As a physician of 40 plus years, I recommend 500mg BID starting with 500mg in the AM. It is a must to have the patient take 1000mcg B12 with 400mcg of folic acid every day. In the long term, with out the cyanocobalamin (B12) they will have serious problems later. The folic acid carries the B12 into the system to be used. Just a FYI.

Doc, isn't FOLIC ACID and Cyanocobalamine aren't the greatest forms of these two supplements. Wouldn't methylated forms be better. Folate, not FOLIC ACID.

The one thing I've noticed that causes diarrhea for me is eating excessive garbage carbs.

Hi Manny, Eating healthy is so important. Definitely reduce the simple carbs and eat more complex carbs with higher levels of fiber.

Thanks for watching, Rachel! It is very strange... Higher dose does not make a medication "brand", but for some reason, it always requires a prior auth. I avoid it, in general, as discussed in the video but that never made sense to me either.

Thank you for talking about B12 deficiency. I have numbness in my hands and knees. I've been to 4 doctors and none had any idea what could be causing it. A1c is 5.6 so it's not diabetes.

Hi Larry, I am sorry to hear that. I hope your symptoms have improved. B12 and other vitamins are helpful in improving symptoms related to numbness and tingling, it's not always just diabetes.

Lots of great information. I have been on metformin for over 3 years and have had chronic diarrhea for that whole time. No one bothered to explain to me the association. I will definitely follow up on this. Thanks

People I got my numbers down to normal range without metformin. There's numerous herbs that help tremendously such as: Berberene, as good as metformin without side effects. Alpha lipoic acid (ALA) Cinnamon Cloves Chlorophyll Chromium Berberine is the best I've found. Also I take a carb blocker. This brought down my numbers immediately. ❤️🌷❤️🌹

Diana Was searching today on RU-vid about the side effects of taking metformin and came across your channel. My Dr. Started me taking metformin 500 mg one tablet bid for a week and the switch it to 2 tablets bid, I didn't notice the onset of symptoms until I started the new regimen. I thought it was because I was taking amoxicillin with the metformin because I had a root canal done and I thought it was causing me to have diarrhea but boy was I wrong I have to say thank you for breaking this down for me you just took the guessing out for me 👍

@@Bro.TommyEJohnson I understand. It’s very frustrating. Side effects to medications are common unfortunately. We wish they only did what we want them to do without the side effects. Sometimes we have to try different things until we can find what works best for the patient. Everyone responds differently. It’s hard to know until we actually try and see, always hoping for the best outcome.

Why do some doctors not know this and others do, that’s why I manage my type 2 diabetes by diet and exercise, it’s hard enough dealing with diabetes than to deal with side effects of medications, way to much stress, I think all doctors should know this, I feel bad for people who live with this horrible disease, it’s a very complex disease, also in some of your videos you use the word obese and obesity can you change that to unhealthy weight. Here in Canada we’re trying to change the wording around diabetes, people with diabetes find it very insulting and not helpful, just advocating for people who live with diabetes, did you know that people with normal weight can develop Type 2 diabetes also, just some food for thought.

Thank you Diana. I've been taking 1000mg of metformin twice daily for 20 yrs.. My primary physician never once has brought up the B-12 recommendation. I took it upon myself to research it. I take Nature's Bounty B-12 always placing it under my tongue for a minute before swallowing. This is a USP tested product which is what I always look for in supplements. I've just found your site and have subscribed.

I have taken Metformin for 24 years. Never knew about B12 or Folic Acid, thanks for the one-up! I occasionally take B12, but will make sure to take it more consistently.

Glad you enjoyed the video. It is always helpful for nurses and patient's to know possible side effects of medications. We find that patient's will undergo extensive GI workup when metformin was the culprit of the symptoms. Unnecessary stress for the patient and unnecessary testing that can be avoided with more knowledge about possible metformin side effects.

I was told I was a Diabetic on a walk in I did. I couldn't believe what I heard cause I was told my lab work was good most of my life.. So I did what I was told by the Dr. Still I couldn't believe so this what I did I took the b12 metformin and BGR and that come up good all the time and also stop sugar goodie till next update appointment and told I was doing a lot better and schedule another update.. And after that visit I stop taking only the b12 metformin and started up the goodies.. My BGR that come up good all the time.. On the next update appt I was told I was doing better and keep up with b12 metformin.. Been almost 3 years and till this day I never told the Dr I stop taking the b12 metformin.

@@OrangeSkyMusic Hi Brenda, sorry you had a bad experience with metformin. Not everyone can tolerate it, unfortunately, There are other medications available if you cannot tolerate metformin. If your blood sugars are not in good control on just diet and exercise, I'd recommend discussing with your doctor to see if there are other alternatives. You want to keep your sugars under good control as much as possible to prevent complications.

I tried starting it a week ago…got exactly what you listed at the beginning ..total Gastro issues, terrible, diarrhea in the middle of the night…I stopped taking it . I started a LOW CARB DIET 10 days ago ..losing weight so far. They gave me 500 twice a day , I cannot tolerate it. I am Pre-diabetes officially so I am going to try with diet alone right now.

I am Type 2 diabetic for the last 15 yrs.I am taking medicines Torglip50/500&GlygometGP1 twice daily and my HBA1C is 6.0.Both these medicines contains 500mg Metformin each and I am consuming these medicines for the last 6 months.That is 2000mg Metformin is consumed daily.Now I am having excess flatulence which make my life miserable.Also stomach issues .Is these problems are because of Metformin dosage?

Sai, thanks for watching. I am glad you are learning from them. Although I can’t give medical advice as I don’t have your medical chart and information… yes, patients doing intermittent fasting can continue to take metformin. Metformin works by reducing insulin resistance and working with the liver to “stop” overproduction of glucose. Therefore, very unlikely to cause low blood sugars. Please advise with your doctor as they know you best. This is in general terms only. I hope it helps. :)

Informative video. I'll be honest with you, I hate metformin. It is intolerable to me. I truly hate this and what it does to me. Probably the worst medication I have taken in my life. I will call my doctor and try the extended release to see if that helps. This med is terrible for me. TY MH

I'm starting it tomorrow..500 milligram Extended Release. I'm very concerned on the side effects especially B12 deficiency. But am aware all drugs will deplete nutrients from the body. I've had issues with anemia in the past.

It can cause b12 deficiency especially with long term use. You can request to have your b13 checked yearly if you wish. You don’t want to be b12 deficient for several reasons. I’m uploading a video on that this weekend which you may find helpful :). Thanks for tuning in

@@thevoiceofdiabetes : I have had to get blood transfusions twice in less than a year. I recently asked my gastric doctor (at the hospital) and my Endo doctor if the anemia, unknown bleeding, and need for blood transfusions could be linked to Metformin. They both said no, but didn't explain why not. Have you ever researched or had a patient who did become anemic and need blood transfusions and it WAS linked to Metformin use? I currently take 2000 mg XR, but I take them all together at breakfast. This was the first time I had heard about a scaled increase or splitting dosage so it doesn't hit you too severe with side effects.

@@sandradeleon6171 Hi Sandra , no, there is no link between Metformin and anemia. You’d have to look for other possible causes. The way in which metformin works, it wouldn’t cause blood loss. There’s a video is how metformin works. Check it out. You may find it helpful. Yes, if you are having side effects, definitely split the dose or take extended release as mentioned in the video. I was diagnosed with GP. I am overweight. In 2003 I had a laparoscopic Nissen and ever since I cannot drink carbonated drinks and I can only drink with a straw. When I was a kid I have always had a very hard time being able to eat many different types of foods like many fruits and vegetables. My entire life if I was to eat a salad I would get sick the minute I put it in my mouth let alone if I was to get any of it down? For example, if I have a burger it could only have the bun meat and cheese and no spread of any kind. No mayo etc. If I was to have a tuna fish sandwich it could only be made with butter plane. I have been trying to lose weight but have been having a hard time doing so? I was just put on a medication called Metformin because I was told I was producing too much insulin and that was causing me to gain weight? I hate the food pyramid and even the my plate because they don't address issues like GP and other dietary issues? Because I am legally blind and use a power chair with limited mobility it's hard to read labels and count calories to be healthy and know what will help me lose weight and live healthier? I wish someone could make a video about this issue?

So it is acceptable if you feel the patient needs injections vs. oral b12. It's patient and provider preference as well.... Hope that helps! Good luck!

I been taking this for pcos in the evenings I noticed it makes me go to the bathroom in the morning and i was very tired till I balanced it with vitamins. Are there any foods in the evening that I can have that will lessen this effect in the morning?

Hello, I am sorry to hear about the am side effects you are having... are you taking the extended-release or regular metformin? if you are taking regular metformin, I would suggest starting by changing it to extended-release as that may make the difference without needing to adjust your diet. There are no particular foods to help-- however, I do recommend a higher fiber diet which includes whole grains and vegetables. Hope this helps!

Hi there Real Deal, thanks for your question. I would suggest taking extended release vs. regular metformin. Also, start low dose. 500mg daily then gradually increase to 1000mg daily, etc, until you reach the desired dose your doctor wants you on. Also, always take this with food. Never on an empty stomach. Hi Butterfly, have you tried increasing your water intake and adding more fiber to your diet? That can be helpful with that. Also, increasing activity is great for constipation. Hope this helps.

My husband only take one because he’s blood sugar are always between 100 and 80 and he lost weight and he’s numbers went from 7to 6 and they want him to take 4 that’s too much like I said he’s doing one and he’s being doing it for 3 months and he feel very well

Hi Reina, I am glad your husband is having success with metformin. My recommendation would be discussing it with his doctor. If he is doing well on a smaller dose and his blood sugars are in target range, I would likely suggest keeping him on the dose that is working. However, if his doctor is recommending an increase, I would suggest discussing with his doctor first. It is always good to vocalize your concerns and question orders to clarify why something is being changed. We love when patients are involved in their care and are knowledgeable about their health and medications.

hello :) thanks for your great video, i have insulin resistant and started taking metformin in december i started with 500ml a day and worked my way up ..i am now at 2000ml a day ,i didn't have any extreme Gastrointestinal complaints but i have a lot of flatulence is this normal ? and will this go away? and I have to say that I haven't lost weight ? ..are there certain things to be aware of when it comes to the diet while taking metformin?

Are you taking regular metformin or extended release? That can be a side effect unfortunately. I would discuss with your doctor about reducing the dose and trying the extended release if you’re not on already. Metformin is not indicated for weight loss but it can help given it reduces insulin resistance.

My husband has been on Metformin for 30 years. Over the past year he has experienced bowel problems. I think he is maybe taking too much now as his weight has dropped by a stone and a half with attention to diet but the dosage is the same. Could he be now taking too much metformin?

Hi Marion, thanks for sharing this. If he is starting to have bowel issues, you definitely want to discuss with his doctor about lower the dose. If he's been able to lose weight with lifestyle adjustments and his blood sugars are in good range, since he is having issues as mentioned, lowering the dose is definitely a wise move. Hi Bufferly, one certain metformin brand was recalled sometime last year. However, they’ve since switched patients who were on that particular one to the ones that haven’t been recalled. If you were prescribed metformin now, you will automatically get the correct ones. Hope this helps. Good luck

Hi Kumud. Thanks for tuning in to The Voice of Diabetes. Max dose recommended for metformin is 2000mg-2500mg (varies between regular and extended release). You’re dose is within the recommended ranges so you are good with the dose. Your readings seem to be in great range! Hi Iqbal, thanks for tuning in. Yes, you can take CoQ10 as a diabetic. Just remember, over the counter products are not FDA regulated so be sure to always follow up with your doctor for regular blood work and to make sure it doesn’t interact with other medications you are on. You’d be surprised how these over the counter supplements can affect other medications and or kidney/ liver function. In general, CoQ10 is not contraindicated in diabetes but always make sure your doctor is aware you are on it just in case you’re on a medication that can potentially interact. Hope this helps.

I have been taking metformin for 0ver 30 years, 2x1000mgs daily. I am breaking them and taking them 4 times daily, No negative effects except over the last year or so I have been constipated. After years of not having that side effect is it likely that metformin in now the cause? Any suggestion on maybe taking it differently? Is it likely to be the cause of my constipation? I have had all kinds of test over the years and never saw any negative side effect from metformin. Also, this does not definitively mean anything, but I will be 79 in a week, which is 5 years older than the next oldest person in my immediate family has lived to be. Perhaps metformin can also increase life expectancy? Thanks......

Hi Robert. Great comment. Thanks for sharing this with us. It's not uncommon for people to develop side effects from meds even years after taking something-- we get older and our bodies respond differently to things. Therefore, we can't rule out metformin as a possible cause. There are some studies to suggest that (longevity), however, there are no indications for it's use for that as this time that is FDA approved. We will have to wait and see if that is added as one of it's benefits. Would be nice...

I started metformin at 1000 mg a day. Since starting 2 weeks ago my feet tingle and have shooting pains that travel. Is this due to vit b 12 deficiency and would this symptom come so quickly? Plus my eyesight got more blurry. 🙄 also when I take vit b 12 supplements it calms the feet down a lot

Hi Lisa, sorry to hear about that. So, it's unlikely those symptoms are from your medication being that you just started it. It's likely because there is a change in your blood sugar values. You most likely went from high blood sugar readings to more normal values, therefore, you will experience blurry vision, tingling, etc. however, those symptoms will get better with time (usually within a few weeks) as your body will adjust to more normal blood sugar readings. Metformin can be in your system for roughly 4 days which is why we have patient's stop it about a week before a procedure or surgery. However, you need to discuss this with your doctor. We take other things into consideration such as kidney function, etc. Hope this helps

Metformin often has no or only minor side effects. I have been talking metformin 1000mgs X 2 daily for 30 years. No adverse side effects...no low B-12 readings, no kidney damage...it just works...... I would say the vast majority of people who take metformin have no or only minor side effects. It's a safe and effective and inexpensive drug that works.......also, I will be 79 in a few days and am hoping the longevity effects that are being contributed to metformin are proven correct in the on-going studies.......anything over 80 and I will give the credit to metformin.....😊

Im taking gliclacide in the morning and metformin after lunch but 2 hrs after my body felt weak, shaking, palpitation and difficulty in breathing. Thats why sometimes i dont take metformin after dinner. But also somethimes i dont feel any side efects so i take metformin after lunch and after dinner.

Hi Vicky, Gliclazide can cause lows unfortunately… not my favorite class of medications. It’s likely the reaction you are having at times is caused by Gliclazide and not Metformin. I would be curious to see what your blood sugar level is when you are having these symptoms. I’d discuss cutting down on Gliclazide and take metformin regularly. Metformin is a much better medication. Hope this helps. Best of luck

Hi Jeannie, Sorry to hear that. Usually this helps with some weight loss but not always. I will recommend making some adjustments to your diet if you feel you can improve. You want to reduce calorie intake in order to lose weight. I'll have a video on this soon

Hi Pintchou, sorry to hear that. Have you tried the extended release metformin or just regular metformin? Sometimes it makes a difference between the two. You may want to ask your provider if they feel it is appropriate to have you retry the extended release if you already haven't tried that.

I'm currently taking metformin at glib ride and lisinopril together I have been on and off this stuff for too many years account I really think I'm in trouble with this new clinic I'm going to with these doctors either some scary times take this pill come see this other doctor come back in 4 weeks to a month repeat I've gone from 5:12 to 3:20 with no help from the drugs I actually stopped going to the doctor for 2 years I don't know why I started up going again I think it was just severe pain all the time headaches wearing some bad times

Im prediabetic. First I started with a 160ch diet. Two weeks later I was prescribed 500ml, two month later 1000. Two weeks after my dose was increased, I started having higher reads on my glucometer. How can that be? When I was on diet only, my readings were better.

Hi Anibassb, hmm, are you testing at the same time of the day? Different foods have different impact on your blood sugars. Also, stress level impacts your sugar readings. Therefore, try to keep your day to day very similar and see how the readings are...

@@SebiSzabi hmm, haven’t heard of that. Normally, metformin “blocks” the release of sugar from the liver and improves insulin sensitivity to the cells… I’ll have to look it up to see how that’s possible. Haven’t heard or come across such. I hope things are better now.

Actually My doctor took me off metformin by now, and Im regulating sugar levels with exercise and diet only. 💪🏻🥑🥇Im doing really well with it, and lost 7kg-s already (through 5month in total). And yes, metformin does block some of the glucose being released back in from the liver, but apparently not all. Im not sure of the chemistry here, but thats what my doctor told me. And when I was cutting back to one 500 mg tablet a day, the sugar was better so it worked. And thats the main thing. 🌷

Hi Viveka, it depends. Sometimes patient's can have some mild side effects for a week or so which get better over time. However, if the patient is having moderate/severe symptoms, I normally discontinue it. We don't want the patient to become dehydrated which can lead to more serious issues. It all depends on the severity of the symptoms. Most patients, fortunately, tolerate it well when given in this manner.... Extended release and increasing slowly.

@@vivekatibblin8359 honestly, not that I am aware of. Usually, I have switch to extended release and take it with meals. I have them try the 500mg first. If they can’t tolerate that then metformin is probably not the med for them and I discontinue it. Please reach out to your provider for more answers. This is how I treat my patients but I can’t give you medical advice or replace what your provider suggests as I don’t know your medical background and specific needs.

Hi Jrow, yes, of course. I have many patients who I eventually reduce or discontinue the dose because they’ve made a lot of lifestyle modifications and no longer needed it. That’s something that you can discuss with your provider and always consult with them. I don’t recommend making any changes without speaking to your provider first. Hope that helps. Good luck!

Hi Alex, I wish we can find a cure one day. However, treatment for diabetes has come a long way... We used to have patient's test their ketones to see if they're under "good" control to having continuous glucose monitors that don't require a single fingerstick. It's amazing. So much more to come...

Is it so harmful to take metmorfin? If so why don't you suggest alternate drug.which is better - extended release tablets or normal tablets? Iam taking metmorfin 500 mg one in the morning and another in the night since about 15 years. No serious side effects till now.

Hi Uday. Thank you for watching the video. Metformin has been around for a long time and has a lot of safety data. I have patients on either metformin or metformin ER. If you are tolerating it, that’s perfect. I wouldn’t suggest changing to extended release form unless you were having an upset GI, etc then it would be worth it. Otherwise, I keep patients on the regular metformin and they do well. I will talk about different medications as well, as I continue to build content on my channel. Because it is not "so harmful" to take metformin and for 99% of people who can benefit from metformin it is a proven safe, effective and inexpensive drug. I say this as someone who has been taking metformin for about 30 years, It was called "Glucophage" before it became available in generic form..........I'll be 79 in about a week and, who knows, maybe that life-extension possibility of taking metformin is true?
