I actually think that he used her to escape the crushing loneliness, a smack on the head will pull you out of that cycle easier that you may think. Depression sometimes leads to feeling like you don't exist at all, and pain can bring you back, it's something real, something tangible, an anchor. And I think that's what Sakura really was to him, but then he had to rationalise why he took it and why he wanted to be around her, so naturally his brain provided "love". But that's just a theory
