The Apexum procedure is a new approach for treating periapical lesions. It is designed as a complementary step after standard root canal treatment.
Periapical lesions are found in 20-60% of all dental patients At present, Periapical lesions are treated by conventional root-canal treatment only, without directly addressing the issue of the lesions, specifically.
Clinical studies show that the bony crypt heals much faster when using the Apexum Procedure than after standard root canal treatment, alone.
As published in the Journal of Endodontics, the studies demonstrated that using the Apexum Procedure in conjunction with standard root canal treatment resulted in 95% healing of periapical lesions within 6 months -- a result unattainable with standard root canal treatment, alone.

If you cut the vascular supply of the canal don't you need the remove the contents as to avoid aseptic gangrene ?

This is a solution looking for a problem! The periapical lesion resolves after the rct. What's the point of poking into a PA lesion and torturing the patient? Selling a few kits?

***** Yes, really. I'd be more than happy to talk to your patient. I would tell him/her that an unresolved PAP is an indication of inadequate rct that can be due to a missed canal (do a CT to verify,) inadequate irrigation/other bad techniques during procedures, and multiple other factors. If anything, an unresolved PAP is telling you something; it's telling you something is off here and needs more investigation. Cleaning out the PAP alone is pointless. If rct is done well, PAP resolves, if it's not done correctly, it remains or comes back even if you clean it out using this method. So I ask again, what's the point of torturing patients? And no, I have no objection to people making money if the product is useful. The thing is that I really don't see any usefulness here... I appreciate your passion though. We just have to agree to disagree. Cheers.

Truth Seeker the situation u are talking about is rarely we see in our patient unnecessary js for the sake of fast treatment one should nt give his/her patient such torcher n risk too of treatment failure

This is very smart! What are alternatives to gutta-percha that maybe better to seal the tooth, and for allergies? What if the tooth has no pocket of absence, but flows to the surrounding are is infected?

Elly Potamianos bioceramic sealers form a good bond with the tooth surface and is very biocompatible as the final compound it forms is hydroxyapatite . although we can use it as canal filling material without gutta percha but that would make retreatment quite difficult .

Apicectomy on molars = difficult! And RCT is needed prior apycectomy anyway. I am a bit concerned about to how to determine the lenght of the blade as to the size of the lesion. There is need of CBCT as Radiograph will not give us the actual size.

This procedure makes the healing process faster and it is not necessary. All periapical lesions heal after the root canal. it is just a matter of time.

The problem is diagnosis. Cyst or not a cyst. Filling with periapical cyst (which can be smaller than size classification shows) is not recommended - the exudates of epithelial tissue from Melassez rests will result in positive pressure of liquid and further osteolysis. We can use CaOH2 to diminish epithelial tissue activity and stimulate alkaline phosphatase for bone regeneration. Dry canal is some kind of sign that healing started.

No point! Apical lesions are inflammatory and resolve after accurate RCT. The risk is definitely not worth the risk.

Nice but 1.what about the cystic membrane of the lesion....does it disappear???? Reoccurring??? 2. The wire rotation is very high so how can we prevent brakes of the wire in the cyst 🤔???

@LaLLudo I back the science behind this procedure. An apical lesion will heal itself but you have to understand the length of time it takes for the restitutio ad Integrum to take effect. The danger in this procedure is nominal when weighed against its benefits. I do this procedure almost once a month with no complications. I would never use you as my dentist.

Even i am a practisng dentist ...........from India.how come...i mean ,anyone think of giving so much pain to a patient.whatever the treatment plan is ,i personally feels that always try to maintain the patency of apical foramen......a good root canal serves most of the purpose...

@e30m3drift : Im a final year dental student in australia. From my limited experience and understanding removing the tooth then cleaning out the empty socket gives a more predictable result. Root canal treatment is costly, and theres so much more procedural error and complications from it. A root canal is the last step before tooth needs to go out.

I fail to see the point in risking any more instrument fractures by rotating NiTi-filaments at 8000rpm periapically - for what purpose? Liquifying and sucking out a granuloma under massive pain instead of counting on a succesful endodontic elemination of its source and letting nature do the rest, seems a little bit overdoing it. That's an "Attaboy" to our US-collegues! Greetings from Germany

It appears that our Australian Dental School educators have some serious work to do with you. Proper Endodontic therapy (NOT using this device) requires dedication, skill and time. If you do not have any of those three requirements, I strongly suggest you refrain from slagging the procedure and refer your cases to someone who does. Either that, or transfer to barber school where your knowledge and talents will be better applied.
