We don't perform augmentation for upper extremity venous dopplers in my hospital. But you can have them squeeze their hands or forearm muscles to help you augment.
It depends where you work, protocol differ from place to place. In my job we have to start in the distal IVC and work down to the common and external iliac then down to the common femoral. Where I used to work we began in the common femoral vein...
