I presume that had this happened today, they would've done tests to see if those types of cargo items really could ignite each other during transport conditions, and if so, if it happened easily or not. Otherwise, maybe one of the crew was a smoker, or perhaps a smoking passenger was a bit careless one day. Drop one or more lit matches in the wrong place, or fail to make sure that what's left of your cigarette/pipe tobacco really have stopped burning... Other fire accidents have started that way.
I am not sure if this was the case on the Khiva but I know that on some of the ships carrying people to Jeddah they also had cooking fires on the deck, which were responsible for at least one terrible tragedy. SS Sardinia also had a very flammable cargo, in that case nitrate, and it was thought in that case that the cooking fire ignited the cargo.
Fires in the coal bunkers were usually the result of chemical processes in the coal itself causing spontaneous combustion. Coal bunker fires could smolder for long periods of time undetected too until enough coal was shoveled out that oxygen would get to it. Basically coal was all around nasty business for shipping and there's a good reason everyone went to oil ASAP.
