Defs would normally never question you because you are one of the best poe players in the world but like... your recombinator crafting was cooked. You are supposed to go for two T1 prefixes on two items then recomb. It's incredibly easy to hit if you do that. Basically you recomb a fire/cold with say a fire/light or a cold/light. Not sure of the odds, but it's far better than what you were trying to do.
