I have so many comments about this list! 💜 1. The rhinoplasty that comes with FFS isn't like a regular nose job. The general intent is to shave down the bridge of the nose to match the work they do to shave down the brow, because just as testosterone causes extra bone to develop in the forehead and brow (and chin, jaw, and eye sockets), it causes extra cartilage to form on the bridge of the nose and trachea, and extra skin in the forehead (to accommodate the brow growth, forcing the hairline back and the eyebrows to sag lower) and upper lip (room for the moustache, turning the upper lip down and making the mouth look thinner). 2. People getting a brow & forehead reduction will often opt to have bone shaved off the sockets around their eyes (orbital reduction), as testosterone makes that bone increase too. It can have a profound effect to help 'open up the eyes' (the most masculinizing feature of a face is the extra shadow around the eyes, produced by protruding brows and deep eye sockets and over-developed nose bridges). 3. An upper lip reduction generally removes about a 5-6 mm sliver of tissue from under the base of the nose, simulating how the lip would have been without a testosterone puberty. That's how the lip gets 'turned up'. Without that extra skin, the lip no longer turns down, and you won't have that very thin male-looking upper lip any more: just a more open, more feminine smile, without any fillers or plumpers. 4. Warning: most cosmetic surgeons out there lack the training or equipment to perform real FFS surgeries, so they market a lot of 'alternatives' to try and get money out of a very desperate and vulnerable market (that's us). They're the ones pushing fillers and injections. I highly urge everyone to learn as much as they can before getting any procedure, and never settle for the one you can afford: wait for the one you NEED. 5. About orchiectomy: one major reason for us to get it is to remove a lot of volume from the 'package' down there. It makes tucking easier, more comfortable, and with less of a 'bulge'. It's also cheap, as opposed to SRS which is very expensive. Many girls will opt for an orchi to give them years of relief before they can actually afford (or just make up their mind about) a vaginoplasty. SRS requires a couple years of electrolysis to prepare the donor area ($$$), airfare and hotel stay for a couple weeks for you and your helper ($$$), dilators and whatnot ($$$), and even if your surgery is 'covered', it isn't covering that other stuff (so we may be up to ten thousand dollars by this point, depending on circumstances), and it isn't covering revisions. Just about everyone getting SRS will need to go back for revisions at least once in their lives (so, $$$ for that, and the airfare and hotel again each time). Where I live, a vaginoplasty is covered by healthcare: but revisions and all other expenses are not. So a 'free' surgery could end up costing me as much as FFS in the end. Orchiectomy doesn't come with all that extra cost and hassle, but it can relieve a whole lot of issues. If you've never had a 'dip' in your hormone therapy, where you start to experience re-masculinizing effects... trust me, it makes you want those little things removed as fast as possible! 6. Orchi addendum: whatever procedure it's part of, removing the testicles tends to provoke the adrenal glands into producing extra testosterone for a while out of backlash. Just something to watch out for and know that it's not permanent, but anyone telling you that you can lower your estrogen or stop your anti-androgen drugs right afterwards is giving you bad advice. But your adrenal glands will still produce a small amount of testosterone for the rest of your life once that initial surge dies down: enough to be healthy. 7. Alopecia reduction (or scalp reduction) is another option as part of a hair restoration plan. Since a person has a limited amount of donor follicles for use in transplant, some of us are left in a position where there just isn't enough possible donor hair to affect a full-ish restoration. So a girl stuck as a Norwood 5 or 6 might consider a surgery to remove part of her bald spot, before proceeding to hair transplantation. It's certainly not an ideal solution, as it will leave a large scar down the middle of the scalp, but a girl's got to do what a girl's got to do!

a really good summary! my favorite was the brow lift and contouring. it also lowered my hairline by over and inch! my least favorite- electrolysis. it is sooooo slow and the most painful. i am having to use it to clean up what laser can’t get (blonde and gray). i still have my chin to do. the worst part for me is not the pain or the cost (high) but having to grow out the hair enough to remove. it triggers my dysphoria hardcore to have whiskers 🤮

Thank you for such a helpful and informative list. I'm finally at a stage where I can start saving for surgeries, I'm so worried about the cost, but I'm hoping to have FFS soon. I've got my consultation appointment in Nov. I don't think you need any fillers. Your lips are lovely. Have you heard of SKN clinics? They regularly have up to 50% off full body laser treatment plans, and they offer some finance deals. It's who I used to remove my body hair. Thanks for the fab content, gorgeous lady. ❤

Here in Canada vaginoplasty does not need to have hair removal ahead of time. The hair is permanently removed during the surgery so that takes care of that. (I'd check with NHS on that procedure as it may be the same.) Cheers.

GRS Montreal offers that surgery-day cauterization, but not everywhere does. And they warn you that any hairs that have been reduced (but not fully eliminated) prior to surgery may well be too thin and small to be seen by the doctor to be cauterized by him. But also, there's a chance that any hair that's lighter & thinner for any other reason (like it just started growing, or you have poor circulation in that area, or it's been temporarily miniaturized due to stress or medication reaction, or you're just an older girl whose body hair is thinning and going gray too) will be missed anyway and then you'll have hair growing inside your vagina down the road. So it's a great option, but not a 100% guaranteed process. If you've already had some hair reduction treatment down there, you absolutely want to finish that off anyway. And if you want to avoid the anxiety of the whole wait-and-see if they did get every single one or if something escaped the doctor's eye: then it's still a good idea to get it all removed in advance so you have the time and luxury of witnessing it not grow back any more. On the other hand, getting it done surgery day sure speeds up the process, and lets you skip a couple years of electrolysis!

Orchiectomy is also used in one of the treatments for prostate cancer, something that as affected other members of my family, by removing the testicles it reduces the production of testosterone, which in effect helps to stop the prostate cancer from growing/spreading. Something for me to keep an eye on in the future, unfortunately. Oh, hi Malibu 🐦

You can also have a glottoplasty to raise the pitch of your voice to sound more fem. There is a surgeon in London who will do this operation. Costs about £5600 the last time I looked. And for $40,000 there is a surgeon in California who will do shoulder reduction to make the shoulders narrower.

Peritoneal is the internal layer of skin that creates moisture. It technically can be pulled from anywhere from the body, not necessarily always from the Abdomen, but some surgeons harvest it from that area exclusively and is the most common procedure done. I had a variation of this done via a method that Mt Sinai in New York uses (Dr Ting - who helped pioneer the method). They use some material from the genital area (already under change) and use it to line the canal combined with the more traditional penile inversion method. As you stated, the end result is that it adds some moisture generation in the (neo) vaginal canal. Even Mt Sinai warns if the donation area does not have enough material, then they will harvest from the abdomen to supplement. This variation is technically not the pull through method, but uses the same base procedure logic/result. As with anyone else, the amount of moisture generated via the method varies by patient (some people are more "liquidy" than others), but not enough for more vigorous use (aka sexual intercourse). So a lube is still recommended for those activities. Hope this helps with your understanding of the variations related to this method.

My goodness Malibu. I could write so much about what you have said. Your age makes some of the surgeries unnecessary. You do have good features and I think you should steer away from fillers. The amount of money I have spent on lip fillers I could have opted for my own body fat injected instead and this would have lasted forever. IMHO you don't need a tracheal shave or removal. My Adams apple is also small and almost disappeared when I had a face lift. I was lucky not to need a brow reduction or chin shaping. You do talk about hair removal and Laser. I may be mistaken but most places use IPL which is often mistakenly called Laser.. With HRT the body hair will get softer and in my case with continual waxing it has virtually disappeared. Start getting the genital hair removal ASAP as it can be a long process. The penile pull through is the safest of the three but if you want the depth a perineal procedure would be better. All I have done is scratch the surface here. I knew so much more after theses surgeries as at the time I never knew what questions to ask.

@@PhilippaJane I've not heard of fat grafting to lips before perhaps that's something I should look into, but I feel like I'd at least want to see if I like how it looks with filler before I do something permanent. The laser I get is all with the NHS I was also worried about it taking a long time as it has with my face but I was told genital hair is easier to remove so I hope that is true for me :S And the SRS I think its almost definitely the penile inversion ill be having but I shall find out soon for sure.

But i am lucky i got weighter hips than my torso (not much) compared to some of my cis female friends.. i am not tall even by women standards, i do not have a shape like a barrel, and i have had been voice training before my transition because of acting 😅 hormons even gave me a well sized bot, small but noticeable boobs (C coup) and my hair line weirdly enough grow back more or less 🥰 Sry for praging 😅 not ment to.. i'm just glad that it went this well
