This is really amazing. 10 years with SIBO and world renowed specialists who were surprised something did not work for me that worked for others and insisted I keep taking it even though was not working. I've been taking iberogast...and it has never worked for me. Thanks a lot...I'm almost at the end of my sibo treatment, but this time I am doing it on my own. This month is prokinetic testing :)....

Wish there were more physicians like you. Too many health care providers refuse to grow or listen or learn or change their opinions. It's really crazy to me and very disheartening as someone sick seeking guidance and help.

I agree, it's really disheartening. Honestly, I think that it's something of a human flaw, and it's not just physicians. People generally don't like being wrong or feeling inadequate. Physicians are also horribly over-worked and nearly all have some degree of burn-out, so they're not always in the best mental space to be receptive.

I didn't know it was known that motility can stop because of autoimmune disorders. I tell my doctors that everything stops in my digestion. It's as though sudden paralysis happens when I have a flare of joint muscle pain,and fatigue. They just tell me take fiber. I tell them the fiber can't move if my intestines aren't moving things along. And they don't know what to do

3:55 I love this commentary on the philosophical aspects of science. It’s so easy to mentally put on a lab coat and pattern match to symptoms. But we need to remember what we don’t know, and that we may have gaps in our knowledge of which we aren’t aware.

You are a revolutionary force…I know you must know this😉 otherwise your passion and focus would not be so well balanced with intellectual humility AND confidence. Your work is brilliant and beautifully powerfully nuanced. I’ve watched a gazillion such videos and no one is doing the work you are. Not to be dismissive of others because there is so much good happening amidst the carnage but humans could not be anymore nuanced and complex if they tried. You are on to it. There’s a lot going on. Time to leave the DarkAges of medicine behind lol. Thanks so much for what you do🙏🏼

I'm in a similar boat, i dont react to almost all food. Just wondered if you managed to improve your situation and or find and correct your root cause? I think my issue is a very leaky gut with a root cause of very poor motility coupled with vit d deficiency. So I'm focusing on motility and weekend mega doses of vit d (I even react to vit d supps, so don't take them during week). I have probably also developed dysbiosis from restricting my diet so much.

Great video as always! Thank you! Question for you: if you take the prokenetic during treatment, will the antimicrobials be as effective in the small bowel since they will be waved down faster than without taking a prokenetic?

That's a great question and I've wondered this myself. Would the antibiotic or herb start to help "awaken" and normalize the MMC as the bacterial load decreases?

I am waiting to be tested for sibo h pylori etc. I am taking acids with meals and digestive enzymes to. I was just recomended the integrative theraputics motility activator as well as iberogast. Is it at all helpfull to do these things before starting the herbal and or antibiotic treatments. I havent been passing stool very little and usually quite lose and small although I eat only breakfast and lunch moderate to small portions. I was suggested to take both iberogast and the motility supplemnet. Does that sound ok for where im at.

If we take prokinetic (say Motilpro) between meals, then that would mean how much time after a meal or before starting next meal? And does it matter if we take it with little/full glass of water?

Great question! I just tell my patients to aim for approximately half way between lunch and dinner and the other dose at bedtime. And you don't have to take it with a lot of water, just drink as much as you like to wash it down :)

Oh! Of course! The ones I use are: MotilPro by Pure Encapsulations ProKine by Vita-Aid Motility Activator by Integrative Theraputics Acetyl-CH Active by Apex Energetics** SIBO-MMC by Priority One I don't have this one in the mix yet, but maybe I should: Iberogast **This one was NOT formulated as a prokinetic, but I personally think it has a weak to moderate prokinetic effect and have people try it out.

Which prokinetic works best on somone without a colon (ileostomy)? And is there any differing or unique advice that those folks with that sort of anatomy ought to be aware of when dealing w sibo, sans colon?

Thank God I found your channel I just purchased the moltipro candibactrim ar and br my question to you is how does this improve your sibo function?by just having you go to the bathroom reduces bloating? or does it actually destroy some of the bad bacteria. My symptom is stomach extension. Its brutal!

I'm glad you found my channel, too! Prokinetics don't (notably) kill bacteria. Rather, they help your body brush the bacteria down into the colon where they belong! This way you don't have to worry about killing the good bacteria in the colon :)

@@melissaulrich6511 did numerous treatments all failed. Started my last treatment today. This is first time I’m adding a biofilm disrupter. 3 days into it only. If this doesn’t work I’m done with it.

@@affordableaquariumservices1663 Have you also tried to do intermetting fasting? And have you tried finding the root cause of your SIBO? Dr. Berg explains this on his RU-vid channel. SIBO is horrible, I hope you are able to soon get rid of it for good.

@@melissaulrich6511 yes I’ve done intermittent fasting. I’ve had gastric bypass tests and other motility test. Came back normal. It’s the sibo. Too much bacteria in the small intestine. Bad luck. Root causes. I got food poisoned after eating spinach at subways. Lost 30 lbs on fasting but stomach bloat never went away. Full feeling and constant pressure. I’m very muscular person. Gym 6 days a week. Hopefully biofilm buster on new regimen kills this bacteria.

Definitely both! I recommend "normal" SIBOers take prokinetics for at least 6 months after you're done treating SIBO. For folks with known PI-IBS (anti-vinculin antibodies) the current recommendation per Dr. Pimental is that you take a prokinetic for a minimum of 5 years.

@@affordableaquariumservices1663 Oh NO!!! You poor thing! Prokinetics and probiotics would certainly be safe. Antimicrobial herbs and antibiotics should be used with extreme caution from here on out because of the C diff. Even herbal antimicrobials carry a risk of killing some good bacteria, so for you it may not be worth that risk.

Hm. I'm not sure, honestly! I'm not an expert on esophageal motility (yet ;)). My gut instinct says yes, though, since the esophagus is innervated by the Vagus nerve just like the stomach and the small intestine. When should the prokinetic agents be taken? I have read they can be taken in between meals but wouldn't that disrupt the mmc since it is suggested to wait 4-5 hours between meals?

Yes- between meals. Most recommend between lunch and dinner and between dinner and bedtime. I think the point is to take them when your MMC is starting to get goin'. If you take them by themselves (no food/calories) the pills alone shouldn't disrupt the MMC.

@@MrH1T Funny you should ask this! I've had a few people (including myself haha) who absolutely can NOT remember to take the mid-afternoon dose, so we decided to play around with a double dose at bedtime. That seems to work fine for everyone who's tried it.

Thank you great video. How does taking Prokinetics several months after antimicrobial cure SIBO for good? I thought when you stop the Prokinetics, then the small intestinal MMC returns to baseline ? So you have to take Prokinetics lifelong. Thank you

Not necessarily! I'll share my thoughts and my "gut feeling" but do keep in mind that this is still a very new field of study. Heck, our understanding of SIBO as a whole is still in it's infancy. Dysmotility results from some sort of damage (ex. autoimmunity/PI-IBS) or signaling misfire (ex. less acidic stomach contents --> reflex in the small intestine not triggered adequately). I see the prokinetic as a way to stimulate motility until your normal mechanisms are able to work the way they should. For PI-IBS, the current recommendation is 5 years, probably because it takes years to fulls build those nerves back to their original state again. In the case of other SIBO root causes (ex. low stomach acid), it will likely take less time to rehab those pathways.

How does Acetyl-Ch Active differ from Avipaxin from Neuroscience. The acetyl-ch is restricted access. I'd have to ask my Dr for it. Although I heard you say the prokinetic that seems to help most of your patients is Motility Activator. I just discovered your channel. I'm finding it helpful thank you. I was diagnosed with mold toxicity illness but I also have weird autoimmune things going on along with a constant 30-year or more struggle with digestive, constipated problems. I thought the only choices were osmotic laxatives or dulcolax
