I worked at McDonald's for 3 ish years. There's no such thing as a secret menu, lol. The "secret" menu is just customer/fan creations But if you're patient, friendly, and clearly say what you want, they'll probably do it for you.

it doesn't "take longer", when I worked on the assembling station these custom orders were a godsend, as the ingredients were lsited on the sticker you put on the box with the custom order, so I didn't have to memorise all the stuff I have to put in a normal item. I tried to memorise the secret menu back then just in case someone came through asking for it, but nobody there seemed to know what they were, though there were local popular customizations that were known by particular names. so I'd say unless you know the crew in question, describe the item's customization rather than saying the colloquial online name, and don't feel bad about asking for customizations, they genuinely are not that much of a hassle.
