Nearing the end of my clinical year of medical school, what is supposed to be one of the most exciting decisions has become one of the most anxiety and stress producing parts of my life. As I finish up my pediatrics rotation, I'm slowly starting to piece together the parts of my clinical experiences that I want to have for the rest of my career.

What about a med-peds residency and a 1 Year EM fellowship? You get the adult and peds training, then you can work in PEM. This route is 5 years and gets you 3 board certs, as opposed to a peds or em residency and then another 2-3 year fellowship which gets you only 2 board certs

I’m a nurse and I’ve been following your channel for a while now, enjoying all your experiences and perspectives. I know you’ll end up where you want! I’m curious to know more about your disagreements with surgery culture because I too have my opinions on it. I have a lot of respect for medical students and residents, you guys put in a lot of time and resources to get where you are and as a nurse that can be a somewhat controversial opinion to have.. anyway, I’ll be cheering you on from the sidelines. I know your channel is med student focused but seeing some content on nurses would be awesome given that the professions are so inter-related as well as dependent. Not to mention, the culture between the two could use a revamp. I’m close in age to you i would assume, so we’re the new guard and it’s up to us to help set new equitable standards. Keep rocking man!! Also, definitely *not* a casserole. It can be made into a casserole form, but traditional recipe I would not classify as one. I’ll die on that hill. ⚔️

I’m sure it’s filled with crazy emotions. I dealt with some on my nursing rotation in peds and my short duration was certainly life changing. Just didn’t think I had the stamina to endure the lows of the field.

If it makes you feel better I’m a resident in a surgical subspecialty and here’s my input. It’s going to be hard to choose a specialty. I don’t think anyone really knows. People who say they can’t imagine themselves outside of the OR as a medical student are just trying to convince themselves because there’s no way to know that until you’re actually doing the surgery, I personally hated watching. At the end of the day you’re going to love your life and you can shape it into what you want. People will always convince you that their field is the best one. You can work more than a surgeon in pediatrics or anesthesia easily. You can’t know if you love the OR or not until it’s in your hands. Similarly it’s tough to know if you like working with parents until you’re calling the shots and having the burden of parents being stressed and relying on you. I love my subspecialty and to be honest I work less hours than my friends in IM/peds. But that is also program dependent. So really everyone’s medical school experience is different and what you see may not be what residency is like and what attending life is like in general. So all in all I know I probably confused you more, but you’re going to have to trust your gut and just choose. There are only a limited number of outliers when it comes to “bad fields”. You can’t go wrong with the options you have in mind.

Good luck in your decision! I know after you do your rotations you will know what ur heart draws you to the most. I am a nurse in acute care working my way to CRNA and I can tell you it took me a long while to decide my choices, but the clinicals and shadows I have done really were what helped me choose my path.

I went to medical school 100% sure I would do radiology. I had a complete change of heart and ended up in psychiatry. It was a true calling that made perfect sense in the end. I also have a corneal disease, and my vision worsened somewhat, and I could end up needing a corneal transplant at some point. Because of that, I tried to have an open mind and consider other fields since I could have ended up with practical limitations in radiology because of the corneal problem and eye discomfort. I cried over that decision since I spent so long dedicated to radiology, doing research with MRI, telling everyone I was 100% radiology from the start, etc. I may have been really great in radiology, as well, from a skill/performance perspective but probably not as fulfilled personally. Good times. Medical school will always be such a great memory. Residency, too, but medical school especially since I still felt like a "kid" with an undetermined future starting with fears of not even passing the first anatomy exam. Absolutely can't imagine doing anything else now. Premed, med school, residency...it's such a trip.

Hey! I’m actually from overseas and was in kind of in the same dilemma few weeks back as you are in right now. I was not able to decide which branch i wanted to pursue and somehow it spawn to me that maybe I’m suited for paediatrics considering my nature and clinical interests so far. I still have my peds rotations up next month which I hope will help me further strengthen my decision. Just happy to know people like me exist everywhere 😊!

It's a bit strange that you have to pre-declare! In Australia, after graduating, we do 2 years of junior doctor years before beginning specialty training. So even tho it's smart to have an idea of what you want to do, our decision making process is postponed and delayed for us, which I'm not complaining about. All the best Andy, it's a long grind and it's ok to take a rest sometimes to come back even better! 🔥

Andy, I’m currently in school also deciding my specialty but I’ve worked in the OR with surgeons for a while. Everyone says the same thing, if you could see yourself doing literally anything other than surgery, do that. It’s the kinda thing you gotta be 1000% sure about.

I'm not in medicine so take this with a grain of salt, but listening to you describe your thoughts about those three specialties, you sound like some part of you prefers pediatric emergency medicine. You described not loving surgery culture, and surgery and anesthesia sound like they will both be part of (or at least, surrounded by) that culture. And you said you love taking care of children (and talking to parents) during acute situations. I think you'd be great at any of them and I'll look forward to seeing what you pick :)

I can't imagine having to make that decision, but you are so lucky in so many ways. Pray you will make the right decision and find the best way in which God wants you to serve your patients. I'm sure you'll find the specialty that fits right for you and your future. I'm currently struggling to even START studying for my MCAT and to prepare for applications next cycle, but your videos make me realize that I have to jump in and get used to it. You might have decided your specialty already so this comment might not matter that much but I wanted to share anyway. Earlier in the year my 1 year old daughter was admitted to the local pediatric hospital for a really high calcium level. It took a long time and a lot of different specialists to figure out a potential cause. As a result we worked with the pediatric er doctor, oncologist, endocrinologist, general med surg attending doctors. The anesthesia team, the x ray team, the echocardiogram team, the MRI team. The doctor that we connected with the most was one of the general med surg doctors because she felt like our life line in a really scary situation. She coordinated all the different care my daughter needed and even came and found us in a different section of the hospital once where my daughter was supposed to be getting tests done to see if everything was ok because things were taking a lot longer than they were supposed to. So if you want to work with patients and parents to help figure out complicated health problems being a doctor in the general med surg section of a pediatric hospital would be a potential alternative to being a pediatric ER doctor.

Pediatric EM is the way to go for you I think. Great lifestyle, moderate salary. I'd say go EM and then go for PEM. I am IM, I applied to both IM and EM. I am mostly content in IM. One thing you need to accept is that you probably would find happiness and contentment in more than one specialty. Once you accept that reality, it becomes easier to let go with the idea of "this is the perfect specialty for me" mindset which I also fell into. I am glad I chose IM just because I have a lot of options available to me, both in terms of specialty in practice variation. Good Luck

PGY-1 EM resident here. I can offer some insight into the PEM route. As an EM physician you can see patients of all ages, no fellowship required. This means that you can pick up lots of shifts working in PEDs EDs if you’re credentialed at a hospital that has a PEDs ED. This is how things are where I work. PEM fellowships are usually done by PEDs grads who want to do EM. Very few EM grads decide to do a PEM fellowship bc they don’t really gain much from it, and the cold hard truth is that they make more money seeing adults. From what I hear the EM physicians that do complete PEM fellowships often go on to have academic jobs in large childrens hospitals and things like that. Hope this helps. Good luck.

I have a mutual friend that just started a combined EM/Peds residency program at LSU New Orleans and she loves it! If you end up going the emergency medicine route you should definitely look into it since you would get the best of both worlds!

I looked into that program! I believe only one of 5 in the country like it, but it trains you in both pediatrics and EM simultaneously but not specifically peds EM which I think is the end goal for me😅I think it allows you to practice both separately but would still need to do fellowship for specifically peds EM

You can practice in many places without a PEM fellowship! Not sure on a academic center, but many community places will. Our local peds ED (a community shop) allows docs to practice without a PEM fellowship. However, they have to be em and peds trained

Based on what you said, I think you would like pediatric anesthesiology! If you are not in complete love with the OR and surgery lifestyle, anesthesia is a great option that gives you a bit of everything.

I think one of the best questions you can ask yourself when it comes to making decisions like this is “What will I regret the most NOT doing?” Will you regret not being able to be the surgeon in the OR? The ER physician helping children? The pediatric anesthesiologist? It’s normal to have interests in many fields and I think asking yourself this question will bring a lot of clarity. You’ll be able to distinguish what you’ll be happy doing and what you’ll be the happiest doing. Hope this helps!

Personally, as an adult with chronic illnesses that originate in childhood - I would love to see more Internal Medicine/Pediatrics doctors. (I think it's commonly referred to as Med-Peds.) I feel that the knowledge base is much wider, and the provider would have an idea how to treat kids once they become adults, thus bridging the gap between pediatric and adult care. (This is a big issue, finding adult providers who understand cerebral palsy, for example) That might be a specialty to consider (however, you will do great in whatever specialty you choose - I'm already jealous of the kids who will be your patients.) Maybe consider doing a 73 questions video with someone in Med-Peds. Or a video explaining the difference between Med-Peds and Family Medicine.

If pediatric surgery is of interest to you, but there is certain cultural aspects of it that you are concerned about, maybe reconsider that you can potentially locate a work environment that is more aware of these issues and addresses them. That way you can do what you love, and work in a positive work environment as well-or at least strive to lol.

Choose the one that you would regret not going after when your 65 brother. Don't let "life style" specialty concept or Surg stereotypes get in your head. Bottom line is when you do the one that is going to challenge and motivate you everyday won't feel like its work and you'll find your people within that group. Any of those specialties you can hang your hat on and be extremely proud of! ( I've been watching your vids the last couple years while I've been in surg tech school and its been awesome to watch your journey, i'm graduating in may and now planning on taking a couple classes i'm missing for med school and applying next year myself. Had this idea just pop up, hear me out: Have you ever seen the movie “When Harry Met Sally?” There is one scene at the end where harry is walking by himself, alone. He recently had a fallout with Sally, his dear friend and romantic interest. It is New Year’s Eve, and they are at a disagreement. As Harry walks along in the cold, dark, street by himself, he reflects on his choices. When the epiphany comes about his certain love for Sally, he runs with every myosin filament he has. When you have a day off, journey on foot as far as you can for the entire day. No Uber, no phone distractions. Just you, your mind, and your dreams. whoa that's quite the spread of specialties 😂 you ever consider plastics? as Dr. Brown has said, they are psychiatrists with a knife when they get consultations for aesthetic body procedures (breast implants, BBL, etc) since 90% of them are body image oriented and do not require surgery. At least the good ones will promote non surgical correction before offering procedures

I’m in a parallel position right now. In my last year of pharmacy school and have to apply to residency in a month. I would love to do peds onc in the end but an adult onc second year residency with a peds rotation might also for that requirement. Two completely different paths if I choose to go into adult hospital vs childrens and end opportunities may look completely different as well

Random non-medical comment based on the end content: I didn't know you got any type of nutrition education in medical school, and the cube rule is completely new to me. In fact, I went back for nutrition courses as part of my post-bacc as a non-trad pre-med because I got the impression that nutrition is not a priority in modern medicine.

Also, chicken pot pie, as hinted in the name, is definitely NOT a casserole unless you choose a lazy crust that is more like a crumble AND you layer the fillings. Using any form of a fully combined and rolled crust meets the criteria for a pie as well as using a pre-mixed filling that is ready to pour in.

In my school nutrition used to be taught as a mini course under integrative medicine but it got taken away for various reasons so now it's lumped into what we can see the curtains are with metabolism in biochem. But now in rotations we have a program called health meets food that we can do in person or online via zoom classes and teaches us about clinical nutrition and catering to certain populations. It came out of Tulane and specifically is for doctors and is a cool program. I wish other med schools had it because it's so important.

Using your cube rule of food, a pot pie is round and is not on the chart. Therefore, a round pot pie is inedible. If the pot pie is cooked in a square pan, then it would be a casserole--or in the terms we use around here--a hot dish. :)

ENT Resident here so I’m biased. Ever consider Pediatric Otolaryngology-Head & Neck Surgery? Itch for emergencies? We do airway emergencies and emergent neck masses/abscesses/infections Anesthesia? We do airway as a huge portion of our training and work closely with anesthesia daily Peds? Well you basically do all things ENT for kids Adults? You can stay in a general practice and just practice complex Peds Surgery? Well it’s a surgical residency and you can be as busy as 4 OR days in academic settings or as non-busy as no OR. Also, ENTs don’t typically fit the classic malignant surgical stereotype. Idk something to consider. Many blessings to you bro

Just think of it this way: *it's impossible for you to make the wrong decision.* The worst case scenario is you realize "Meh, maybe the other thing would have been slightly better." Once you are in and working it won't matter if you are a pediatric toenailclipper, you will be working where your HEART has called you, and you will be amazing and the best possible doctor you can be ANYWHERE.

I think its super unfair y’all have to choose a lifetime career with so little exposure or time to reflect. One of the reasons I chose the PA route is that I don’t have to choose. @9:40 you state that you have some disagreements with the culture of surgery. This can be a precarious statement to make on this social media platform which can hinder or even halt progress in your future goals. I suggest you bleep this audio segment out of at all possible for your own benefit. If a hospital hears that from you, there may be a hurdle they may not want to risk with you. Good vid.

Go for emergency medicine....more interaction with people, which it sounds like you want .....plus I've watched RU-vid videos where er docs still do minor surgery. ( I'm just a nursing student though so feel free to ignore me. Just seems surgery and anesthesia might have less interaction than er)

So I guess "Pediatric trauma surgery" it is💀. I know it sounds like a first yr student wanna be Pediatric Cardiothoracic Neurooncologist Neurosurgeon but looking at your whole situation you"ll deal with kids who have acuity and require surgeries. The only caveat is its gonna b general surgery for 5 yrs , pediatric surgery for 2, and Trauma surgery for 1 yr in a total of 8 yr after med school without taking any gap yrs 🙂🙃🤧

Pediatric urology is pretty bomb. Daughter is applying to med school and I suggested this specialty bc my son was born hypospaidus[sp] and there was only 1 ped urologist in a super metro area. He treated my son and friends kids for various issues. It’s ped’s and surgery and office hours. Just a thought. You’re doing great and I love watching your journey. Fact…my daughter sub to your channel and if she reads this comment I will get MAJOR eye rolls😂🤷‍♀️🫣
