i dont really understand how he would die hours later and not instantly since knives arent really a thing where they would kill you hours later after you get stabbed unless you are slowly bleeding out

How kakyoin died: Option 1: He was killed because he was wounded from the knifes DIO threw at him before he died, leading to Kakyoin bleeding out on the plane. Option 2: On the plane Kakyoin got sad about the death of Iggy and Avdol so he killed himself by either jumping off or jumping into the engine.
My theory on why kakyoin just suddenly died on the airplane was due to fate, you see, fate in jojo is very important and set in stone, and kakyoin's fate was to die in the hands of dio
If you watch closely, DIO throws knives at Kakyoin before he dies. That is likely the reason for Kakyoin's fatal injury. And yeah, it was likely fate taking hold. Even if Kakyoin had reported his injuries, he was probably destined to die of it no matter what.
He could’ve been hospitalized before they stepped on the plane if he really wanted to be healed. The simple fact is that his fate is always death at the end of part 3, even though he beats DIO here he refuses medical treatment knowing his wounds are probably fatal. Jotaro even asks him again if he’s okay and he says he’s fine and doesn’t need any help, he wanted to die and be with iggy and Avdol.
