Here is a 2007 footage at the NOW Remodeled Chuck e cheese in San Jose, CA back when they had the alpha, a lot of phase 3 art, and phase 4. As you can see they also have the blue kid check sign back then even tho' all chuck e cheese's had that back then.
This location has remodel to 2.0 back in late 2018 and there alpha is no longer there anymore
