This form is the SSA-3369-BK. Go this particular, official SSA web page: www.ssa.gov/forms/ ; -and scroll down to: SSA-3368-BK Work History Report.

I got approve in May of 2020, my first check won't come until December of 2020. They told me I won't get back pay because in a discrepancy on when I was actually disabled, but would tell what the discrepancy was, how can I get my back pay.

Another big thing is the supervisor questions on the form.

I worked and pay ssa like 10 years and they put me on ssi im blind why no ssdi?
@ticks4ticks4

+j p : You wrote: "I worked and pay ssa like 10 years and they put me on ssi im blind why no ssdi? " My answer: This depends; is on a "case-by-case basis." This is tricking one to answer without knowing your circumstances. ...(con'td)...
@ticks4ticks4
@ticks4ticks4 3 года назад
+j p: (..cont'd...) The answer to this may depend on your "EOD" ["Established Onset Date"] that you became disabled or blind "according to the SSA's rules." (...cont'd...).
@ticks4ticks4
@ticks4ticks4 3 года назад
+j p: (...cont'd...) Now, aside from the above: NOTE: The date at which you apply, or even initial an application for SSA disability benefits, you are establishing a "[protected filing date." The DLI ["Date Last Insured"]-if any-from your filing date-and your age; determine whether SSDI adult SSA disability benefits are even a possibility (as opposed to SSI only adult SSA benefits). Also, you may have worked in particular jobs-or received earnings from particular jobs-exempt from qualifying for "work credits" that could never be considered-according to SSA regulations/laws-for qualification for SSDI adult disability benefits. (..cont'd...)...
@ticks4ticks4
@ticks4ticks4 3 года назад
+j p: (...cont'd...) Now, you say that you are blind. Were you found disabled or blind under the SSA's rule according to your "blindness" only?? Or was the approve for severe vision impairments along with other impairment(s); perhaps along with age/education/work experience rules; & now your vision has deteriorated to the point of blindness? Or the same, but your blindness is corrected only marginally with vision devices that you were find disabled on the "vision impairment" and (an/other) impairment(s)-possibly combined with age/education/work experience? (...cont'd...)
@ticks4ticks4
@ticks4ticks4 3 года назад
+j p: (...cont'd...) Regardless, the adjudicators the state-level DDS (Disability Determination Services)-or the ALJ (Administration Law Judge) determined that you "EOD" (established onset date) was either: one at which you did not have enough "qualifying/eligible work credits based on your age and DLI ("Date Late Insured"). Note that the "EOD" ("Established Onset Date") of disability and/or blindness may be different from your "AOD" ("Alleged Onset Date"). Hope this info is helpful to you!
