What if the patient exhibits all the symptoms except the bowel movements are in reverse mode. The beginning of the bowel movement is dry and hard and the end of the bowel movement is wet and diarrhea and is accompanied by post bm palpitations along with achy pain in both testicles and burning pain in left flank.

I was diagnosed with SMAS in March 2020 (right when lockdown for COVID) I was hospitalized for a month and sent home with a surgically placed Peg/J tube. I had an NG Tube for a week. This syndrome is horrifying and scary. I weighed 85 lbs (at 5”8’) and was slowly dying until I was diagnosed and treated for weight gain. I went 100+ days without eating or drinking (obly fed through tube) Now after 2 years it’s been a year since I got my tube taken out and I’ve gained a lot of weight (I weigh 160 lbs) and live a healthy life.

My daughter has less than 22 degrees and 4mm and left renal vein compression noted on her scans but we cannot find anyone who is willing to take it seriously (since her CT shows normal emptying). She is in constant pain. Unable to eat solids for over a year. They have put her on nutritional support and it isn't working. If anyone knows a doctor in Saskatchewan that knows anything about this please help. All they will give her is psych meds for a functional diagnosis which are compounding the weight loss and worsening her symptoms.

@@carrieklassen2389 If her ct shows normal emptying, there is no active compression indicative of smas. A ct artery angle is not always indicative of a compression. There are many people with a narrow angle who don't have smas at all. It is not until a barium swallow fluoroscopy shows even delay of liquid that they know it is affecting the emptying. Have you checked for mals? I am in Calgary myself and I know not many people know of it. I was diagnosed in January. Maybe try a doctor in Calgary! I'm so sorry she is going through this. I would try to advocate for a nasal jejunal tube. It is non invasive and it will show if she can gain weight that way and elleviate symptoms. They are less likely to opt for an abdominal feeding tube if they can't actually find a compression.

@@carrieklassen2389 Google Dr Thomas Scholbach. We have an appointment with him in July (in Germany). Everything has to be checked. There are several vein compression syndromes. Nutcracker, MALS, May thurber syndrome, pelvic congestion (think it's called that). But if you look at his site, you'll get ideas. I hope you get this message cause I probably won't see reply

I've heard a story about this before. A woman was complaining about unspecified abdominal pain. Workups were unrevealing. Somewhere down the line, a worm doctor took over the case. Maybe a worm was causing it, I'm not sure. But the clue to the diagnosis SMA was the patient said that the pain felt a lot better when she was doing yoga, specifically that downward facing dog pose. The problem I hear about definitively diagnosing this is it takes a very specific angiography.

@Brandan yeah only a CT angiography (i would say with contrast material) is the most sure way to diagnose SMA syndrome, even if there are other like for example sometimes even a CT scan could diagnose it, or if you do an x ray test of the small bowel with swallowing barium and the barium get locked or his passage is very slow trough the duodenum, or for example if during an examination of the upper small bowel, if the endoscopy can't pass the third portion of the duodenum. But yeah, the CT angiography with contrast material is the less invasive and the most efficient.

@@mike0133a barium swallow or manometry with impedance is standard for achalasia. it makes sense that a barium test would show smas as well.

@@aaravaaruu6813 i was diagnosed with a CT with contrast so I had to drink a liquid solution before going in. The treatment has just been liquid nutrition and soft foods. They told me a feeding tube or surgery is the last option.

It took almost dying from malnutrition twice over a year and 8 months to discover I am suffering from SMAS. At my lowest weight of 108lbs, I was put on a NJ feeding tube, but still we were unsure what was causing my wasting away. What delayed the diagnosis? Marijuana is legal in my state and every doctor wanted to write off my symptoms as CHS. Thankfully a doctor 6 hours away from my home was too smart to fall for the easiest answer. While I did quit smoking marijuana, it was a CT scan that confirmed the SMAS diagnosis. Quitting marijuana was never going to be enough to save my life. I have since been put on Cyproheptadine, in an attempt to gain an appetite. I was told I need to gain 20lbs. I am struggling to eat more than 1,500 calories a day, when I personally need a minimum of 2,000 calories a day to recover. I am trying very hard to get through this for the sake of my young daughter. As I am very depressed after my diagnosis, I appreciated the humor in this educational video. If anyone reading this also has the condition; please let me know how you are doing. 🙏

@@Fomites my duodenum was being compressed by the lack of a fat pad between the artery and organ which caused my appetite to be nonexistent. Dyspepsia prolonged the lack of appetite because whenever I did try and eat to regain fat, I would feel pain and nausea.

I have SMA sydrome found on ct scan with contrast after losing going from 130 pounds to 109 pounds in a year of constant nausea last week I have gone down from 116 pounds to 109 pounds and constant nausea and agonizing pain in lower abdomen. My condition is so bad I got another diagnosis on top of everything which is chronic fatigue syndrome and it’s not my fault because I have been averaging 2700-3000 calories a day for the last 4 years and haven’t had a problem until a year ago. I have been forced not to eat lunch since nausea peaks at this time so I’m stuck eating a 1,500 calorie breakfast and a 1,500 calorie dinner and nearly dying of dehydration even though I’m consuming 5 liters of liquid a day.

I have sma syndrome... varicose veins in my leg is due to vascular compression ... the sma causes my veins to work sluggish because of the compression at my l4 ... nutcracker also adds to the issue ...

My question is actually addressing the anatomic & the hemodynamic rationales for developing varicose veins in the superficial venous system of the lower limbs in case of SMA syndrome ! The superficial venous system of the lower limbs ultimately drains its blood bilaterally in the femoral veins ==> external iliac veins ==> common iliac veins ==> inferior vena cava ==> heart . The superior mesenteric artery (SMA) & its branches along with the blood flow through them does NOT interfere ANATOMICALLY or CIRCULATORY wise with the aforementioned venous return pathway from the lower limbs at any point ! I posted that question for a while but I did not get a scientific answer from Medicosis or his patrons so far !

I am thankful more doctors are learning about this syndrome. its hard to get treatment when no one knows how to treat.

Hello, my name is Osama Ayoub. I had many examinations and passed 9 doctors to diagnose my condition. Only one doctor was known. I was diagnosed with mesenteric syndrome, and I underwent an operation. A month after the operation, my condition worsened until I vomited food and could not bear the pain. WOW I was diagnosed with Nutcracker Syndrome in a CT scan. I was just looking for information about this condition and where to find a good vascular surgeon. Unfortunately, in my country (POLAND) there is little information about this condition. I would add that I am now very thin due to problems with heart arrhythmia and a diet limiting sugar and unhealthy food. Is there a chance to gain a healthy weight and to put some fat there so that I do not have to have an operation and by the way do not harm myself with a diet?

I have been having chronic cramping feeling when I run since march 2020. It is right under my lower ribs, on my sides, and above my pelvis. I also cramp when I walk too much or sometimes when I'm not doing anything at all. Tests showed I have sma syndrome but the symptoms dont line up because I do not vomit or feel nausea. I also do not feel pain in my left testicle or anything like that. But I have been constipated lately, and the pain is extremely weird. It seems like a side stitch but nothing that normally helps a side stitch helps at all. I have been trying to gain weight but eating large meals makes me feel lots of pain. Please help me

Look into Median Arcuate Ligament Syndrome “MALS”. Here in the US, Dr. Hsu in Conneticut is the best Vascular surgeon in this area.

@@Spidermonkey260 Symptoms can fit many diseases or no disease at all. That's why it takes a lifetime of study to be a doctor. The danger of relying on online comments is that you can either get hurt from the wrong advice from a doofus, or have false hope that what you're experiencing can be cured or identified for that matter. If you've seen multiple doctors/specialists it might be time to find consolidation in what all of them have told you, rather then search the comment sections.

Sir. Am digonsed with SMA SYNDROME 1 week ago by abdominal ct scan .I have this problem from 2022 . in 2022 i was suffering Stomach Lymph node T.B and i lost my weight between 15-20 KG then my T.B IS cured.after that the smas symptoms are appeared. And my symptoms are . early satiety when i eat or drink something my stomach full very quickly and after eating and also i feel abdominal discomfort sometime. if i also eat my favorite or very tasty food even then i feel full very quickly . is this desiese very dengerous ?

You need to eat samll meals frequently and eat High calories food that makes you gain weight and helps to recover from sma symdrone. And my doctor also gave me some Medicine .

my sma started after rapid weight loss due to cancer ... others in my support group have either had bad scoliosis and its compressing now ... and some its genetic in body with the superior mesenteric is to short. I am no expert just looking for pain relief and found this ... I hope it helped

Gastroparesis here too. Have you had a CT scan? That's going to tell you what you need to know! Currently going through getting referrals for surgery. Seems rare to not find blood in the urinalyses for nutcracker though.
