This comment is to everyone asking questions about treatment AND to whomever made and posted the video. First I want to say: my Interventional Pain Management doctor had no clue what Trigeminal Neuralgia or Anesthesia Dolorosa even was. He had no idea what the meds or protocol was. He was 100% clueless. He's had to learn by trial by fire. He wanted to start me at the beginning after I'd been dealing with it for YEARS. I've had to be the one to ask or suggest a treatment option, such as the Botox. He was absolutely against it for months until I finally demand it. That's when I learned that he had no idea how to use Botox for ANY treatment, even migraines!! I told him he better quickly start learning some other treatments for chronic pain than just throwing narcotics at everything or I was going to report him!!! I DON'T want to be taking narcotics for the rest of my life and most people don't. I've had TN for almost 6 years now. I've had every treatment out there. THERE IS NO "BEST" TREATMENT. Each person is different and, for whatever reason, things that are currently working will suddenly stop working. Nerve damage is tricky and fickle and can progress with no warning. I'VE HAD TO ENLIST THE HELP OF OTHER DOCTORS MYSELF!!! I found a dentist who would help. A Rolfer (which helped soooooo much) for myofascial release in my head, face, neck and shoulders. A gastroenterologist. And a WONDERFUL primary care physician who is CONSTANTLY researching and asking me questions. I'm hoping the gastro doc will give me a referral to a dietitian or nutritionist, or both. I found a chiropractor who communicates with my primary physician, dentist, and Rolfer and the three plan treatments for me. Those treatments have been the most successful of all and no meds, other than lidocaine injections, are prescribed or used. Almost three years ago, my TN progressed into Anesthesia Dolorosa. I THOUGHT trigeminal neuralgia was horrible, the absolute worst pain imaginable, then the Anesthesia Dolorosa hit. For almost two years I suffered. I lost over 80lbs and at one point weighed 79lbs, I'm 5'4". The pain NEVER STOPPED!! Nonstop, 24/7 pain. Eventually I had an experimental brain surgery called a Nucleus Caudalis DREZ. Even though the hospital experience was brutal and traumatizing, the surgery helped relieve the pain on about 75% of my facial area. The only area still hurting is the left side of my lips down under my chin, the left half of my tongue & teeth and a small circle on my forehead. I still can't brush my teeth or eat but a few bites every few days. Currently I'm on four medications for the pain and I start Botox injections in 3 days. Here's a list of everything I've tried thus far: -Carbamazipine* -Oxcarbazepine* -Dilantin -Topomax -Gabapentin (helped tremendously) -Olanzipine -Hydrocodone (helped) -Fentanyl (helped the most-least side effects) -Methadone* (minimal help) -Cyberknife surgery (helped for about a year) -And my neurologist puts mass amounts of lidocaine up my nose about every 2 weeks. Everything with an * had such incredible side effects they rendered me non-functional. What works for me doesn't mean it'll work for you. My TN & AD is from Multiple Sclerosis and unique to me just as yours is unique to you. Finding the CAUSE is the first step. Usually TN is caused by the nerve and an artery rubbing over such an extended, long period of time that the throbbing of the artery eventually rubbed the myelin sheath away exposing nerve fibers (like what happens in the whole brain of people with Multiple Sclerosis or in the body in people with CIDP) except the person has neither. Most of the medicinal options are secondary use. There's just not many options. You and your neurosurgeon need to sit down and discuss all the treatment options and just start one. You'll know within a day if it's gonna work for you but you have to begin taking it to know. If you can find a trial, do it!! Soooooo much more research and development is needed for the "suicide disease"!!! You'd think there'd be an abundance of research happening for a disease that causes the literal "worst pain imaginable that's known to mankind", but there's just nothing. The entire medical community needs to do better because we just can't live like this for very long!!!
