Hi I've been recovering from a moderate ear infection I had fluid build up in both ears and tinnitus and it has slowly improved over the past 6 weeks. Usually when I sleep the fluid will go to the ear I have facing the pillow. Today I woke up and my one ear felt clogged and slightly muffled. I was crying last night and that probably caused some more fluid to build up but I'm worried. I can still hear fairly normally just slightly muffled. Is this conductive hearing loss??? I'm very scared and have problems with OCD and don't know if I'm making a bigger deal out of this then it is. Thanks

Sounds like you probably have a conductive hearing loss from the fluid in your ear(s) - but I cannot say for certain. I'd recommend getting a hearing test and making sure an Ear Nose and Throat physician is addressing the fluid. It makes sense for that to be stressful!

I'd recommend searching for ear nose and throat specialists in your area or a city near you. You cannot know for certain how good they are but it is good to ask how many of those surgeries they have done, what they would consider a successful surgery and how often the surgery is successful. This may help you understand if they are a surgeon that works for you. Another way is to ask a trusted general physician if they have an Ear Nose Throat doctor they recommend and WHY they recommend them. A hearing aid or other kind of amplification would be the non-surgical option. You can also do nothing but there are potentially risks associated with doing nothing. I hope that helps!

Six weeks ago, I had a wheelbarrow tire blow up when I was inflating it. My face was about a foot above it. I had some hearing lose prior to that, but it is now noticeably worse, plus I now have a cricket sound in my left ear when I swallow air with my mouth closed..... Question... what kind of damage would you expect from that kind of air pressure hitting my ears suddenly?

I don't know exactly how loud that would be but I'd say it's likely it could cause noise related hearing loss. I'm sorry that happened to you! I would go get a baseline hearing test.

hello dr degroot.., i had an abr test the result was mild to moderate hearing loss...?? the configuration was flat said by the audiologist.., i dont know the type.. she said mostly flat configuration is of the conductive type... is it true.., thanks

I was born with Auditory Processing Disorder. Which as you know has nothing to do with the ears, it's purely neurological. I don't wear hearing aids, but some people with APD actually do. I'm watching from London UK.

Very good, thank you just got diagnosed yesterday, sunday, august 29th 2021 with Sudden Sensorineiral Hearing Loss on thr right. I happened one day before and looked for help, in a Public Emergrncy Hospital here in Milano, 30 hours after it happened and was really well taken care of. Got Cortisone starting yesterday and today I took a barometric teraphy with oxigen. Still no emprovements but most of the time positive. Health and the time we have here are the most valuable things....

My Left ear is less sensitive than right ear. I went to the doctor and he told me that I have conductive hearing loss. When I asked him for reason, he says no reason for this. I told him that last month I was in a party and bear gone in my ear and after that I am feeling left ear heavy. But he told me that your ear is clean. There is no reason for conductive hearing loss and only two possible remedy is using a hearing aid or surgery. Please suggest. I am from India.

I'm sorry Anoop, I don't know what is causing your hearing loss but if you ask what kind of surgery is needed that would probably explain what the issue is. If the doctor says you need a "tube" placed in your eardrum then maybe you simply have a lot of fluid behind your eardrum (not anything that was spilled in your ear like water from the shower). So, I would ask him/her what kind of surgery. I'm sorry I can't help more.

There should be different latency intensity functions for conductive vs sensorineural losses. However, as I understand it (I do not perform ABR's anymore), bone conduction testing should really be done to verify and you should see an ENT. Best of luck.

Typically in cases like yours BAHA is an option but CROS hearing aids (non-surgical) are also.

hello dr de groot for conductive hearing loss.., if there is a blockage of sound..??? how then it can reach the cochlea..?? then we cant hear anything at all if its blocked and cant get there..., thank for replying

Just like someone with normal can hear something when they plug their ears with their fingers or earplugs the same is true if the ear is blocked by ear wax or middle ear fluid. Some sound vibrates through to the cochlea but some is not.

I recently suffered middle ear fluid from severe barotrauma from a recent flight. Had fluid in middle ear. Had myringotomy with tube in left ear only. However 3 days in hearing is muffled. And I have very loud sound in my right ear. Its like it shoots through my left and tunnels to my right ear very loud. I was told I may have Round window interference. What is that? And will My hearing return? Ear drum had previous scar tissue from tubes as a child. Please Help. Very Worried.

I'm so sorry to hear about what has happened to your ears! Unfortunately I've never heard of round window interference, nor do I have another explanation for what you are experiencing. If you are able to I would seek a second opinion from another reputable ear nose and throat physician if you do not feel confident with the one you have. I'm sorry I can't help any more.

My son was referred to Sac ent who told us conductive hearing loss is "behavioral" and "comes and goes" which doesn't seem right because she (Katherine Shnepp) said no treatment was available for that. 😶

Conductive hearing loss can often be treated medically. I would see a different ENT doctor. Conductive hearing loss can come and go if it is caused by something like middle ear fluid (which can block the middle ear but then clear up) - but either way it sounds like you need someone to sit down and go over your concerns more thoroughly. I think what you're saying is correct but just to clarify; a conductive hearing loss is an interruption in the pathway from the outer ear up to the inner ear (but not including the inner ear), a sensorineural hearing loss is a deficit in the pathway from the inner ear to the brain (so including the inner ear and auditory nerve).

Cochlear implants aim to replace as much of the function of the hair cells of the cochlea as possible but it requires a functioning auditory nerve to do so.

Dr. Alex, does the ear canal diameter affects hearing? I had hearing problems since I was a kid. My doc told us that I had clean ears but he mentions about ear canal diameter. Is it true? How does ear canal diameter relate to hearing?

As far as I know, the diameter should not effect your hearing out and about in the real world. However, when your hearing is tested using an earphones they must fit properly - size and shape of the canal can make it so the person testing you need to use a particular size or style of earphone to get accurate results. Also, ear canal shape and size can effect how hearing aids fit and how the sound resonates in the canal, for this reason the gold standard of hearing aid fitting is to use a test called "Real Ear" that takes in to account the acoustic effects of your unique ear canal. Lastly, a very narrow ear canal may make it more likely for your ear yo get completely plugged with wax, which can indeed impact your hearing while there is wax present. I hope that answers your question.

They would need a hearing test with bone conduction thresholds. You may also be able to get a general idea from performing tuning fork tests (Weber, Rinne) which you can look up online.

What I'm trying to find out is I have loss hearing do to whatever caused it I get steroid Injection in the ear if what ever hearing comes back how long does it take before the glogged ear feeling goes away

Recovery from sudden sensorineural hearing loss usually occurs within 3 months after onset of hearing loss. I usually see recovery of hearing after steroid injections within a week or two. But, if you have a conductive hearing loss I'd expect steroid to work pretty quickly but I don't know what research studies indicate. I hope that helps!

A sensorineural hearing loss measures the same through bone conduction as through headphones. So if they have a sensorineural hearing loss at 4000Hz, they do not hear any better through bone conduction at 4000Hz than they do through headphones in that ear.

I'm 34 and I had sudden hearing loss when I woke up one morning. I read that SSHL can affect any age group, but it's mostly late 40's to early 50's. Anyhow, I was so concerned, that I downloaded a phone app and the test came back as 13 db hl in left ear and 37 db hl in my right hear. Trust me, I can notice that difference away from the app. Heck, in the right hear, 2k frequency, I got like a 57 db hl and 41 db hl for 4k. These numbers are not great, but I think people are talking back even more hearing loss than what I see here. I just enrolled during open season for Blue Cross and I would love to wait until January, but I would happily pay thousands of dollars now, to protect my right hear. I used the Mimi Hearing test app and yes, I know that it's not as accurate, but it gave me an idea. Do you think I need to go see the doctor ASAP?

Well, it kind of depends on how long it's been since your hearing dropped like that. If it's been more than a month or so then there is less urgency for treatment being effective, unfortunately. I would go to an ENT if it's been under a month and if it's only been a day or so get in ASAP. Depending on your medical history and what they see (or don't see) when they look in your ear, they may or ma not be able to figure out a cause and an appropriate treatment, but better to be safe than sorry especially if there is hope for treatment. I should say I'm sure there are physicians that would disagree with me and say come in with urgency no matter how long it's been. I would ask your primary care doctor to be sure since I don't know your history.

Sounds to me like they are suspecting your daughter has middle ear fluid or another type of blockage in the middle ear. It could be something relatively benign like uninfected middle ear fluid but you can't know unless you follow up as they recommend, make sure to see the doctors when they advise. I hope that helps.

Yes, definitely, though high pitched or a "flat" configuration is more common for a sudden sensorineural loss.

Can you please tell me what is the role of MRI & BLOOD test in sensorineural.. I had a sudden hearing loss in my left ear during my dinner time. I took steroids since one and half month. still it's not recovering.. just 20% recovery is there.. it's been nearly two months.. and tinitus is continuously there in my left ear.. can u please suggest me what to do..I had been many doctors.. Thank you

An MRI is looking for any kind of growth that might be causing the hearing loss - for example a benign cyst on the hearing nerve. Blood work is likely checking for autoimmune markers to see if an autoimmune condition may have caused the sudden hearing loss - but of course it is best to ask your doctor what specific tests they ran and why. I would talk to your ENT about if there is anything else that can be done. For example, for some people, injected steroids in to the ear is an option - but the farther away from the date you had the hearing loss, the less likely these types of interventions will be recommended. If your hearing does not fully recover on that side, there are two potential hearing aid options you could use that send the sound from that side of your head to the better hearing ear. I would talk to an audiologist about if these options are right for you. Best of luck and I'm sorry to hear about your sudden hearing loss!

@@alexandradegroot my first audiometry graph was almost at zero.. But now it's lying in about moderate hearing loss..is still there is a need of MRI . I'm scared of that.. and sometimes I'm getting little strain at the backside of my head..I'm getting so much scard about it.. I have taken lot of steroids as treatment.. Can u please suggest me what to do now.. I'm scared about it..worried. Thank you

Having middle ear fluid can sometimes make people feel off balance. Conductive hearing losses don't cause imbalance though.

Well-explained Dr Alexandra. Can the fluid be sucked out from the middle air? Does that require piercing the eardrum? I hope you explain this point.

Yes, it can be but you are right it does require a hole in the ear drum to remove the fluid through. There are also other interventions that aim to open the eustachian tube connecting the middle ear space to the back of the throat. As this is part of normal physiology a new opening does not need to be created. But there are reasons you might chose to put a hole in the eardrum instead. It just depends.
