In the book it is stated that a lot of the animals had various illnesses and diseases so Rexy could have suffered from an eye condition. Or the Renna DNA used to clone her could have had an affect like was also theories in the book.

But in the movie even before Jurassic park Grant at the digsite claimed Rex had a Vision based on movement. In the novel It is something he discovers by accident when the T-Rex is mere inches away from him. The Rex in the book had no frog D.N.A. in fact the book, it's the computer that does the handy work with the genetics of the animals hence why the scientists don't know much either. Best guess is that it's a genetic defect from the gene splicing.

Rexy always had bad vision JP 2 and 3 use different rex individuals. Rexy just had bad vision + in JW she's elderly so of course her eyes aren't as good .
