Another brilliant video - clear and logical and to the point. You got me top marks in my FRCEM primary (A&E speciality exams are very anatomy-heavy) and now helping as I start my anaesthetics placement! Thanks so much!

Very interesting, I just came to calm myself from the lumbar puncture I'm getting and I stayed for your brilliant explanation! Great one, thank you for the video.

After a spontaneous csf leak I had an epidural blood patch done in an attempt to close the dura. The blood fills the epidural space, surrounds the spinal cord and clots hopefully closing any holes. They managed to get in 18mls before I could start feeling it so they stopped. Its all so interesting.

My dad had Cauda Equina Syndrome and so I am looking more into this part of the spine. Doctors said it was a grape sized tumor and he had herniation near there too on other parts of the lower spine.

@SamWebster I love all your videos and appreciate your wealth of knowledge 🙏 If one wanted to inject a peptide for healing with a 5/16 needle/syringe into the lumbar, sacrum or coccyx region would there be any risk of hitting a nerve? Or are all the nerves safely hidden under the vertebrae?

Sorry your comments on epidural anaesthesia for obstetrics are misleading. I am qualified and experienced anaesthetist. Epidural needle is inserted at L4/L5 or L3/L4 space in obstetrics commonly(or level above), and not in caudal epidural space. Other Lumbar or Thoracic spaces are used in other surgeries in adults. Caudal epidural space is used kids and some adult epidurals.

Hi Sam, I sustained a severe CSF leak of the lumbar cistern back in May and I still have the epic migraines, pain and inflammation. Is there any way this will heal on its own? It has been ages. The ketogenic diet and intermittent fasting is helping, but seemingly nothing cures it. Any help on this issue would be incredibly appreciated (by anyone). Thank you in advance. It is a horrible affliction.

My symptoms are swollen and saggy skin around eye socket and eyelid, thin unusually shiny fluid film over eyeball, severe migraines, grey complexion from time to time. Any guidance would be amazing.

"this week we going back to the back" and i am going from Joints to Joints every day one Year time,🤣 but i got some resson why...👣to turn musculatur back in place.🤔this not for studens🤣😉✌You are the best..🎯..all body connectet ..🎯.How important is tissue release, release parts,where need to release..stretch muscles where necessary....getting to a primary muscle problem is not easy.....turned calf somewhere and shortening of the Achilles tendon...ankle biomechanic will turn our life and musculature some where👣🙏if human organs start contracting ,What could be the consequences?this question is for students😉?muscle is also an organ But no one heals them😭😢😭

I recently had a lumbar puncture to get an opening pressure they were able to get fluid in the needle hub but it wouldn’t flow to get an opening pressure why would this happen?

Hmmm so when i thought i had an epidural with my first born, i may have but as he was awkward and didnt want to come out, i was taken to theatre for a c section so i think i must have had something stronger as i didnt feel anything. Turned out i had dilated enough to avoid c section and had him via forceps down below. The lady telling me to push as he had to get out now but i couldnt feel anything to push lol. Must have had a spinal block. Then on my fourth baby i gad a c sec as he didnt want to come out too 🤣 so def had a spinal block there. No wonder my back hurts being injected twice there. Maybe that and the babies weights in my pelvis had something to do with my back pain now 🤔

Sorry used google translater from Russian language...it's got logic?😁?Numerous studies have proven the close relationship between spinal cord compression and spinal canal deformity. In this case, the magnitude of the deformation of any of the (PDS) structures can play an important role. Thus, a decrease in the height of the intervertebral disc by 1 mm causes a decrease in the size of the intervertebral foramen by 25-30%, and by 3 mm it causes a decrease in the area of the intervertebral foramen to 50%. It was also shown that a decrease in the sagittal size of the spinal canal by 3-7 mm leads to an increase in the thickness of the posterior longitudinal ligament by 2-5 mm. With an intensive course of the degenerative process, a decrease in the size, and hence the area of the intervertebral foramen, leads to the development of pathologies and neurological manifestations characteristic of a certain level of the spinal canal

Excellent video. Just thinking though, what is the difference when the patient has primordial dwarfism or there is some form of spinal defect resulting in retarded growth of the spine (thinking of a pair of identical twins who I nursed years ago who had severe kyphoscoliosis, but whose individual lumbar vertebrae were unusually shallow resulting in a reduced height of approximately 6 inches had they not had this, obviously, the kyphoscoliosis further decreased their height to roughly 4 foot 9 inches.

I have a question Why we can't puncture in cervical region? @Sam webster
@SamWebster
@SamWebster 5 лет назад
You can puncture in any region, but the risks are higher in some regions than others, and some are anatomically more difficult to access.

Neurosurgeons and interventional radiologists do so, but they use imaging to guide their needle and avoid damaging the spinal cord
