Wanda is a witch. As a child to stop the 2nd stark bomb from exploding, she unconsciously used a probability spell on the bomb shell which caused it to be defective. The mind stone was in the blue container of the staff. When wanda came into contact with it, not only did she absorb some of its power, it amplified the witch powers dormant inside of her and made her into the eligible host of the Scarlet Witch energy/entity which is creation/chaos energy from the big bang.

The Mind stone which is yellow which is in Visions head, is yellow, it was encased in something that made it appear to be blue. Similar to how in Infinity war, Thanos broke apart the Tesseract to get a stone, but that one was the Blue Space stone, Which Loki got a hold of in Endgame.
