Case study example for use in teaching, aiming to demonstrate how a behavioural experiment might be used in CBT for social anxiety disorder. The character of Hannah is played by an actor (Malak El-Gonemy), but the scene is not scripted, and as such it represents a natural therapeutic exchange.
The video was filmed and produced by Ishan Siddiqui and Christopher Werrett.

i was so disappointed when i saw that this was played by an actor :( but then i remembered that a person with social anxiety would never agree to be filmed during such a vulnerable moment.

I've had social anxiety my whole life, it's gotten to the point where I can't go to school and talk in front of my classmates, my parents have no idea what's going on, I try my best to hide it, but when it happens I get extremely red, I shake like crazy, I start crying and I can't stop myself. It's really embarrassing and I'd give anything to make it stop. However, in this video it's different when she talks with one person and it's rehearsed, she tried to control herself and they had been talking for a while before so she probably started feeling safe talking with her at least a little. If she was in a group of people or one person, it doesn't matter, I think she would have really had those symptoms, but only if it was more natural so the mind could really think it's in danger (fff response as you all probably know happens).

I'm 58 now and only found out I suffered from this about 10 years ago so I have been suffering all my life just thinking I was shy. It wasn't until I got a job and had to use the phone occasionally that I decided to do some research online about that particular issue. Whilst researching I came across Social Anxiety issues and a list to see if you suffered from any of the symptoms and I ticked most of the boxes. I could have cried when I realised thst I had this condition and that there was a name for it and there were thousands just like me.

@@athy_dostoy hi, I don't think it's something that ever leaves you but I have gotten a lot better as I gave got older and since I found out I have it I push myself out of my comfort zone and the more you do it the easier it gets, seriously, you have to push yourself. As an example, because I have no choice but to use the telephone now I find it fairly easy to make and answer calls. Another thing that seems to have helped me enormously is telling people I suffer from it if I get the opportunity and you'll be amazed when you open up a bit how many people are just like you but they don't realise they have it either. My HR Manager at work suffers from it too but you wouldn't know, it's only because I told her about it that she opened up to me. Be brave, it won't hurt you and when you do something you wouldn't normally do give yourself a pat on the back and do it again and again until it starts to get easier. Good luck.

I'm 54 and after having gotten out of a 20 year "friendzoning" on new years eve, had decided to try improve myself to not fall into a shitty relationship ever again. Tonight after having fallen into bad depression, considered calling the cops on myself because I started feeling suicidal. After a half hour of feeling hollow and despair I calmed enough to get on here by searching loneliness, depression and anxiety, hoping for answers why I am like this. 2 videos and I understand there's a name for what I am. I'd already started in the right direction by deciding to get out there out of my comfort zone and meet new and different people. Now I can ask a doctor who to see about it. I can remember wishing I was anybody but me way back in upper primary school, only getting worse in high school. I've always stewed on things and overthought, self doubting etc. Feel like a major revelation has just taken place and a great load has started lifting off me. Feeling somewhat positive especially after reading many other's comments.

For any body suffering with this problem. I have a few tips that might help VITAMINS: b6, b12, are d3 and magnesium are know to reduce sweating and increase mood in the brain and reduce anxiety and fish oil. SEROTONIN: consider taking htp 5 as it's a precursor to Serotonin ( a feel good supplement) that reduces anxiety. TEA: sage ,Chamomile, green tea (decafe), and valerian root are know as natural antiperspirants and calms anxiety EXERCISING: when you exercises it releases natural Serotonin levels as well as an endomorphin chemicals to the brain that creates physical relaxation as well as taking out stress (weight lifting, jogging) you name it. AVOID: sugars, spicy food, definitely caffeine as it's responsible for anxiety sweating. EAT: greens like spinach, or kale. Anything that's green, they are Rich in magnesium Or eat healthy in general. Also eat probiotics, anxiety can be caused by a bloated stomach, so considering adding probiotics to your diet. ANTIPERSPIRANTS: consider buying antiperspirants for the face or other parts that are of concern. Carpe is a really good product that I like to use. Here's the link mycarpe.com/ also no sweat scalp antiperspirant is a good product for those who want results fast hairtape.com/ Difference between both? Carpe reduces pores up to 70% after 4 weeks of consecutive use. as opposed to the other product, it will clogge your pores for 24hrs Both should be applied at night after a shower before bed. I really hope you find these tips useful, and don't give up i wish the very best to all of you people who suffer from the dreaded feeling🙏

great tips, although you should be careful with taking 5htp, especially long term. it can lead to serotonin syndrom (too much serotonin) which can be really damaging. i wouldn't take it without discussing it with a doctor as some people already have normal serotonin levels and taking a supplement as 5htp might lead to getting too much serotonin. if you do have low serotonin i would try to stop using it after a while to decrease the risk of serotonin syndrom :). it's a supplement that doesn't need a prescription (at least where i live) but that doesn't make it harmless. also, Odorex works really really well for anxiety sweating but that might be a European brand.

I absolutely hate my social anxiety. It gives me such a weak filter, which makes my dad look down on me because he doesn't understand what it's like to have a disorder like this. Having Social anxiety prevents me from doing things that I really wanna do. This is the reaosn I hate going to stores by myself. I couldn't speak up even if my life depended on it, and I hate it.. I'm always seen looking down When ever waking anywhere within a huge crowd. Because of this. I was never able to ask my teachers any important questions because I was afraid of looking dumb. Whenever this happens I sweat like crazy and mumble to myself. Even when I just tell someone not to worry about it, that even triggers my nerves even more. I can't help it, it really makes me uncomfortable.

I purchased a hidden camera to keep an eye on my children while I’m at work. I’m selling baby birds and forgot the camera was there so when I watched it to check on the kids, I seen how I acted . I cried! I’m here because I’m searching ways to control my anxiety and this was not at all how I acted. I was sweating so bad it was running down my face. Then... I made up stupid excuses as to why I started sweating profusely. The looks on their faces said it all when I left the room to get something. I swear people think I’m on drugs. It looks like I’m on drugs. I can’t control it no matter how hard I try.

I always had friends, but the more i grew up i became an outkast.. high school was horrible and adulthood even worse. The same exact panic attacks she described is what i get occasionally, if its bad’ i will get red hot, sweat and not be able to breathe. I never been confident and always worried about what people think of me. so im sure that is my biggest issue..i dont plan on getting help with meds, i know my day will come where I beat this anxiety

Absolutely true. I have SAD and currently I have myself a lot. I am constantly hearing those 'voices' when I get outside and they keep telling me that I am no worthy, or that I look bad, or that I'm terrible. And the worst thing is that I realize how absurd and unreal that is, but at the same time I can't shut those voices, so I get back to my safe place (my home) where they finally stop. I am trying to fight it now, I analyzed some of the reasons why I have it in the first place - bad memories from my childhood and teenage years, parents kind of neglecting me (not on purpose), I've been mocked, etc... My SAD caused me to gain weight and that is making my self-esteem go even lower. But I try. And I talked about it with my husband and he is very supportive and understanding. I also opened about the problem to my friend and she is also trying to help me.

I never graduated because of my social anxiety. I went to university, having new classmates, it was very uncomfortable around them. I just stop attending class and stop learning. Bought PS2 and go to my uncle house. He was unmarried and loved me so much. I could do everything in his house. My mom just thought I was going to the university regularly. I failed 2nd year. Next year, I repeated doing the same things as last year. Failed again. If I failed for the third time, the university would expel me. I thought I would be going to the class regularly in next year. But I repeated doing same things again for 3 years. When the final exam comes I know nothing what to answer. Coincidentally I got fever and diagnosed with TB in the second day of exam. I have my own excuse to tell other people that I got TB and didnt graduate. But I only know the true reason why I fail not only graduation but also the opportunities of my life.

The most challenging part of social anxiety for me tho (if you are interested) is when my hand shakes because i can't hide that and it's really annoying because people will start to notice and it's so awkward especially when you are with a group , it happens whenever i try to do something with my hands. Take my yoke upon you and learn from me, for I am gentle and humble in heart, and you will find rest for your souls. For my yoke is easy and my burden is light. Those are the words of Jesus. Also in the Bible is written in the books of Acts 16:31; Believe in the Lord Jesus, and you will be saved, you and your household. This is one of the few things that gets me through any venture involving people/going out. Remembering how temporary this life is and that the real deal is yet to come. If you listen to near death experiences involving Heaven people will often say how everything felt a thousand times more 'real' than this life. It's funny how it's the exact opposite to what most of us think of when we picture an afterlife. Also remembering that every one of us is a sinner, even all the bubbly confident people, helps me feel that little bit less alien. Don't get me wrong, I still struggle to the point of crying my eyes out some days, but then I try to remember these verses. Psalm 34:18 - The LORD is close to the brokenhearted and saves those who are crushed in spirit. James 1:2-4 - Consider it pure joy, my brothers and sisters, whenever you face trials of many kinds, because you know that the testing of your faith produces perseverance. Let perseverance finish its work so that you may be mature and complete, not lacking anything. It's hard, but remember that our entire reason for being here is to grow with God. He uses our personal weaknesses to push us to trust in him like a child trusts their parents. So try to remember that even in bad there is good. I hope this helps someone. Feel free to message me if you need a friend. I pretty much just have my husband and nobody else. Too anxious and imo ugly to befriend anyone else. I'd say I'm used to it but honestly the loneliness still hurts even after nearly 30 years like this.

I have been dealing with anxiety/social anxiety for a while. I don’t think looks really play a part on who deals with it and who doesn’t. A lot of times I think I come off as “stuck up” to others because I am so anxious and it’s hard for me to socialize. People think I think I that I am too good for them but it’s quite the opposite- I don’t think I am good enough or interesting enough for them to want to socialize with me and I am scared of being judged or coming off as boring or stupid. It really sucks- I hate coming off as stuck up because that is literally the opposite of what I really think!!! 😞

I hate to have social anxiety. I lose opportunities because I’m too scared to take risks. I’m scare ALL the time if there’s people watching, because I feel like they are judging me. I can’t walk normal in public, I feel that I need to walk some type of way, that all the eyes are on me, and not in a cocky way. I only have three closest friends, and I don’t like to spend a lot of time with other people. I don’t like big groups of people, I don’t feel comfortable. I get nervous when I have to talk in public, my heartbeat raises, my palms get sweaty, I feel like I’m gonna pass out. When I was getting into an online class the teacher asked me something and I knew the answer, but I was too scared to say it and for it to turn out wrong so what I did was to disconnected from the meeting, and later I gave the excuse that my internet failed. I’m not shy, I don’t consider myself shy, but I’m a nervous girl. I don’t want to talk about it with other people because, again, I would feel judge. Because I think they are going to say something like :” it’s okay, don’t be dramatic.” Or “this is normal” but I know me, and this is not “normal” because I feel this way always, I live with fear and I always try not to show it. Hope someone else can relate.

I've been considering CBT therapy for social anxiety. Let me preface this by saying that I think social anxiety can totally be improved and worked on and you can really turn things around if you put the work in. It can get better. I think a major trap a lot of people with social anxiety fall into is that they settle for less. They spend all of their time and energy on just getting by. They force themselves to do just enough to survive and for their loved ones to think they're doing okay or at least okay enough (they force themselves to do just enough to be able to pay the bills, do the shopping, and make necessary calls, but there's no energy left for things like meeting new people with similar hobbies, or dating, or going for that job interview). Just doing what you have to do to survive is so EXHAUSTING. It really blows my mind to think about the idea that most people don't think twice about these things. And they aren't miserable the whole time they're doing them. Getting through the day can feel like you're going to battle and possibly to your death. Every day. Sometimes I think, 'Is it supposed to be this HARD?' It really wears you down, but you're so focused on just getting through the moment that you don't realize how tired you are. And then one day ten years has passed and you realize you've spent a decade putting all of your energy into just getting by. Energy that could have gone toward bigger dreams and more fun and feelings that actually feel good. That's really sad and I guess not how life is supposed to be. I guess my point is that I think therapy would probably help a lot, but a lot of people with social anxiety may not consider it because they've been getting by. But life isn't supposed to be miserable the whole time.

CBT is a great tool but there is a simpler solution, just through a better understanding of how the "social anxiety" feeling is created within your own mind, and how your reaction to it is often worse than the feeling itself. It's funny how the psychologist was suggesting that the girl's anxious bodily sensations weren't as obvious to others as she thought and that's its all just in her head, but for me mine is obvious because when you hear people say to you "Hey, are you okay?" after you turned really red, or "are you nervous?" after you try to make conversation, then you resort to just keeping to yourself so you don't have to feel like you're fuckin weird in every social situation, and even with that you can't win because they say to you "you're too quiet".... Then you just get tired of even trying anymore...

Maybe the problem is, everyone is so fake and it feels so forced trying to have conversations with random people that it does make you feel uncomfortable because it’s just not how you role. Some people are born into this world as fake ass MF, others are just real to the core, hard for other to be fake when a real person enters the room, maybe that’s the reason you feel awkward because people have to take their masks off. Keep being you, surround yourself with understanding people and be proud of who you are. So long as you are trying to be the best you can be each day, that’s all you can do. Don’t beat yourself up about your past, live in the moment, learn to die while you are still alive so you can truly live without the fear of death, then you can start to become fearless. Stay strong

I’m honestly not sure if I have social anxiety disorder. I love to socialize with friends and family but whenever I’m alone I start looking back on the conversations I’ve had with others no matter whoever they may be, and I start overthinking about the smallest things I do or say in the conversations and I get overwhelmingly anxious. Anxious about if I made someone not like me as much or how do they really feel about me. That anxiousness causes a shit ton of self esteem issues and I start to hate myself for not doing as good. After that although I don’t get anxious or shy really to socialize with other people, my conscious tells me “Yo, you need to do really good or they’ll hate you. If you fuck up they won’t like you anymore. They’ll want to stop hanging around with you.” And it gives me a ton of pressure so then whenever I inevitably make a mistake no matter how small, I start to panic in the back of my mind and it takes everything in me to hold myself back from flat out crying sometimes after making even the smallest mistake in a conversation. I’ve turned to porn, youtube, video games, anime, manga, etc and although they’ve helped me a lot it’s really not enough to keep me from hating myself and worrying an absurd amount about my relationships with friends and family. I’m thinking about seeing a physiatrist but I’m really nervous about the idea of getting my parents and other family involved in my own problems and I don’t wanna worry them any. Wasn’t really expecting to make such a long comment, just wanted to get this off my chest lol.

A LOT of people have the same thoughts about past conversations like you do and feel bad about that, but that Is NOT social Anxiety disorder, trust me I have been experiencing it for almost 5 years and it's much worse than you think, the lady in the video portrayed it really well.

Hello, I know this comment is a year old and I don't know if you already solved this problem of yours, but I'd still like to say something. I went to my school's counselor after suffering for who-knows-how-long, and she helped me realize that even if you think that your problems aren't related to a condition, it doesn't mean that they are less important and shouldn't be taken care of and looked through. She also taught me that if your problems affect your life and keep you from enjoying "normal" moments then it would be best to ask for help. Don't be afraid to tell it to your parents or guardians, they only want what's best for you and they want you to feel alright. This happened to me just a few months ago, I cried a lot, but in the end not only it helped me get my burdens off of my chest, but it also brought me closer to my family and friends. Now they understand why I act a certain way and they try not to push me around too much. I'm sure your parents will understand too. Just think that this will better your future! The earlier you learn to fight these thoughts and and impulses, the earlier you'll be free from everything that's tearing you down. Good luck! I hope this helped (sorry if it's too long!!).

My defense mechanisms in the moment are similar to hers. But when I get home from work, is where the real defense comes in and where the real destruction is. I use porn, masturbation, tv/RU-vid, isolation as methods to escape the world and get as far away from the awkward, insecure, social anxiety memories from the week of work. And I spend the weekend running and hiding, dreading Monday. Any suggestions?

11219tt try to focus on the present. I believe there are many cures to social anxiety. You just basically have to fix what you fear. Many people who have social anxiety have different fears.

That's exactly how I cope with my social phobia as well. Everything you just said but I go as far as using drugs as well because I just HAVE to escape. I've been addicted to benzos for a long time, (Xanax, Ativan.) And even do fentanyl. It's sad. I feel so bad for myself and anyone who deals with this on such an extreme level as me.

I suffer from social anxiety since i was a child; no friendship, no love, it's not funny. I have many interests, and this makes loneliness more bearable, but I'm sad for all the opportunities I loose, and all the love and good things I could give to others but i don't find the right way. I'm my own prison; i hope i'll find a way to run away someday and start enjoying life.

Wow!!! I really liked this. The way I’d put it is that you used the “scientific method” to determine whether your clients notions of her behavior and her impact on others was accurate. Overall, I consider this intervention to be SUPET-effective. You moved right into the heart of your clients world-view and challenged her without provoking her defensiveness. Really, really good stuff. I’ll be thinking of this video the next time I’m working with a client who’s got an unrealistically negative view.

I think one of main reasons of social anxiety is that constant fear that people might judge you or find your insecurities. But let me tell you that ignore those fears and don’t worry about that what if they judge or find your insecurities. You truly know yourself, you know your abilities. You have to believe in yourself.

you cannot "ignore" mental illness. if people with SAD could just ignore their anxieties, then well we wouldn't have the disorder in the first place !

Eventhough I learned not to care I still get angry when someone sympathizes with me or even worse look down on me. What a shit disorder

I think I have social anxiety (and maybe depression)? Ok so I wrote a whole lot before, but it got deleted which is annoying but: I’ve tried to tell my mum about this once, whilst nearly tearing up (it took me a lot of courage to say that), but she literally told me that I’d get over it and I can’t be like that. Like wtf?? And thing is, I’ve been like this since the start of high school (11), so it’s been around 5 years now. So no, I still haven’t gotten over it. I often get nauseous, my heart would thump really fast or experience heart palpitations(?) like really bad. Even the thought of having to talk to someone, gives me the feeling of my heart dropping to my stomach. And it doesn’t even matter if it’s in 4 hours or 2+ days. I’d also often avoid eye contact and use any excuse to look at something else (for example if something dropped), to get my eyes on that instead. When I would be told by the teacher, to talk in front of the class, I sometimes hear sniggering. That just lowered my self esteem a lot. Especially one time, when I heard a teacher telling them off for doing so, I know they mean good, but it made it a lot worse for it just proves how clear the sniggering was. Another time, a teacher told me off really loud because I had another subject’s work on my table (I wasn’t even looking at it, I was looking at the screen) and was shouted at, to put my work on her desk. As I did, people were LITERALLY SNIGGERING SO LOUD and it was the worse day for me. I silently cried during the rest of the lesson, but put effort to not show the tears, as I didn’t want to attract attention or seem selfish. The teacher had anxiety herself, so it was just awful how she did not imagine if she were me in that position, and how anxious I was when walking to her desk and back. (P.S she was also very bad at teaching imo.) All of these type of negative situations literally made my self esteem worse over the years, therefore making me extremely lazy/not being bothered with anything (maybe why I would have symptoms of depression too?) and constantly paranoid about my looks (thinking that people are always looking at me and judging me for my appearance even when they’re probably not), despite not taking care of myself and dressing nicely. There are so many other things I could talk about, but this is just a few things that have bothered me… And btw, I’m able to say this out here because nobody knows my real identity, otherwise I don’t think I would be able to openly say this with anyone. I want to get diagnosed, but I don’t have the courage to tell my parents about SAD and if they were to book an appointment, I don’t know if I could handle talking to a complete stranger about my personal issues, due to obviously, my suspecting Social anxiety. I’d also probably cry which is also embarrassing. 🙁 Can anyone give me advise on how to get this diagnosed of someone please?? I don’t want to keep assuming. I want to get it diagnosed and get the closure I need, also being able to have medical proof to excuse me in certain situations. I apologise for this annoying long comment, but I’ve been bothered about this years. Thank you for reading…

I appreciate all the comments here it’s all very supportive. Mine has been triggered by traumatic life events and all these years has improved gradually but I still suffered from it. I turned to alcohol and that has come to a pint where I’m hitting a bottom while at the same time having a rebirth. I have many alcoholic traits that have brought me to realize that the drinking is itself and addiction now but it has also come from unresolved childhood and youthful wounds. I know that the more I’m zooming in on my healing and the deeper I can look and heal the greater I can be my authentic self. Being comfortable in my own skin is all I’ve ever wanted my whole life. I’ve never had that. I asked God to give me this and lead me to it and I will let go of the alcohol. I can see that he is working this miracle in my life. We can all find healing and a path out of the darkness of anxiety disorders. When I was a teen there was nothing out there to help. Now the internet is full of help and there are so many people talking about it. I prayed for a day when this would happen and I’m grateful to be alive to see it

Thank you for your great videos. However, I'm not sure how realistic it is the recording and interaction with you with a camera in your office, since she's not really in her job setting. So, is this really a good sample to disproof some of her false beliefs? What I mean: She will probably act different in your office than she would in her work setting, since she already feels comfortable with you etc. Thanks for the reply in advance, and keep up with the great work.

Yes thank you! I already commented on this (see above) but I couldn't describe what I meant quite as articulately as you just did! Thank you! The role-playing is a little silly and pointless as like you said, she's obviously not going to act the same way in her therapist's office and at an appointment as she would in her natural work environment out there in the real world! That's the only thing that kinda annoys me about this. Like how is this going to help? The fact that therapists think that this type of thing is just going to cure or even mildly help someone is beyond me. It's like repeatedly throwing someone afraid of fire into a fire and saying after 6 months, "There! Are you cured now?". I don't believe this type of therapy (or any of the current therapies) even works (well not for me at least and I've had this my whole life since a child and had countless therapists/doctors/different types of therapy) and it only made it worse and exacerbated it... if any thing at all. I wish there were more therapeutic options for this debilitating illness. This can't be the only way!

Mine one is a rare social anxiety - I hate my same aged people. Bcoz of that I have literally 0 friends. I find them too much more energetic and active than me. My mind always thinks I won't fit among those boys and end up in failure and they will laugh at me. I hope I could find some boys like those in comment section in my locality who can understand the social anxiety difficulties to befriend with. 😢 Every boys seems like they already engaged in a friendship zone and no extra person they need

I'm anxious that people would see me as stupid that's why I tend to be quiet. Everytime I go out I need to have a script incase I need to talk to someone. If something unexpected happens, my mind goes blank and I don't have anything to say. My mind is slow, it really needs time to process every information.
