I always thought the tube enters the oral cavity with the bevel facing left, then turn it 90def counterclockwise to follow the natural curvature of the oropharynx, pass the epiglottis, if the tube got caught at the glottis opening, withdraw and turn clockwise( so that the tube won’t obstruct the view) and advance to avoid the posterior cartilage on the way. I know some people suggest turning the tube counterclockwise when it got caught going down a bougie. That makes sense if the tube is caught by posterior teeth or the pillars. At the glottis opening, it really doesn’t matter whether you turn right or left to avoid the posterior cartilage as long as you can maintain your view. My 2cents
But if our ET tube during direct laryngoscopy is so far inside the trachea, then why not just pull out the stillet, push the tube marking past the glottis and inflate the cuff?
