What is Major Depressive Disorder with Mixed Features?

This video describes major depressive disorder with mixed features. Major depressive disorder MDD with mixed features is a disorder with a specifier. There are a number of specifiers available with MDD, and mixed features is just one of them. This is one of the more controversial specifiers we see with major depressive disorder. It's also a specifier available for bipolar disorder. This specifier is often missed and this set of symptoms is oftentimes misdiagnosed as well, because it's easy to confuse with bipolar disorder. Whenever we use a specifier with a disorder, the full criteria for the disorder have to be met first and then there are additional criteria for the specifier. In the case of mixed features there are a number of additional criteria. There are a number of different potential symptoms and an individual has to have three or more of those symptoms to qualify for the first criterion. Criterion A contains the symptom criteria. These different symptom criteria need to be present nearly every day during the majority of days of a major depressive episode. In Criterion A we see elevated or expansive mood, grandiosity or increased self-esteem, talkativeness or having pressured speech, a flight of ideas or racing thoughts, increased energy and goal directed activity, engaging in activities that have a high potential for painful consequences, and a decreased need for sleep. Criterion B notes that this behavior is observable and represents a change from the usual behavior. Criterion C indicates that if the full criteria for mania or hypomania is present then the diagnosis of bipolar disorder should be made. Again, this mixed feature specifier is often confused with bipolar disorder. Criterion D says the symptoms are not attributable to substances. There are several characteristics associated with mixed features that make it different than what we might see with other specifiers or MDD without any specifiers. We see that mixed features are a significant risk factor for developing Bipolar 1 or Bipolar II disorder. We know that oftentimes with Bipolar Disorder, an individual who has that disorder would have started with a major depressive disorder diagnosis. We also know that mixed features aren't rare. About 25 to 40% of individuals with major depressive disorder would qualify for the mixed features specifier. Oher characteristics associated with the mixed features specifier include: increased risk for suicidal behavior, increased use of substances, a poorer response to treatment, an increased number of major depressive episodes, and an earlier age of onset of major depressive disorder.

Thanks for this insightful video! I was originally diagnosed as bipolar 1 because my psychiatrist at the time believed mixed episode only occur in Bipolar 1 disorder. So I was given fairly strong medication and told it was necessary to reach stability. After a few years another doctor looked at my history and thought bipolar 2 was a much better fit for me since I experience mainly depressive episodes and hypomanic episodes with mixed episodes in between and I'm currently tapering off dosages on my mood stabiliser. Before this video i believed (and evidently some psychiatrists do) mixed features only occured in bipolar disorder. It's news to me that it is also afflicting major depressive disorder patients. So I guess this specifier is important to add to depressed patients. Mixed episodes are particularly dangerous indeed because you can feel so incredibly agitated and frustrated but at the same time have normal energy and motivation levels, it's the only period of my cycle where I experience suicidal ideation. When I'm really depressed I don't even have the motivaton to contemplate suicide. That's why mixed episodes are so dangerous.

I have MDD and anxiety. When I'm anxious I get a little restless and I get less sleep. It's like my mind won't shut down. I overthink everything. It seemed some of the list are also symptoms of anxiety. Taking to much can come from nervousness? I just don't get the surge of energy. And I don't get elated. My overthinking gets me thinking I have a little bit of every disorder in the book. A least I'm aware of my insecurities and irrational thinking. My self awareness is both a blessing and a curse.

extremely complex, I am getting confused between all the disorders and specifiers. However I am getting a better idea of what it may look like in real life.

Thanks a lot, this is really informative! I have an mdd diagnosis made 4 years ago and stopped treatment then and as of late it's gotten worse, but I do appear to have mixed episodes at least once a month. I'll try to get proper treatment soon though, as to not self diagnose. Thanks a lot :)

I reckon that cases of MDD with mixed features should really be given a provisional bipolar II diagnosis, or maybe even be made into a bipolar IV diagnosis or something, because the whole point of the specifier seems to be to say "looks like MDD but is probably bipolar disorder"

This was a very informative video on MDD and mixed feature specifiers. I did not know that mixed feature specifiers are risk factors for developing Bipolar 1 or Bipolar 2 Disorder. I also was unaware of the high percentage rate for individuals with mixed feature specifiers.

Thank you for this. My doctors aren’t able to figure out if it’s bipolar ii or mdd with mixed features (at the moment). My hypomania is mild but still there, which confuses them and me. It’s frustrating when you want to figure out which category you belong to. It will feel like closure sort of.

I originally was diagnosed with major depression, then bipolar 2 when the meds wouldn't work correctly. Ive recently fallen back into a depressive state, and now I'm being diagnosed with major depression with mixed features. Do you think you could elaborate about the difference between bipolar 2 and mdd w/mixed feat?

I have had my Bipolar 1 diagnosis since I was 17. My depressive states cause melancholy, or major depressive disorder. I've attempted suicide 3 times because the melancholy and shame from madness during mania is unbearable. Why I continue to try is beyond me. All I can say is that there is no pain that is explainable that you feel with bipolar disorder.

Wow, DSM doesn't allow bipolar to be assigned as a diagnosis until 18 and up. You had a rebel doctor.

What would anti-depressant induced hypomania fall into as a diagnosis? I have mixed state/hypomania issues only while on a anti-depressant. Even then, it's still not clear exactly whats going on. I end up feeling 'high/euphoric' and a lot more agitated but my depression is still bad. Literally they never help. I have no clue what the hell to even call that, as it doesnt really seem to fall in the category of mania/hypomania. It feels like im on the threshold of bipolar, but not quite there.

Usually, and there are unique cases, hypo mania caused by antidepressants indicates the depression isn't the garden variety. Some psychiatrists call your condition unipolar bipolar...that is, you have the bipolar illness but are displaying only the one pole, depression. I don't recall all the other details of the condition. But mood stabilizers are usually helpful.

I understand the controversy. I agree that the wording describing severity and duration of symptoms needs clarification. Technically, 1 week most of the day every day is different than the majority of the day nearly every day, but not much. This would have to be observable to distinguish the difference. I have seen the manic episodes, but not one day on, one day off, and on again to differentiate the specifier from bipolar disorder.

I am really confused. How does this differ from hypomania and cyclothymia...or do these diagnoses no longer exist? The overlap makes it quite blurry...

If patient has a previous episode of MDD with Mixed Features should the diagnosis then STICK. Just like One episode in the lifetime of bipolar 1 or 2 patient means patient is bipolar for life ?

You forgot the psychomotor agitation. Anger is a also a sign. To simplify it, it's basically *Agitated depression* in opposition to *melancholic depression.*
