This game absolutely *requires* a SAVE feature of some variety, even if they want to keep the "death means go back to the beginning" mechanic. You would literally be forced to complete this potentially multiple-hours fangame, in one sitting, and death means starting from the beginning. Even if you want to keep permadeath, the two are not mutually exclusive. Just delete a player's SAVE file upon death, but do NOT force a player to complete the game in one sitting if it's going to be longer than an hour or so.

The developer is not good at making games, nor being professional. One of his spriters and admin stole sprites from primus. I'd stay away from this one.

Revisit some of those rage games he cheated through/quit due to time restraints before, try out a few other recent fandoms and see how he likes them, maybe even react to some Undertale AU sprite comics, change the pace. I'd even be fine if he uploaded on a slower schedule in exchange for more quality control/making sure he isn't doing a video on games with Flowey developers.

Possibly, but you have to keep in mind that toriel doesnt harm any children that comes her way. Underworld is no exception to this, due to every character's personalities being the same, but their appearance being widely different. That, and you're forgetting that this is an au where everything went to shit. The monsters in underworld could likely bleed, not to mention, sans, in canon, has been shown to bleed before, too. Not only that, there is also a cut mark on toriel's face when you kill her as well, and while it's black, it could very well indicate that monsters can be wounded the same way humans can.
