Giving noble/heroic characters unnaturally small heads is a very common thing in modern art in general. Usually not by this much, but Rennala also has a really tall crown, and her silhouette would be a harder to take seriously with more realistic proportions

You’re not the only one who thinks so. It could be her robes & all, but honestly, FromSoft kinda lost the plot when making her full model the way they did. It’s either her shoulders are too broad, her head is too small or her robes are too big, but you can’t really tell what is what because FromSoft’s model perspective here is trash.
