I have not heard a podcast this straight to the point in a while. Thank you both for bringing very rich content to this platform and for the health / bodybuilding community.
This has been one of the most informative podcasts I’ve listened to in a while! Your dynamic with Adam in this was electric and the flow was perfect! I could have listened to you both talk for the whole day!
Never have I ever listened to an entire 2 hour Podcast!! Doc H is one of my favorite humans this is my first time listening to your show you’ve got a new sub.
Absolutely one of the most informative podcasts I have ever listened to. Thanks to both of you for a very professional presentation. I would love to find someone in Ontario Canada that would be comparable.
Congratulations to you both! What a pleasant, well-balanced analysis! Unlike the typical commercially-driven TRT screaming. The LDL comments were spot on, too.
Clarification: there are studies showing aggressive systemic suppression of cholesterol production with high dose statins does, in fact, decrease sex hormone production. Aggressively lowering LDL with PCSK9 inhibitors-which do not suppress intracellular cholesterol production-has no impact on sex hormone production. This is important nuance.
www.ncbi.nlm.nih.gov/pmc/articles/PMC3621815/ The studies in this meta-analysis each have their limitations, but I think there's enough of a signal to suggest that T suppression can occur when statin doses are pushed. The online statin debates often fail to distinguish between suppressing cholesterol production and suppressing circulating LDL levels. Cholesterol in every cell is essential; LDL in the blood is not. Cholesterol plays important roles in sex hormone production and brain health; LDL does not. This distinction, in my mind, makes PCSK9 inhibition preferrable to statin therapy for treatment of hyperlipidemia, especially when taken long term.
By scientific definition, testosterone is absolutely a steroid. It is an organic compound (meaning containing carbon) with 4 fused rings and is a signaling molecule. That is the literal definition of a steroid.
After 15 minutes I discredit this guy. I like most of what he says but he is also incorrect a lot. DHT is not anabolic to muscle tissue. It is metabolized by 3 alpha-hydroxysteroid reductase in the muscle deactivating it. That is why there is no bio identical DHT injection or cream because it has no use. That’s why modified DHT versions of anavar or primobolan were invented. They are not deactivated by the muscle tissue. Studies have also proven that finasteride use does not affect muscle growth from testosterone.
You know what, I thought he knew something about DHT that others didn't. Yes I understand that DHT is only used when in the womb for males and during puberty. Otherwise DHT enlarges the prostate and kills your hairline..I'm glad I read this post thanks
when do the benefits of trt out way its potential negative effects on long term health. would it be when one transitions from their 50s to their 60s, or even 70s that they cannot maintain an appropriate testosterone level naturally that trt may be a viable option??
Such a good question that I've never been able to find an answer to. Is there a reason or testosterone naturally declines with age? If there is, are we harming men by keeping it high in advanced age? I haven't seen that truly answered. Anecdotally, I've seen if men stay healthy (low body fat, insulin sensitive, low inflammation, active) they're able to tolerate having high normal levels and it seems to potentially have a positive impact on health parameters. Where it is always problematic, at every age, is when we take a man with poor lifestyle, who has a body actively suppressing testosterone production, and we fill them with high levels. Its like putting premium fuel in a beat down old car. I truly believe TRT is best for healthy men and steps should be taken to improve baseline health before ever considering it. Often times that auto corrects it anyway.
The moment you feel all negative symptoms of testosterone deficiency, you go and do your lab test, I am 47, but start to feel symptoms few year before that, wasn’t aware it is that, thought is related with too much work, other life related issues, then try to fix it by more training , but no effect, long recovery, feeling beaten, nervous…started TRT, felt better from start, regret I didn’t started earlier, planing to stay for all life, negative doesn’t exist, only negative exist with abuse of testosterone levels like everything else in life
@@EarlofGreystoke that’s something a lot of men experience. Most don’t want to get help and just tough it out, eventually bad becomes normal and men just deal with it. Sometimes I see a guy and I’m like “I know you think you’re fine now, but I think you’ve felt bad for so long you just consider it fine and you won’t realize it until you fix it”
@@EarlofGreystoke increased risk of prostate cancer, is the big one for me. I'm 49, bike and run a lot. As long as I can keep my levels above 500 I'm going to avoid it as long as i can. Having a normal.bmi, doing high intensity training, weight training and eating a plant based diet might get me pretty far in age before even considering trt. I see it as a last resort when literally everything else fails, nor a quick fix.
@@DrAdamHotchkiss completely agree, first deny , then struggle to accept, which lead to even more mental issues, ruin relationships, families, avoiding friends, taking bunch of short term “ natural” boosters which are just money wasting … and then one day hopefully decide to check levels, do their research , learn all about it and start TRT and feel all the benefits snd life improvement 😊
Adam with a misdemeanor lol 😂😂 that aint nothing. Pretty much everyone has 1 of those. I have 1 E felony conviction and did a year in jail behind it. So trust me you aint that bad. I battled opiate addiction for 8 years. Ive been clean for 41 months.
vitamin t is just only one part of the puzzle... if theres no training and the food intake is not on point... its pointless... not even talking about cases of enlarged prostate, cancer cells in body or high blood pressure ....
Thats not cool talking ppl down from TRT. What if they really need TRT and it is beneficial. Thats just totally wrong. Im not for pushing TRT on ppl either. theres nothing wrong with TRT!!! I reccomend it if someone has low t. So not cool.
100%.. meaning here talking people out of it WHO DONT NEED IT, obviously if they need it I'm not talking them out of it. But these days 18 year old kids with an 800 total test will see ads of jacked influencers repping a TRT company and they think they need it.. that's what I meant.
After 30 your test starts declining. I’d say every healthy guy over 30 should have bloods done every year to monitor it and their mood. I’m 39, got on trt 4 years ago. 250 mg test a week. I’m at very high end of normal, estrogen perfect, psa perfect. Blood does get thicker and give blood every 10 weeks. I’m O+ so it’s nice my blood is helping save a lot of people too.
@@garycarnivore833 everyone is different, I’ve noticed you can’t have caffeine etc or anything that could elevate your heart rate but it’s all person to person tailored which is the dialing in phase
You guys mentioned HRT for men with symptoms of low T. For aging men in their 50s, how do we test for erectile dysfunction in relation to low T or cholesterol levels for causation. Doctors don't really test much and prescribe a pill. My cholesterol is not too high, healthy weight, non smoker, good blood pressure.
Great question! I'd refer to my good friend Dr Twyman who's a functional cardiologist and examines this exact thing! He has a lot of testing modalities and therapies for ED secondary to cardiovascular disease. I'd maybe start by looking at some of his work and then see if there's someone in your area doing similar things.
at 66 my T was (oddly) 350. felt good, no ED, run 6k/day, cross-fit .. super healthy .. got TRT (again after a break .. pellets). T now at 1100 .. feel freekjn fantastic .. i had forgotten how “youthful” i felt on TRT. really went back on to ward off sarcopenia .. sexual desire massively ramped up .. but at 66 .. can easily put it to one side.
@jp7357 - What do you mean "pellets?" You mean TRT injections? I am your age and will be starting TRT soon. T = 292 ng/DL ...., cyp or T compounded cream is available never heard of pellets.
@@JohnPiperBoots no. definitely "pellets" -- they put 12 or so pellets in your a** and they deliver a constant "drip" of T for around 5 months. The injections haver been shown to thicken blood and that the last thing I want. costs around $700 def more expensive that shots but so much simpler and no highs or lows. my T went up from 220 to 375 when I went vegan and lost a ton of weight .. now - 4 months in and its around 800 ... I run trail 4mi/day and do crossfit 3d/week ... can definitely see muscle mass being put on ... not looking too bad for an old guy. Even have some abb definition. I also feel around 40 ... most def one of the benefits .. tons of energy, sexual desire WAY up. All good. Would strongly recommend pellets - I found the injections (weekly) too variable. biote.com/bioidentical-hormone-replacement-pellet-therapy/pellet-types/testosterone
@@JohnPiperBoots I actually do mean pellets … they numb your rear end, insert 12 or o slow release T pellets. You get a nice steady T infusion over 5 or 6 months. No highs or lows, no blood thickening effects from the injections which I feel is importantly as I don’t need any ASCVD enhancers. My T went to 1500 after about 2 weeks then leveled off slowly to around 850. I started off t 350. Prior to going plant based I was at 220 I exercise like crazy .. run 4mi/day and do CrossFit .. this “may” accelerate how quickly the T pellets dissolve .. I like the constant “flow”. I have used the cream but this would get onto my wife and kids (girls) which after an episode of “House” I stopped immediately.
@@JohnOconnor-ju6gb that was one reading at one point in time. Can’t make much from that. I wouldn’t go based simply on the numbers, like we talked about at length here.
@@DrAdamHotchkiss ok thanks those numbers are from last week So if you don’t go off numbers How about if I just want a boost for r rest of my life regardless of how I feel I feel ok but not like I think I want or feel So those reasons would you say it’s worth giving a try to see if my body likes it or should I just carry on This is my dilemma
@@JohnOconnor-ju6gb I’d recommend finding a good team to work with who can help you with that decision. As discussed here, TRT is not a miracle and often not as great as people hope it is going to be. Taking a normal testosterone and making it a higher normal is often not noticeable. Now you’re dependent on an injection that isn’t helping.. that’s what you want to avoid.
Normal Physiologic according to who Dr Tom Travsion lol. Wow this is more Endocrine Soc nonense and wait coming from a foot doctor who had 15 mins of education on Test lol wow this is really funny. Next time get a real expert in hormone replacement like Dr Morgentaler or Dr Dean St Mart this was really funny...
I don’t think you listened to the podcast at all. I argued against “normal ranges”. I have no idea who Travsion is but I consider Dr St Mart a friend and agree he’d be an awesome guest!
I do like and agree with most of what he says. However I suggest staying away from subjects you don’t know much about or have enough information rather than making assumptions or bro science. Parabolan was the human prescribed form of trenbolone in France for almost 3 decades.
