The script is working without errors on my side. If you are using xampp put the code in your htdocs folder and create database.

but you haven’t show if it is stored in mysql database you only show when it is transferred to a folder destination

the image is not stored in the database, it stored in a folder and the image name is stored in the database as a binary large object (BLOB)

are you getting any error? the image is supposed to go to a folder. and image name is stored in the database.
