Great video, thank you!! We're in the very strange, rare group where our IVH was just detected *before* birth and at 37 weeks gestation. Baby is measuring just under 6lbs, so he isn't growth restricted either. There's a "mild to moderate" bleed in the left ventricle with "some" dilation. I haven't gotten my blood test results back yet, but could you do a quick overview of thrombocytopenia? And outside of that, why else would this happen now? We had a previous biophysical profile done at 33 weeks and everything looked great, so we know it happened in the last 4 weeks. We were just monitoring since my first babe was IUGR, that's how we ended up finding this.

Hello, I'm so sorry you're going through this. Obviously I don't know the specifics, and I can't give medical advice, but will answer a similar hypothetical situation. Most often when there is an IVH in utero, it is because of low platelets in the baby. Since it is your second pregnancy, the highest risk here would be NAIT (your body is producing antibodies against the baby's platelets and destroying them). This normally slowly corrects itself after birth, but efforts will need to be made to ensure further bleeding is minimized. We wish you all the luck in your pregnancy, and for a fantastic outcome.

Thank you, My baby was born 25weeks 2days. I was able to get both shots for his lungs, and NICU medics were ready for him. He has and IVH. They said he had on on the right side(level2) and one on the left side(level4) they ran a second ultrasound turned out he only had the one on the left side and almost no bleed on the right. He has no other issues besides the IVH on the left side. And have high hopes it can clear the bleeding. I had questions that you have help me understand better now. Thank you !

Yes Yessica- with IVH (I-III) the hope is that it 'clears out'. It is so difficult to predict an infant's future outcome- the fact that the bleeds mostly cleared- or weren't there in the first place (?) is a good sign. Many other factors come into play though, too, in development. Good luck XX

I really need help, my baby has a grade 3-4 ivh ventriculomegaly (17mm) on left side and severe hydrocephalus.. neurologist can’t really say affected he will be, is there any quality of life for him?

from what i understand, the treatment is mainly supportive which is draining the excessive accumulation of CSF right?then what about the bleeding..do we do anything to stop the bleeding or what about if baby developed severe anemia?is it supportive also like giving blood transfusion?

Hello Natasha- you're completely right- the treatment is supportive. So supporting blood pressure, and transfusing and supporting the apnea if that develops. There is little to do to prevent the baby from bleeding further- if the baby is coagulopathic (i.e. PT, fibrinogen, platelets etc. are all off), then we may transfuse blood products as needed. And then it's up to the baby's body to absorb that blood- we don't 'drain it'. We just use drains when the pressure is too high, and some of that CSF needs to be evacuated. My grandson is 28 weeks in utero with a grade 4 intracranial hemmorage in his left frontal lobe. What will be his prognosis? Can this heal on his own? A few articles mention it being fatal.

I’m so sorry you’re going through this. It’s unusual to have a bleed in utero- unless there’s a bleeding issue like with low platelets. (Eg NAIT). It’s so difficult to ever predict anything but I think what we have learnt in the last decade is that outcomes are better than what we would have expected- especially with excellent therapy. Grade 1 bleeds don't need treatment and they usually resolve. We don't usually get ultrasounds on term babies unless something else going on- so obviously I can't add a lot here!!!!

Bleeds rarely happen before delivery. Normally something very abnormal has happened (e.g. infant has very low platelets- usually secondary to NAIT, or a severe trauma, or infant had an AVM, or a tumor that bled or some sort of thrombosis). These all are different etiologies with very different outcomes. So in this case the underlying cause would affect prognosis as much as the size of the bleed. We hope this helps?

Need help. My baby had a grade 2 IVH when born (27+6). When being discharged from NICU after 2 months, the doctor's report said "Grade 2 IVH has resolved. No ventricular dilatation". But when they did the scan 1 month after discharge, they said there is dilatation observed. Now the latest scan taken 3 months after discharge says, "Bilateral ventricular dilatation V1 R1.56, L 1.66. Third and fourth ventricles normal Periventricular areas normal. No midline shift. Sulcal gyral pattern normal for CGA.' This scan was taken after 3 months from discharge. Can you please clarify what does this mean? Will this get resolved by itself as I am being told by the doctors? The baby is 5 month old with corrected age of 2 months

Hello Tyrell- great question! If an infant has elevated intra-ventricular pressure then really that pressure needs to be relieved as soon as possible. The most common device I have worked with is an external reservoir (that needs to be drained every day). (This is a small membrane attached to the ventricles, that you can pierce and take out 10ml/kg of CSF). There are other temporizing methods too- like a subgaleal shunt. Different neurosurgeons have different lower limits for when a VP shunt can be inserted. People were definitely more aggressive in the past (down to current weights of 1.5 kg) but now more studies are coming out that show the later the VP shunt is done (giving time for the brain and skin etc to heal), the more successful the shunt is (i.e. needs less revisions, less chance of infection etc.).

If I may, I have another question that is in conjunction with the first. Is there a percentage of patience that dont need vp shunt after the intraventricular catheter system is ready to be removed. Also are these procedures supposed to be offered to us as parents or is it something we have to inquire about at the hospital. Are doctor's allowed to deny your baby these procedures.

Hello Tyrell. Yes there are definitely certain patients who outgrow the need for a shunt (they need drainage less and less and so there becomes no advantage to placing a VP shunt). I don't know the percentages, but by my experience I'd guess around a 1/3 of babies with external devices don't end up needing one. Placing a shunt or drainage system when a baby needs one is standard of care. If doctors are not ready for the procedure it is either because the baby's brain does not definitely need one (i.e. need to show ventricles are continuing to increase in size) OR that the baby's body is not ready to tolerate a procedure (e.g. infant has sepsis or is very sick from a cardiac/ respiratory standpoint). Does that answer the question?

My Baby was born at 25weeks, and was diagnosed with a grade 3 and 4 IVH, thankfully the bleeding stopped soon and is now reabsorbing, the ventricles don't show any sign of inflammation, it all seems it's decreasing slowly but surely. My question is, does the time it take to reabsorb affect his brain tissue more? Or as long as it's reabsorbing is good? Also from your experience, what signs does a baby with a disability in the future show now? My baby will be 2 months and still in the nicu, he has passed his eye exams, he has very good movement activity, he is able to lift his head, move his legs and arms incredibly fine. I'd appreciate your opinion on my baby since it's been a very hard situation😢

I’m so sorry you and your baby are going through such a tough time. I really can’t comment on exactly what is happening (and even if I were taking care of your baby I wouldn’t be able to predict the outcome). All I say is that it has been absolutely shocking seeing how well some babies who have had bad bleeds (and even shunts) have done. In their right grade at school and with minimal physical disabilities. So keep hoping and pushing and try to stay optimistic! With love xx

As you know, all units try to keep the head midline the first few days because of the suspected increase in risk of bleeds. (Physiologically, this make sense, since moving the head can change venous drainage as well as arterial flow- and affect dilation etc). Like everything else in neonatology, this is almost impossible to prove! We do know that units that have bundled positioning with other Golden hour stuff have had decreased bleeds- but not necessarily only because of positioning. Also- we know that micro preemies that need to be transported have a much higher risk of getting an IVH- presumably also from all the moving/ traveling they need to do. So yes- it would be reasonable to conclude excess manipulation increases the risk of bleeds- you're just going to be hard pressed to find a study that proves it definitively!

And the noise level...the AAP recommends keeping the noise level below 45 dB for over all development. (It's louder than that if you bang on the incubator). Again- it would be very hard to prove loud noises result in bleeds- but think about your pulmonary hypertension kids- we do everything we can to decrease stimulation because they desat so easily. It wouldn't be a stretch to think loud noises would have the same effect on the vascular bed in the brain. Or when we admit new babies- with all the bustle of new lines etc- often nearby neonates will have a few more desats, likely in response to the noise. Not convincing evidence, but enough to make us want to keep a quiet environment!!!! Thanks, as always, Emily

@@emilydelossantos4097 absolutely. A fairly common cause of CP (which is a non-progressive brain injury that affects motor function). Generally- the higher the grade the higher the risk of CP

Hello Andy- great question. A grade 3 implies the ventricles are dilated and there is > 50% blood in them. The various cells in the brain break down and absorb the blood. (Hopefully). The ventricles may stay a little dilated, but as long as the brain is growing normally and the ventricles are not under pressure- then we just follow this. Does that help?

That’s a great question. Enough irritability to cause seizures is probably days to weeks- and blood is resolved in weeks to months. But there’s still lots of remodeling and growing to be done- so can be years before babies get over initial insult

Usually it takes a few weeks for the blood to be re- absorbed. (Sometimes this doesn’t happen and babies end up with a post hemorrhagic hydrocephalus and need draining)

The blood can get reabsorbed without intervention. Generally infants with a grade 1 IH have similar developmental outcomes to infants without bleeds.

Hello mam I am 27 weeks pregnant my babys ultrasound says that there is ivh with dilation . My blood group is negative and husband’s blood group is positive my ICT test also came positive. My Blood pressure sugar test normal and there is no viral infection as per TORCH test. Right lateral ventricle is 7.8 mm. Left lateral ventricles 13 mm hyperechoic mass lesion in the left lateral ventricle and extends to the peri ventricular space medially size is 36 x 19mm hyperechoic rim is seen along the brain surface in the subarachnoid space. Growth and weight of baby is normal . Please tell the grade of ivh and whether there is a possibility of survival of the baby

Hello KK. It is so difficult (and actually not acceptable!) for us to give any advice on patient care.If that is actually blood then it sounds like there is a grade3/4 on the right. Checking the platelet count in the baby would be important in a case like this. We hope you get the answers you want.

Hello my repeat ict test came negative and there is no fetal anemia. Baby born on 39 th week. Absolutely normal. Cried immediately movement and blood reports came normal. Growth is normal. MRI shows no damage to brain. The blood clot is in left lateral ventricle about 1.8 to 1.2 cm .

Is this condition by any chance genetic ? My first child was a baby boy who died at 8 months due to intraventrical brain tumor (malignated). He had to go through tapping every 2 days ut we lost him very early since doctors told us there is no cure for it. Please tell me if my future baby can get it too or not ?

We're so sorry for your loss. Since we don't know what the tumor was, we don't know if it was genetic. Most likely it is NOT genetic- and since all cancers in babies are incredibly rare, it would be very unlikely another infant would get it too. We definitely recommend you speak to your doctors about though. We wish you peace, luck and healing XX
