too late. Dextroscoliosis at 25 degrees is another consequence of the LLD. Acupuncture was of no real help (45 X), Chiropractor care helps a little. Had epidurals 2X - very little short lived help. And Episurals degrade all other connective tissue. What else can I do??

My pain happens if I’m standing too long , I got my pain in my SI area from tennis. Very interesting and helpful presentation!

This stuff always fascinates me. I've had a history of "crooked hips" and I've always managed to heal myself without the help of a medical professional, all because of these videos. I've learned that poor sleeping and sitting posture played a huge part in these issues. Recently I've been sleeping with a pillow between my knees and between my sides and it's corrected a lot of it along with proper workouts that target these areas. It's insane how intricate the mid section of the body is.

Wow! The scenario presented in this video seems to have considerable explanatory power for my situation. Some of my symptoms are consistent with piriformis syndrome but I’ve also had symptoms that suggest that the SI joint is involved. Thanks!

I have been in pain for most of my life ever since I fell off a 2 story railing at the apartment building my mom lived in. I was about 5 years old when I fell off the rail and I fell about 12 feet to the bottom floor hitting my left side of my pelvis on the concrete slab. I was scared of telling my mom and I tried to hide my pain as long as I could. One day I went to my grandparents house with my mom and my grandma noticed that I had a limp. She asked my mom why I was limping around and she said that she had no idea. So my grandma and mom took me to a Orthopedic doctor for children and they diagnosed me with Leg Perthis. After 50 years of severe pain in my left pelvic area and going to multiple doctors for the pain I finally decided to go to a chiropractor and he did some X-rays of my pelvis on the left side. He said that he had found the problem. My left side of my pelvis was turned to the back side and not be in line with the other side. He said that the Sacrilaliac Joint was misaligned and pinching the nerves. I told him to Pop it back in place and he said that he could do that but I will experience so much pain that I may want to go back to the hospital and get a surgeon to fuse the sacrilaliac joint. He said that he put it back in place and then told me to stand up. When I did this - it popped out again. I had no limp for the first time in 50 years but it popped back out of place. That's why he told me to call the surgeon and get him to fuse the sacrilaliac joint in alignment with the other side. He had to adjust my mid and upper back multiple times because from the limping around and leaning forward all those years my upper back was bent forward so much that I stood like a 90 year old man with his upper back bent forward. I told him to adjust my upper back and as far down as he could - trying not to agrivate the 3 low back fusions I have had done. When I went to the surgeon that worked on my lower back and fused the 4 vertabre in my lower spine - he kept saying that he needs to go further up my back and fuse the vertabre above the previous surgeries. When the chiropractor found the issue was the Sacrilaliac Joint he told me that unless they fuse the sacrilaliac joint in line with the other side that all the fusions they do will not get rid of the pain. The chiropractor told me to get back in the surgeons office and have them fuse the sacrilaliac joint and he would be happy to keep my vertabre in my upper back moving and straight. I'm going to see the surgeon on the 24th of January. I can't wait to be able to stand up straight and walk not bent forward and have less pain in my body. The chiropractor told me that ( all the nerves in the lower back and along the pelvis on my left side are connected to my pain. And unless they can stabilize the joint and move the nerves away from the joint I would continue to suffer. I've been watching the videos of the sacrilaliac joint fusion and it is not as severe as a back fusion. It looks like a small cut about 1/12 inches long to fuse the sacrilaliac joint. I've got 3 scar's on my lower back from the fusions and it didn't want to heal properly for some reason. It's been 6 years since I got the first fusion in my lower back and enough is enough.

Excellent thank you Dr. My pain has been three months now, injured and reinjured lifting an ALS patient much heavier than me. I've done every exercise in the book with little relief. Finally made an appt to see my doctor. I am convinced it is rt side SI joint pain. Hopefully not a disc bulge though it could be.

A few weeks ago I was in the ER with severe hip pain. It was actually a bit more outside than where the SI pain showed above, but it did not radiate. Doc diagnosed me with sciatica with no tests at all. I'm thinking it's actually SI Joint now. Makes a difference in treatment.

When I visited my mother in Iran, for couples of month ago I've got an MRI scan. The conclusion was: - Thoracolumbar spondylosis. - Herniation disc at T11/T12, T12/L1. - Extrusion disc herniation at L1/L2(left paracentral)&on L2/L3 central. - Canal stenosis. I’m having these exact symptoms. I’ve been dealing with it for 3 years haven’t been able to keep a job due to pain. What if the SI joint injection only gave me relief for 8 days what does that mean?

Great explanation, thank you! My GP sent me to do an x-ray which I’m sure that it won’t show anything as the x-ray doesn’t show details. I’m still waiting for the results. I have been diagnosed with AS as well years ago, and uveitis. Maybe SI joint has caused me sciatica as well? I’m in excruciating pain, like throbbing pain and can’t walk , I’m using crutches and only can do pigeon steps.

I had sciatica . But in one I was normal in morning . But starting to feel the pain early afternoon but it affected the spine and left leg . I couldn’t stand or walk . But I fell on a very hard flooring . But didn’t know that I pinched a sciatica nerve

What if if i have chrinic back pain with l5s1 lumbar disc protrusion, i have made ct scand and more than 3 mri all clear on si joint and injection? Recently a docotor sugested fusion of l5-s1 due to a lumbar flexion and extension test. I repeated the xray test in another place with no abonomeralities. I am stuck in the pain daily wirh no response.

They usually give you gabapentin or Lyrica. I tried the gabapentin it doesn't work for me.

Hello I have elbow pain and it hurts when I do anything and I do gymnastics so I tried a round off and it hurt so much and I have liver disease so I tried a medicine and I had an allergic reaction to it so it causes permanent damage do you know anything that could help?
