Fun fact: San Pedro actually came from the Los Angeles (highway 101) sign that is right before the Hollywood Blvd exit. There is a small town near Long Beach called San Pedro that this exit brings you to. Madonna said she remembered this freeway sign when she lived in Los Angeles and would always pass it on the way to the recording studio and liked the name. She thought it was very fitting for the song.
"Como puede ese olvidar" (how could he forget). That's the spoken line at the beginning. And then she says "te dijo te amo" and "él dijo que te ama", both meaning "he told you he loved you". And yes, you're absolutely right, the music sounds more latin than spanish, it sounds caribbean. And latin and caribbean music from South America and Central America have spanish influences of course, because of colonization, but also portuguese and a huge huge influence from native latin american cultures that lived there before colonization.

I think she is living in a Spanish culture in LA and she is fantasizing about where that culture originated and how life could be better somewhere else. She accepts her condition toward the end of the song and turns her longing into something beautiful .

I'm not familiar with that particular interview you quoted her from, but Madonna's my favorite music artist (for many reasons other than just her music), and one thing I've noticed is she has a funny sense of humor. The name of the island may have randomly come to her while writing the song, so she probably made a joke out of it being on an exit ramp sign. It sounds better than, "I don't know, it just sounded good." But I don't think your overall interpretation of the song was wrong. She's probably in love with a guy that doesn't love her back. She's longing to be somewhere other than where she's at, and that place is a beautiful island where she can be that girl who loves a boy that loves her back. 👑 🇺🇸💙🇬🇧
To me, this is simply a fantasy of escapism. The video reminds us to bring a little color and romance into our everyday lives and the value of sharing that with others.
