basically theres this movie called human centipede where a doctor guy kidnaps three people and stitches them together by the ass and mouth( i wouldn't watch it,it was traumatizing) but yeah the episode is about kyle going through that and having his mouth stitched to a guys butt, so thats how he got the mouth scars.
