once while equipment changeover, for handover of preventive maintenance, mistakenly forgot to open a valve, that was hidden by a platform on the top, this caused equipment downtime. i got warning letter, is that fair? there were many factors that caused distraction, as i was busy endorsing permits and taking routine rounds and logging readings in the plant, when they unnecessarily asked me to do this job at that time which could have been done later as it was not urgent at all. it was just a bad day, too hot and humid and also too bright, desert life you know ... of course i was to be blamed :(
