When they arrive at hilltop for the first time some random guy there attacks Rick so he stabs his throat, spilling blood all over himself resulting in everyone staring at him, but his calm reaction is somewhat justified because they all saw him getting attacked by the guy. The moment you mean is much earlier in the show.

@@djwawrzus7357 they were supposed to bring to the Saviors the severed head of Gregory. One of the heads looked really close to his except the nose. So Rick went ahead and punched the nose to make it look messed up so the Saviors wouldn't tell the difference.
