Towing vessel and barge crash into swing-span bridge, causing $2.9 million in damage

On November 14, 2020, towing vessel Island Lookout was transiting eastbound with a crew of four on the Albemarle and Chesapeake Canal, pushing ahead the 295-foot-long barge BH 2903, which was loaded with scrap steel. As the tow was attempting to pass through Centerville Turnpike Bridge, the barge struck the swing span of the bridge while it was opening. No pollution or injuries were reported. Estimated damages amounted to $34,000 for the barge and $2.86 million for the bridge. The bridge was closed to vehicular traffic for more than six months after the accident. Bridges are expensive.
​The Island Lookout mate was at the helm at the time of the accident. The mate told investigators that he called the bridge operator four times before he received a response. According to the mate, he slowed the towing vessel’s engines after each unanswered radio call. The bridge operator stated that after receiving the call from the Island Lookout, he began the procedure for the bridge opening. The total time required to open the bridge from the first operator action to fully opened span was between 3 minutes 30 seconds and 4 minutes 30 seconds. By the time the bow of the barge was about 175 feet from the bridge, the tow’s speed had reduced to 2.8 knots. While it continued to slow, its momentum carried it into the bridge, eventually hitting the bridge before it could fully open.
The NTSB determined that the probable cause of the contact of the Island Lookout tow with the Centerville Turnpike Bridge was the mate’s misjudgment of the tow’s speed of approach relative to the status of the swing bridge opening, which resulted in insufficient time to slow the tow and avoid striking the bridge before it was fully open and safe to navigate.

For me, “What You Haven’t Seen” is a brilliant title and attracted me to the Channel in the first place. But of course whatever WYHS decides to do will be fine. One word of caution - just make sure there’s nothing “click-bait-y” in any future renamed version. That goes without saying, because I know you’re too smart for that.

Looks like an interesting case caused by human factors. Bridge operator was slow to react, and tug crew didn’t slow in time due to wanting to avoid inconveniencing the bridge operators.

To me, the accident was caused 90% by the bridge operator, 10% by the mate of the tug.

yup, sounds reasonable to me. maybe even like 95% like the 5% on the boat being that, well after 2 unanswered calls it might be reasonable to assume that the guy isnt paying attention and you should cut engines then instead of lowering them.

What if the bridge never opened? Would it be OK for the mate to pilot the boat into it if it remained closed? It's up to the pilot of the boat to avoid obstacles when possible. They are in control of the boat, and in this case, he was going too fast.

@@OMGWTFLOLSMH The bridge operator was far too late in acknowledging the radio messages and opening the bridge. When he finally woke up, he apparently gave the tug a signal or message to proceed, or 'bring it on' before opening the bridge. If there had been a problem with the opening, he should have warned the mate of the tug at a much earlier stage, (or delayed giving the 'proceed ' message), so that the mate would have had more opportunity to stop the vessels. In order to maintain steerage way, the tug and tow have to maintain a certain speed. To stop, he would have to had run the engines astern, which meant the stern would swing to port, (as happened when he was near the bridge, resulting in a danger of contacting some yachts.

Nope, it's the same as driving an automobile, if you hit someone in the rear, you're at fault. It's your job to avoid the Collision. The bridge operator has no control of what the boat does

"making the transit 'a thousand times'" well, would you believe 999 times? totally the mates fault. i think he was the victim of his own previous successful runs. lulled into a false sense of complacency. the bridge operator may have been distracted, but you have to make allowances for real world events. the mate didnt and cause millions of dollars of damage.

Who explains this? The level of detail reaches academic levels up to almost an exact level of scrutiny and description of the device and the place. This is beyond professional work. This is a full report by a body of engineers. This level of explanation is a literal work of art. I haven't met a channel that describes up to the decimal every inch of these events including the circumstances. The channel deserves an award for the extensive documentation and consequences of these events. This is literally a format that many other channels should follow.

Main rudders are for steering while moving forward, flanking rudders are used while moving backwards.

It beggars belief that a very highly experienced professional mariner would cause such a catastrophic incident on such a peaceful waterway! To have so misjudged the time that the bridge required to be fully open to vessels and to have powered his barge into the still opening bridge, the acting Master absolutely failed in his professional duties due to his recklessly negligent judgment. The damage to the bridge bears testimony to the huge forces unleashed during the collision. It is almost as if the barge changed course in order to strike the bridge at the final possible point...
