i wouldn't include driving or getting dressed in the equation because those don't include the risk of malpractice when reading cases.

I hear you but disagree. The time value of commuting is approx 2h or $500-700 USD. Weighed against the nominal increase of malpractice risk of reading 30 more cases. The improved income would hedge you against future risk more than the liability risk of reading the cases. Plus you have insurance for that which should protect you

Thoughts on how screwed I made myself. I’m teleneuro, solely during the day, 7 on 7 off, and on the weekdays I read on average 105 RVU/day, so about 13rvu/hr. Worst week was probably 110-115rvu/day, but I didn’t specifically calculate that. I’m salaried and my comp hasn’t gone up since I join about 3 years ago despite volume ramping up and up and my definition of a ‘good week’ has gotten worse and worse. I’m assuming…this is bad. I do about 60% Neuro, rest is body ct/us, plains.

@@sarelgaurMD funny enough, my counterpart (the one who does the other week) and I did, since other teles in different roles got a raise. It was refused but we may be getting quarterly bonuses, not much though relative to base salary and % of increase of workload. Anyway, we may leave, it’s always the question of trading one set of troubles for an unknown one in a new job. Have you worked both the salary and the rvu model for tele? Or just the eat what you kill model?

Great video. 👍 a.Is it difficult to get out of teleradiology to do normal radiology job once youre in? I have heard this being said a lot. Why???? b.Also, can we do IR from 9 to 5 and work as a telerad few hours at night ? Is it possible? c. Since radiology pay/hour is better, if a radiologist DR works surgeon hours from morning till evening, does compensation rise significantly above or at least better than a surgeon? I would love to hear your response whenever you have time. Thank you.

A. I have heard that too not sure why. Tele cases are er mostly but also inpatient and outpatient cases. Hospitals that are understaffed sell their cases to tele so it encompasses all specialties. B. Yes it’s possible. Get an IR job with no compete and then sign up for part time tele job C. Compensation is rate x cases x duration. Yes your make more if worked more. At some point you might have a hard time getting more cases for decent rates

Hello, Thank you so much for sharing all of this. I have two questions and would greatly appreciate it if you answered them. 1. Given that it is teleradiology, are you able to also practice outside the USA? 2. Does it matter if you are a diagnostic radiologist vs interventional radiologist when it comes to teleradiology?

1. not sure, i've heard you can practice outside the US but I would say to be safe, probably will have to stay in US 2. No. as long as you can read cases, you can do tele

so the question becomes: what is the AVERAGE number of RVUs an attending radiologist produces per hour in teleradiology? Let's forget about how US is less than MRI etc. just an average. Is it actually near 6 RVUs or much more?

Hey great question. I'm not sure the avg. I would say in order to be successful you'd have to read 9-10 wrvu / hr or more. It's doable but have to work hard to reach that efficiency

You can moonlight internally a lot, or do extra shifts a lot of times to get that number up before partner. After partner, you'll be let's assume almost 1.75-double salary.

Hey. My numbers may be off but I feel that’s accurate. Proximity to a large metro costs a premium and pushes MD salaries down. I specifically discuss tele here as an alternative to a boots on ground job in the metro areas

You are leaving a huge part out of traditional private practice in the comparison. Your first year salary is going not what you will be making the rest of your life if you stayed at that job. Most of the time people are trying to become partners in the practice and subsequently have a fairly substantial increase in income.

Hey, true. The point of this was to think about your hourly rate. I'm also talking about metro areas where many people wish to live. Take your presumed partner income of 500k and 10 wks vacation. Add in 2 days call per month. Divide that by 9 hrs a day and 2 hrs commute. I get 194 for hourly rate. If can read 10 units / hr, would beat that as a tele and importantly would do so in your first yr

Add in some late days working longer than 9 hrs (which happens) and any other time commitments (ie meetings) and your rate further plummets
