I think this video should be taken down because clearly there's a picture in the preview and I too was depending on pictures, seems not even Google is helpful and straightforward, so when this video promised pictures and what I needed, I too was let down by this dentist's Dunning-Kruger cognitive bias assuming we know what they're talking about without helpful pictures you, me, and everybody can check to see if we have a blood clot or dry socket.

Hi Dr! I got my wisdom tooth extracted exactly 6 days ago. Everything has been fine, but out of no where a little white/red-ish clot popped out. No severe pain, just a weird feeling. There still seems to be some protection in the socket, and there's no bleeding whatsoever! My post-op isn't until this Thurs, so i'm hoping you can give me some clarity to ease my mind before then! Thanks so much.

Hi there. I recently had a broken/infected premolar extracted. The extraction was a little complicated due to the brittleness of the tooth, so my wound is larger than I expected. I am in the middle of day four currently. I experienced a spike in pain during day 3. Around the same time, began to notice a strange/sour taste coming from the extraction site. My clot is still in place. I am on a 7 day round of antibiotics. I am concerned that I am developing an infection - mainly due to the taste I am experiencing, which seems to be worsening over time. So far, no fever, and swelling seems to be decreasing. Should I be concerned? Thank you for such an informative video!

If you’re on a proper antibiotic and the swelling / symptoms remain as they are I wouldn’t be too concerned. Certainly best to contact your provider sooner than later if you notice any changes. All the best!

Hey doc! It is normal for there to always be slight pain throughout the healing process of 8 weeks or so. I'm talking very slight every now and then such as after eating or every so often. Almost like a dull ache

The video was really informative thank you! I do have a concern though, I got my first molar removed because of a fracture and infection. It took a bit more work to remove my tooth so the extraction site is a bit larger than the photos I’m seeing online. How do I know my tooth is properly healing? My swelling has gone down and I am no longer bleeding but I can still taste a weird taste in my mouth. I’m now on day 4 and I still have to use pain medication. In the morning and afternoon post-extraction I feel fine but at night the pain gets worse.The area has a whitish/brownish gray thing with blood clot showing. Should I be worried or will the healing time just take a bit longer?

May - sounds like you're doing well from what I gather in your description. It's not uncommon for a difficult extraction to be painful for an extended period while healing. The good news is that the mouth is one of the most forgiving places of our body in terms of healing potential. It's always wet which helps and it's quite vascular. Assuming you're healthy and you don't develop fever, swelling, or an increased level of pain from here moving forwards, you should be ok! All the best!

#14 molar was extracted on 8/25/20 without complications before or after. It was an act of mercy as the decay had penetrated under the crown into the exposed root and nerve due to receding gums. The daily flossing was not effective in getting the trapped food out of the crevice. I lived in pain on heavy doses of ibuprofen, penicillin, and acetaminophen for 5 days. And receiving the second local anesthetic shot pain level was thru the roof! Almost made me jump out of the chair. BUT I would do it again! I kept the cotton swab in the socket for 5 hours, changing to a damp one every 40 minutes while biting down lightly and noting less blood on it as I continued down this road. Day 3 I started with a salt water rinse and had been in much less pain as healing progressed. DO NOT EAT ON THAT SIDE OF YOUR MOUTH. PERIOD! And eat very soft foods while the clot in the socked is becoming established. FOLLOR THE GUIDELINES OF THE ORAL SURGEON and you will get back to normal. DO NOT RUSH THE HEALING PROCESS!

no discomfort once the tooth and root were removed. Be very gentle after the socket clots as you DO NOT want to break the blood clot loose. No strenuous exercises, jumping, running, or any violent motion that will break the clot loose. Eat soft foods and no chewing on that side of the mouth. Most definitely purchase some table salt and fill an 8 ounce cup with warm, not hot water and add a tablespoonful of salt to the water to make a rinse that you take a mouthful and GENTLY swish the salt water in your mouth for minutes 3 times a day. This will promote healing and kill harmful bacteria that can cause infection to start. It takes 4-6 weeks for healing to take place.@@abraham5554

Hello Doc. I had molar #2 extracted a month or so ago (due to 2 failed root canals) I'm feeling a little better. Not much pain but my cheek is still a little swollen and it's affecting my right eye a little. Still feel a little pain behind the right ear and right cheeck. Should I be concerned of an infection? I was very prone to infection on that tooth prior to extraction. Does the healing take months? I just want to feel normal again. Tired of suffering for years now. Thank you in advance.

Xenia - this does not sound normal at all and I would advise seeking some care as soon as possible to find out why you're still having symptoms. This could certainly still be infection that is causing your discomfort. All the best!

Hey doc! This is day 4 of my 2nd molar extraction, I had 4 of them out up and bottom. Some weird white stuff has been forming over only one of the sockets but it's not really hurting. I'm also scared of food getting stuck in the sockets since its day 4 , when can I start eating normal foods and when should my socket close up?

Hi! I got a back molar taken out almost 6 days ago. I’ve tried searching for pictures to see what the hole should look like at this point and can’t find one. How far up should the blood clot be at this point? Like in the middle of the socket or at the very top? I’m paranoid about getting dry socket. I’m scared to syringe Because when I did it yesterday for the first time, some of the white came out and my hole wasn’t filled as high.. if this makes any sense. So I’m wanting to know how high the hole should be filled or empty and if I’m out of the woods yet with dry socket. Thanks!

@@shylosoulstar8068 I’m fine thank you! It was a while ago at this point. Thankfully I did not get dry socket, I was so paranoid about it because I stupidly googled it & saw how awful it is. But as long as you follow directions everything should be fine.

Just wondering about dry socket myself. That occurred this morning. It doesn’t hurt but it feels, different… idk if that makes sense

I had a molar tooth extraction yesterday. I am paranoid of dry socket as well. I am an anxious person and healing makes me more anxious and depressed. Oh did I mention that I have a three month old baby? So I feel tired all the time and I don’t know how long is the pain and soreness expected to last.

I have done some research on the healing stages after getting a tooth extracted but I would still appreciate an experts opinion. I just got a baby tooth molar on the bottom right of my mouth extracted 12 hours ago because the roots had been decaying and it was causing a bad odor whenever I would floss. My orthodontist had quite the time getting it out since it wasn't loose at all. Anyway, I don't smoke, drink or do anything that would hinder the healing process. I did as instructed and replaced the gauzes, the area did stop bleeding after about 3-5 hours but I never actually looked to see if a blood clot had formed. All I can see now is a mostly white filling in the middle of the socket area with a dried blood ring around the edges. A few hours ago I started swallowing my saliva once the bleeding started to slow down and I heard a popping sound in the extracted area. I'm wondering would the area possibly be infected, lost the blood clot, showing signs of dry socket, just the tissue forming very early or could it be the bone? Please let me know your best guess at what it could be. Thank you!

My guess is that it's just the bone under the tooth that was removed. Often retained baby teeth have little to no root left and can be fused onto the bone. This should fill in with tissue over time. Not sure what to say about the popping sound. I wish you all the best!

Hey ive had 6 teeth removed including a wisdom tooth removed 2 days ago, my sinus was perforated during the op. ive got some swelling in my upper jaw 14 stitches also, what colour clots should I expect and discolouration around the removal site? should I have a slightly yucky taste in my mouth?

At this point in time, a bad taste is generally not too alarming and swelling can persist for 48-72 hours. if swelling gets worse after that time, or you develop a fever, difficulty opening, or see pus coming from the site, it’s recommended that you see your dentist. what I tell patients at your stage of healing is that their symptoms should either stay the same or be improving each day. If that’s the case, generally you’re doing okay and all is well. I wish you the best!

I’m on my 4th day after wisdom tooth removal at the bottom left, the swelling seemed to go down and I have really no pain, I have a little stiffness when opening my jaw but I do have a bad taste in mouth. I rinsed with salt water and brush my teeth on the middle and right side carefully. But still seems to come back hours later. Is this normal?

Bad tastes are common and sometimes can be from food that is starting to accumulate and become stagnant in the sockets back there. Keep in mind that under the gum (even though it looks like a little hole on top) there is quite a void underneath. You could visit your dentist again and have them provide an irrigating syringe to help rinse the sockets. Bad tastes can sometimes be associated with infection as well so keep an eye out for any pus coming out from the socket, or swelling that comes back in the area. All the best!

Doctor- I am 43 days since #2 extraction. The tooth was pretty large with 4 roots. Extraction done by an OMFS. The past two weeks I’ve been feeling an off and on pressure in the extraction site. It’s not sharp pain but it’s really annoying and discouraging. I have to make an appt to see the surgeon but looking at the site it appears a light pink in the center (like a collagen appearance) and recently I’ve notice the edges appearing a bit darker

Yes, tissue that is healing will often have a greyish/yellow colour in the first few weeks.

@@SSM654 Heat, Pain, Redness, Swelling, Pus - those are things to watch for. Sometimes may develop a fever or a bad taste as well.

I had my most rear bottom molar (left side) removed 8 days ago. Now I have a lump on my lower jaw below the gum line. It isn't painful unless I push on it. Left several messages to the oral surgeon but no response. Not sure if this is normal or if it might be infected. I am very concerned about infection because I am taking Enbrel which lowers my ability to fight off infections. Any advice would be helpful

Hello! I would continue to try to get in touch with the surgeon, however if the lump is hard sometimes you're feeling the edge of the bony tooth socket under the gum. This often becomes more noticeable once the tooth is removed and eventually will smooth out and disappear. Hard for me to say, but what you're describing is what many patients say who have that same problem. All the best!

I hope you're still answering this but I'm on day 2 of ex 30 being removed and I have been feeling nothing amazingly but I did notice I have visible white stuff with I guess clotted blood in the tooth extraction spot. I'm not bleeding, no bad odor or anything. Is that still something to be concerned? Also Is it possible for dry socket not to hurt?

Steezy - Yes, all extractions are accompanied by some degree of pain. This generally should be manageable with over the counter medications. If it's worse than that, you should consider contacting your dentist and perhaps they can advise as to whether a stronger medication may be needed for a few days until the healing gets underway. So it's been 27 days and my socket still looks the same since the bleeding stopped. There are tiny whitish things in my socket but really small ones. Is this normal or am I not healing at all?

Orxl - sometimes there are small bone chips that will work themselves out from the socket. 27 days is a long time and I would expect the site to look somewhat improved from day 1. Probably best to contact you dentist. All the best to you!

If you mean just a couple days ago, then yes generally some yellowish tissue is normal and a sign of healing. If it's yellow liquid oozing out, that's a different story and can be a sign of infection. All the best!

Hello Doctor! I'm in some dire need of your help. I see this video is 3 years ago but I'm hoping that you might still be able to respond to me. Unfortunately I suffer with anxiety and OCD big time. Many years ago at four wisdom teeth pulled at once and didn't have problems with dry socket. Now just last night at about 5:00 so it's been barely over 24 hours, I had three molars pulled. This time around my anxiety and my OCD is just off the chart! Probably because I just tragically lost my baby brother.. but all I can obsess on is looking and looking and looking at the sites to see if they're doing okay and am I getting dry socket? I've been watching so many videos and doing so much googling and everything I look at looks different. All the pictures and videos I find online one looks different from the last. When I look at my extraction sites I see the stitches that they placed. I see a bunch of you know like normal looking blood red gooey. All is very normal. Nothing is empty. It's all full with something whether it's blood goo or I'm seeing white as well and it's only been a little over 24 hours but as more time goes on the more white I'm seeing. It looks like tissue of some sort. But from what I've googled it's too early for that I don't know.. any insight doc? I'm just freaking out that I have or I'm going to get dry socket!

Sounds like all is well to me from your description. Whitish material over the extraction site is normal and will become more pronounced with time until it eventually heals. The big thing is to just leave those areas alone and make sure your symptoms are gradually improving. If there are changes where you have swelling, more pain, etc then contact your provider. Thanks for all the information its helped a lot! I'm on my 10th day of the healing process i had the bottum left molar removed but I did have an infection prior and still had the infection a few days later, I thought that was the main cause of pain in my gum but turns out I've actually found a bone fragments stuck under the gum where the gum folds over if that makes sense, I've only now felt it and it feels pretty big! I was wondering whether or not I should go to the dentist or whether it will come out itself?

Depends Charlie - if it's loose, and a bit larger like you say it's best to have it removed. If attached and very bothersome, same thing. When they're smaller they will eventually fall out over time with minimal discomfort. Always best to follow-up with your provider as they'll be able to know best once they see you. All the best!

I know this post was a year ago, but I figured I would respond for anyone else reading it now who is going through the same thing. I actually had an issue very similar to what you explained. I had a top left molar pulled and I was in severe pain almost two weeks after having the extraction. I could feel something sharp sticking out and I thought it was a piece of tooth that was left by mistake. I also could feel a lot of pressure at the top of my gums and when I felt it with my tongue, it felt like some of the dissolving stitches were possibly stuck to my cheek and causing that pressure because it was pulling on the gums. I saw online it was sometimes common for tiny pieces of a tooth to remain but fall out on their own and I kept pushing it off and hoping that was the case. When it wasn’t getting any better, I finally decided to go back to my dentist and it’s a good thing I did. He found a really tiny piece of bone that had somehow gotten stuck and was causing the pain because it was keeping it from healing. It was so tiny, my dentist even had a hard time seeing it at first, but it still caused a lot of pain. As soon as he flushed out the site, found it, and removed the loose piece of bone, the pressure immediately went away. He told me to give it another 4-5 days for the pain to go away and to continue my antibiotics. He also suggested doing the salt water rinse for two more weeks. It’s been about two weeks since then and it’s slowly healing, but I haven’t been in pain since about four days after he got the piece of bone out. TLDR, So, lesson learned from me: Don’t push it off and think it will get better. It’s always safer to get it checked out, just in case. If I didn’t, I would have continued to be in a lot of pain and it would never have healed correctly.

Hey doc, I got my lower right molar removed and noticed a white patch on the extraction site. There is no bleeding and a clot has supposedly formed. I want to believe it’s granulation tissue but it’s only been two days since the extraction and I still feel pain, but it’s not extreme. Should I contact my dentist to examine it further?

Hey, I hope your still answering comments on this video. I just got a tooth extraction a few hours ago and after finding out about dry socket I became paranoid because I think I accidentally spit a few times (2/3 times) but not violently just soft enough to let some blood out but then I read I shouldn't spit at all for 24 hours. Did I just increase my chances of dry socket? And saliva shouldn't be in the hole right?

Alexcia - I wouldn't be too concerned. Spitting a couple times in theory could dislodge the blood clot, but in reality I imagine you have done no harm. Also it is next to impossible to keep saliva out of an extraction socket as your mouth is constantly bathed in it. The saliva will not interfere with the healing process. All the best to you!

Hi, I had a lower back molar out almost 3 weeks ago and I'm worried about food particles becoming trapped in the socket, there's a stitch that still hasn't dissolved and the hole is still very noticeable and deep enough that stuff looks like it kind of gathers in there, I rinse after meals but it never removes it all, am I at risk of infection at this point? I can't stop worrying! Thank you ✨

Sarah, this is fairly common and the socket heals from the base upward. It's not super common to get an infection or have trouble healing once you're this far in. You're doing the right thing by keeping it clean. The dentist could perhaps provide you with a syringe that can be used to irrigate the site more easily. All the best!

I have had impacted teetg removed 2 weeks ago and the left one seems to have a little bit of blood coming out. Not dramatic but i wake up to blood in back of my throat like old blood but i dont know if this is normal. Because during the day i have some yellowish blood in my invisalign retainers.

Hello doc! Today was day 6 since molar 18 was extracted. I have been eating soft foods and plenty of liquids. This morning during my gentle rinse with salt, I noticed the white part forming over the socket was gone when I checked after. It doesn’t hurt as of now, it’s been 12 hours. It does feel, different. Slightly tender. But nothing worse. It doesn’t look like a big hole, just not as raised as the surrounding gums. I’m afraid of developing dry socket. I called my dentist and he said no pain is good and to just be careful. Just wanted to get a second opinion. Is it normal for the white surface to fall away?

The white eventually will turn to pink / red as the new tissue forms. I would agree with your dentist based on what you have described that as long as your symptoms are minimal you're on the path to healing well. All the best!

Mine is kinda Greyish, with white, and a bit of red. My moms says it looks normal and fine, but I’m not sure. There is no pain in the socket at all, and it is on day 3 of healing just about day 4.

hey Doc, great video. I am wanting to ask a question. i went for my follow up today (extraction wsa bottom right wisdom tooth) exactly 1 week/ 7 days ago. i had been slowly rinsing with salt water. and being very careful. anyway. today he said the healing looks really good and i can now eat anything i want, excersie as much as i want etc.. and he used a syringe to clean it out and then gave me one and told me i can use it to clean the hole that is stil there until it is fully closed up so that nothing gets stuck under it. however . i can't even see anything unless i am at home with my small magnifying mirror and put that light on even so. i am confused as to exactly what the hole looks like. up til now i have always seen a dark black spot, although it seems t be shirkning. altough just to the right of m ytooth where it was. i see almost like a slighy indentation. but i am confused as to what this hole is and how to use it. although i will keep rinsing very good with salt water and burshing a lot . but hopefully i am doing what i ened

The granulation tissue just built and built until it was the size of the surrounding molars, then it fell out. That happened only 3 hours after extraction. I thought it was a clot of blood but I squeezed it in the sink and it was white and tissue like. More replaced it and it seems to be staying a manageable size in the socket and has not budged.

Thank you for this information video! Is it possible to develop a dry socket without experiencing pain? I’m on day 4 since the extraction and I can see that one of my extraction sites, which was the only one that hasn’t been stitched, is just a deep empty grey/white colored hole.

In my case, these long stringy blood things keep coming out of my upper left extraction site (got all 4 removed Thursday) and idk if I’m gonna get dry socket it doesn’t hurt but it’s also not bleeding like crazy idk I’m really confused 😭 and my surgeon isn’t open until mid week

Hi doc. It know this video was made 3 years ago. I'm quite paranoid when it comes to this. I had my left lower wisdom tooth removed 2 days ago. It was not completely taken out. According to the dentist, it will either absorb naturally or come out thru the gums. I checked my extraction site, I noticed some whitish thing. My pain is improving very well. I'm just concerned what's the whitish thing. Is it too early for the granulation tissue to form? Is it normal, doc?

i’m on my 6th day after getting all 4 wisdom teeth removed and one of my blood clots came out during my sleep and woke up with blood on the bed and in my mouth. Should I be worried about dry socket ?

@ssyy_xmu - I wouldn't be concerned. You're already past the point when dry socket typically occurs which is about 3-4 days postoperatively. Often times you can get some bleeding as you sleep and this ends up on your pillow. Indeed it could be a clot, but it likely wasn't the clot from within the socket. Hopefully that makes sense. Hi doc, i had my lower right wisdom tooth removed 5 days ago and was healing nicely since. My swelling is almost gone and i no longer feel sore when I open my mouth (mostly). However, i’ve been tasting some bitter liquid around my extraction site. I have been upkeeping my oral hygiene and followed my dentist’s instructions and finished my medication (i was prescribe antibiotics, as i had pericoronitis). To my surprise there is an accumulation of white-yellow substance between the first and second molar, which i have careful rinse away. What could be the cause? As the foul taste and smell had only started 1 day before and seems to be getting worse. I don’t feel pain, but occasionally a mild discomfort on my right jaw (nothing that is alarming or even moderately painful). Also, my extraction site is stitched up, and i brush my teeth(being gentle or try to avoid the surgical site) and use prescription mouthwash and started salt water rinsing today.

Sherry - thanks for the comment. I can only speculate as to the nature of what's going on, but it could be a couple things. Sometimes the sutures used are ones that dissolve and they are white/yellow looking. They tend to collect debris and would be located in the area you described at times. The other thing I often see if food getting impacted in the sockets. It stagnates and collects odorous bacteria that would create a foul smell and taste. Sometimes rinsing the sockets helps with this. Beyond that it's difficult to say. In the absence of pain or swelling I would suggest that you are likely healing okay. Make sure to follow-up with someone if your condition changes. All the best!

Certainly you could have some clot material come out of the socket. This doesn't necessarily mean that there is no longer a clot present or that you will have troubles with healing. Just keep an eye on it! All the best my friend!

@@darleneayala8887 to be completely honest I was scared of getting a dry socket after seeing a blood clot but the recovery actually went pretty well I just stopped doing what I would normally do for about a week so I can heal faster but if you don’t feel any pain I believe you should be good. Eat ice cream or yogurt it’ll help with swelling too

I got my upper front tooth next to my k9 removed and a bone graft done Monday at 5, I am severely addicted to nicotine and am going through withdrawals but am too scared to risk it because I’ve heard about the horrors of dry socket.... with that being said I have seen people say that maxillary teeth are the least likely to experience dry socket and my recovery seems to be going better then people have described online (no bleeding and no more distinct hole considering my surgeon stitched me up pretty good on the gums of the extraction site). I was just wondering in your personal opinion if I was okay to take a few small hits every few hours with gauze in and then wash my mouth out salt water, I just feel like the chances of me getting dry socket at this stage would be less than likely but I’m no dentist... If you could just give me some advice on this with more of a realistic answer I feel like everyone just says don’t smoke anything for a week but their is no way people listen to that haha I smoke a vape if that makes any difference.. thanks!

Nathan, smoking changes the physiology of your mouth (and body) so that it's not as vascular and is less able to repair itself effectively. Just because you smoke doesn't mean you'll get dry socket. Hi, I got tooth molar #26 taken out 7 days ago. Seems to be helping pretty good. Ive literally only had icecream, yogurt & liquid soup this whole week & starving! Im mostly paranoid of eating something more (chewing wise) and distupting the healing process. Is 7 days safe to start chewing softer foods like eggs, mashed patatos, pasta? Im so hungry but also paranoid of hurting the site & starting from scratch. I'll try to chew on the other side..but what if the food accidently touches that gum??

You'll be good to go Katja. Unless your surgeon advised you otherwise, you are typically okay to eat whatever you are comfortable eating at this point. If you were provided a syringe to irrigate the sockets, then use that in the evenings to keep the sites clear of food. All the best!

Hi, I got my wisdom teeth out five days ago, I’m not having much pain at this point, but there is white looking growth over the extraction sites (which I have heard is normal), and I can’t tell if it is food stuck in the socket or just normal healing at times. Should I just leave the socket alone? I don’t want anything to stay in there and cause an infection, but I also don’t want to give myself dry socket, which I don’t know if I’m at risk for at this point. Thanks!

Hi Doc, after my lower wisdom teeth removal, two small holes remain after 4 weeks and one looks pretty deep. The situation has not changed since week one. Is that mean that socket is not going to close? I have no pain except some irritation in the extraction site. Should I concern that it is not healing properly?

Great video thank you! Quick question, I’m now on day 5 and only my left side hurts as well as just a little ear pain. My temp is 99.5. I have been taking ibuprofen 800mg but am curious as to why only one side would hurt. Both sockets are clean with no food

I assume that you had both sides worked on? It's not unusual for one site to heal normally and the other to cause problems. If a site is free of food and debris that is great, however you can still encounter pain from other sources. I'd recommend following up with your dentist. All the best!

So I recently got 4 premolars extracted and the day after the extractions I started noticing that my gums are turning a gray/yellowish color. Now it's been three days since the extractions and I don't feel any pain or any irregularities. Is this a normal healing process?

Sounds like it is normal Patricia. Depending where the colour change is, it could also be tissue sloughing off from the anesthetic which happens the odd time. Usually on the roof of the mouth moreso. all the best!

Hey doc I am 13 days post op from getting all 4 wisdom teeth out. The bottom left site is causing a burning sensation that radiates to under my throat, chin and neck. It just started 4 days ago after I started to use mouthwash again (closys sensitive brand). It’s not unbearable but it is a very irritating and uncomfortable feeling and I do think it is gradually getting worse. I do not have a fever, no swelling, no chills, no trouble swallowing. I’m going back to the dentist on friday but was hoping I could get an answer or idea now. Thanks !

Hard to say Nia. Your dentist should be able to offer more insight. Might not hurt to stop the mouthrinse. Sometimes the back portion of our last molar is more exposed for a period of time during healing which can lead to sensitivity, but again each case is different and this may not pertain to your situation. When i get teeth pulled, the dentist has to sew the hole and cartize the gum because i bleed as much as 1/2 a pint of blood. So not sure if i may have a minor hemophilia, never been tested yet.

Got my back 4 molars out 8 days ago and last night the blood clots appeared to just be gone. I don’t know what time of the day they left but I have none it seems. I have holes that are all pink w no discoloring. No pain at all. Wondering if this is normal at this stage. Thank you so much!

I got my tooth pulled 5 days ago. The swelling is all gone. My stitches fell out. But the socket seems to be empty and feels empty. I don't really have pain. But I do have throbbing in my temple. And almost like a feeling of pressure. Should I be worried. Or is this normal?

That doesn't sound too great, but it's tough to give you the proper diagnosis online. At 5 days post-op I'd be expecting that your pain level should be decreasing and you should not be experiencing that throbbing pain. I would advise that you get in touch with the dentist who performed the procedure and see if they want to have another look. All the best my friend!

Hi Jason, I have a question that I can't seem to find an answer for. I had my first molar (mandibular) extracted three days ago. A clot formed with white substance over the top of it, and today I've noticed that the level of the clot is lower in the socket. It still has some white substance over it, but I'm concerned that this might be the onset of dry socket. There is no foul odor or taste. In terms of pain, it has been slowly getting better since the extraction day. No sudden worsening of pain (yet?). Is it normal for the blood clot to shrink after just three days? There is still some kind of blood clot in the socket but it's not as much. The best analogy I can think of is a bath half full.

About 8 months ago I had a tooth extracted and the next day I got very I'll. The worst symptom was a strong deep pain in my upper right back. It lasted for 4 days. No dry socket at all. Now 8 months later there is still a crater and some pain if I place my tongue in it. My dentist and DNP said this could not happen due to anesthesia. I still dont know why this happened.

When does the blood clot form? Like how soon? My dentist didn’t stitch my top or cheek wound. He said it heals quickly. However, I took out a gauze (bleeding was bad) 3 hours after surgery and there was a clump of blood. Is that the clot or could it be just dried blood from a wound? Cause I didn’t do anything wrong to ruin the process.

Alondra - Hope all is going better today for you. What you have described sounds pretty normal. Sometimes the gauze isn't quite in the proper location and some blood leaks out and coagulates on the gauze. When you remove the gauze, it's not necessarily the clot from the socket that comes out with it. Sutureless third molar removal (especially upper third molars) is getting to be exceedingly common. These areas heal just as well without sutures. Your blood has stuff in it that acts like glue which immediately sticks the tissue down into place and that 'bond' becomes stronger the longer you're healing. the clot forms almost instantly (within minutes) and matures over the initial days of healing. This is why you're directed to avoid disturbing the extraction site or to avoid aggressive rinsing/spitting. All the best to you my friend!

At this point, dry socket would be quite unlikely. It usually sets in about 3-4 days following the extraction. Sounds normal to me, but if you are concerned you should contact your dentist.

@@Jpd_30 That's a tough call on here to say what may be happening. Dry socket it pretty intense and most people experience significant pain that does not respond much to over the counter medications. If you have concerns it's best to give your dentist a shout. Wishing you a speedy recovery!

Hi I just got my wisdom teeth out a few days ago and my dentist instructed me to use a syringe to clean out the sockets but I can’t see the holes! There are stitches and gum but I can’t seem to find the sockets

Jacqueline - this is pretty common as the sites can be sutured fairly tight. Generally you can find some access by sliding the tip down the back of your last lower molar. This video was very informative however I had a question of my own. I got number 17 and 32 impacted removed last week. I just went in for my follow up yesterday and was given a syringe to flush out the food but I forgot to ask this my to my dentist, when will the holes close if I don’t see a blood clot,? I am not too worried by my dentist said I was fine and I don’t have much pain anymore I am just paranoid about the open holes in my mouth

Glad things are going well! That's the best practice to flush the sockets as was directed by your dentist as this will help to keep the healing on track and hopefully minimize the chance for future issues. The socket will actually begin to heal from the bottom upwards or in the case of a situation where the clot has been lost, the socket granulates over meaning that tissue grows from the edges down into the socket to cover it over. Either way it sounds like things are going well for you so I wouldn't sweat it! It can take months in some cases for the bone to completely smooth out back there where the lower wisdom teeth are. I was wondering if it’s ok that my socket on day 3/4 is off white almost grey and yellow? There’s no food in the socket, and i have no pain except for the excpected soreness from the extraction. As well I have been rinsing gently with salt water about 5 times a day and brushing my teeth but not near the extraction site 2 times a day. Please reply if you can, thank you.

Thanks for reaching out to me! In most cases, the answer is yes. Your dentist did the right thing by removing the offending tooth (source of infection) and they supported that with antibiotics. You should heal totally normal assuming that you're relatively healthy with no underlying conditions / medications that could impair the healing. All the best to you!

@@lorettajent It could be normal depending on the situation. A difficult extraction that is perhaps prolonged with an incision and bone removal around the tooth could result in swelling. It would be normal for this to peak around 48-72 hours. If the tooth was infected at the time of removal, it could be possible that you need the support of antibiotics for healing. I would have to defer to your dentist on this as they will know the history better than I. All the best to you!

Yep, had my back, lower (last molar) extracted, bled for around a day, thankfully not too drastic, stopped later that eve. Now day three, tender, no bleeding - but, this video should of contained pictures, as to me? it is silly to put up a video like this,, as most folk (myself included) want to make sure the healing is going well.

That's hard to say Iretiola. I recommend following up with your dentist. Speech issues can be a sign of infection, although it's hard to know with the information provided. If everything heals normally, you're about 4-5 weeks for significant healing to be complete. It can take several months for the site to completely fill in with bone (2-3). Pain generally shouldn't be present for more than 7 days.

I just got mine removed too but I wanted to check on it so I put my phone flashlight in my mouth to look and I see what I’m guessing was pus and old food particles mixed with blood(gross Ik)that got stuck in my gums before the extraction. So I scraped it out wit toothpicks and qtips and bro that shit was stinkyyy I couldn’t believe it was in my mouth after I got all I could out I brushed and used mouthwash and my mouth taste a lot fresher I plan on doing this until I find no more particles in thee socket

Mark - you can brush any time you're comfortable doing so. Gentle salt water rinsing is okay too. The big concern would be in the first 24 hours or so in terms of not disturbing the clot. Hopefully all is well now, sorry for the late reply!

Hi, I had 3rd molar removed a week ago, and bone grafting placed and then gums stitched over top. For the first few days I had something white (collagen membrane? granulation tissue? bone graft itself??) and now as it is healing, there is no more white, just a hole/indentation. Does this mean something went wrong? Why does it look open if he stitched it up? Stitches are still in but loose now. Not sure what's going on

Hard to say Trisha. It's possible for bone grafting to come out from a socket. Usually you would know as it may be a bit gritty feeling in your mouth from time to time. Regardless, it's rarely necessary to graft 3rd molar sites and this isn't routinely done (not aware of your situation in particular of course), but that site will fill with bone over time on its own. Hopefully that helps?

Yellowish stuff on sutures is generally plaque and debris. Also the sutures themselves can be somewhat yellow if the surgeon used a gut suture. They tend to swell over time and can look a bit like a food particle where the knot is.

i got my wisdom teeth out about 6 days ago and i’m still experiencing some jaw pain but i have noticed this nasty taste in my mouth that have been occurring from the sockets. i don’t think it’s dry socket because i’m not experiencing excruciating pain but i have been practicing the best oral hygiene i can but the taste has gotten worse, would it be beneficial if i get a syringe and rinse out my sockets or is it too early?

You're at the perfect stage to start flushing those sockets and it is very possible that there is just some food that has accumulated which is starting contribute to the taste you're experiencing. Even though the gums often look to be fairly closed up at this point during healing, you have to remember there is a significant defect in the bone under the tissue where things can collect. Christian - most likely bruising from the removal of the teeth. The gums in many wisdom tooth cases are cut open to reveal / expose the tooth. When this happens it creates a space for blood to get under the tissues into potential spaces (kind of like deflated balloons in your face and neck if you think of it that way). Once the blood starts to break down, you will see some pigmentation through the skin. Often purple, green, or yellow depending on the stage of resorption of the blood. Hope that helps!

It looks like you're still active here and was wondering if you could answer a strange question. I had my wisdom teeth removed July 21, on around july 24 i noticed that the gums in between my second and first molars on the right side of my mouth were flappy and could hang out off the tooth. The next day my left side was doing the same thing. I had my post op today and forgot to mention it to my oral surgeon. He didn't say anything so i assumed it must be normal. Am i assuming correctly? I is something wrong?

Yes, that's a normal occurrence and it should rectify itself with time. You'll need to keep it clean. It's like a secret trap door that you don't always even see until you pull back the tissue flap! What happens is when the incision is made to expose the impacted teeth, the surgeon actually cuts the gum in that area to take some tension off the tissue as they pull it back. It's not always sutured back together as that's not necessary, but occasionally that flap becomes mobile and heals a bit different as you have described. Again, it'll go away with time. All the best!

Hey Doc! I have a few questions regarding my 4 impacted wisdom teeth removal surgery. To start, I am on day 6 of my healing process and I still have a lot of radiating pain in my jaw and what feels to be either my gums or the extraction site, is this normal? Question two is that I have been rinsing with salt water, is it ideal to use a syringe or can I just be doing what I have been doing this whole time with just a cup? Another question is is that I still have no formation of a seemingly to look like blood clot and I am one to be very paranoid about everything, is this normal? Last question is that I have been brushing normally avoiding the extraction sites but can't seem to brush my back molars because I am worried to accidentally touch the extraction site so instead I have been using a floss stick to dig around my gums to take the plague from my teeth out, how and when can I start brushing normally because I am a maniac when it comes to oral hygiene lol, Thanks! Oh and one last thing, I got a canker sore below my tongue, canker sores are normal I'm assuming? Thanks lol

You're fine to brush normally now and will not harm the area by doing so. A syringe is nice, but not essential, just do you best to keep it clean back there. I have m7 removed due to the wisdom tooth pushing and damaging it.. After the extracrion ive got some bone exposed on the inner gum parallel to the hole.. Its causing major discomfort. How long does it normally take for new gum to heal over this bone? I went back to dentist to get it grinded down because it was such a sharp edge. Thanks

New tissue should for in 10-14 days. You did the right thing. Often this bone is thin in that area and it can sometimes poke through the tissue following an extraction. As long as the irritation has been removed, your discomfort should fade away within a couple days. All the best!

Hi I just got two molars extracted and only left gauze in for 45 minutes instead of 3 hours like everyone else is saying. It’s still oozing a bit especially when I drink water. (8 hours later) Should I be concerned?

The usual bleeding time is 2-10 minutes when someone gets a cut. It's not uncommon for an extraction site to 'ooze' for 8 hours and it could even do that for 24 hours. As long as the gauze is just tinged pink and not dripping wet and bright red saturated with blood you should be good. Often just another ten minutes of good gauze pressure biting down on the area is enough to stop the bleeding. Thanks for the question!

We see this every day and yes extractions can still be done. There are rarely any complications and the treatment an opening to the sinus is generally fairly straightforward to do should this happen (which it rarely does). All the best!
