Usually the one at the head of the bed is a Respiratory therapist. I know this because this is what I do when I work on the COVID unit. We have to make sure airway is secure at all times and that the patient doesn’t extubate during this procedure. Even though it might seem a simple inexpensive thing to do, the body doesn’t like being on that position for that many hours. 16 hours prone, then 8 supine then 16 again, until patient maintains good blood oxygenation. The body takes a beating, and the patient requires a lot of sedation for the body to comply while this is going on. This used to be a very complicated thing to do, and the use of special equipment (rotary bed) was needed and a lot of medical staff for a patient that was intimated. We had to adapt to this unsophisticated and simple way of doing it, but it work. All I have to say, you don’t want to be one of those people.

If people in government really thought that this was fake, they would have launched an investigation and proved this was fake right away. Can you imagine trying to cover up 3000 people death in 1 day across all 50 states? How many people would have to be involved in the cover up? These people don't even deserve a reply. All I have to say is, take care of your health. Most people with cover might just need rest, fluids and some oxygen while the body recovers. Some, their body never really recovers. Recognizing it took just 1 person that may not have worn a mask because of their illperceived rights and how many other patients might be clinging to life because masks are uncomfortable. I am a nurse practitioner with speciality in critical care, or the ICU. I can attest that we lose patients left and right to COVID. We lost 4 in the last 24 hours, in one small ICU of 18 beds, just yesterday, in a Midwestern hospital. The youngest patient was 37 years old. We prone our patients for 16 hours and then supinate them for 8, if they tolerate it. If they don’t, i have my nurses prone the patient again to try to oxygenate them as quickly as possible.

As someone who does this multiple times a day and night (often single handed) for my daughter who cannot move, I can tell you it is back breaking stuff. I have spinal crush fractures. Even with a whole team it is exhausting and labour intensive. You are literally breaking the backs of your nurses.

No, that isn't true. You sedate the patient first and try to ventilate them. They can move, but are heavily sedated. If that is not sufficient, then you add a paralytic. The condition you described is when you use a paralytic without a sedative. Something that is rarely, if ever done. The only times it's that way is a patient a develops a tolerance to the sedatives. In the old days, it can happen if you don't realize that the patient is undersedated but you can't tell because the patient is paralyzed, but now, it generally doesn't happen because of the use of a monitoring system called a BIS monitor. I guess you can imagine a set of events where it may still happen, but it would be unusual.
As a nation we need to understand medical professionals and education professionals better. We are a nation of care and a nation of education. We need a education system that brings us well paid teachers and nurses, for example, unencumbered by school debt, to quality schools and medical facilities in every part of the country. We need to end the stranglehold that greed for profits has on us. We need infrastructure, engagement, and leadership that brings us through pandemics competently.

@@rinoponce1537 just because the facility can hold thousands of patients, there might not be enough medical staff that can look after a patient. A covid patient in an Icu needs a one to one care. There had been nurse and doctor shortages even before covid hit so if you need five people to just change a patient's position can you imagine the strain that would do to the medical staff if they have do to that to about 100 patients every 12 hours?

I haven’t worked in the medical field for a while now, but I was never asked to put someone who had the Flu(CV19 aka SARS CV2 has similar symptoms to that of the flu)or even Pneumonia on their stomach. This is all new to me! Like the medical professional said last night on Rachel Maddow, she had only did proning 1 time through out her whole career prior to CV. Now she is doing it repeatedly to assist her patients (paraphrased). 🤔

Thank you for showing and explaining this "simple" procedure that usually takes a team of at least 5 people. It takes even more if the patient is morbidly obese. Also, person at the head of the bed is the Respiratory Therapist ( we almost never get recognized) RTs make sure patient isnt extubated/ tube isnt dislodged during the turns. Whats not shown here because of obvious reasons is how bloated patients faces get from being proned for so long or how most of them end up with pressure sores/ open wounds on their faces ( all due to proning on/off for so many hours) despite the proactive measures we have in place to try to avoid that. Sad thing about this is that no matter the video evidence, people still think its a hoax or just dont care.
ICU RN here.. think people should see what these patients faces look like after being prone for 2 hrs. Maybe one more person will wear a mask after seeing that? I’ve never had to prone anyone without a Stryker bed until Covid.. it’s just nuts. we never needed to have more than 2 on the unit and could go months without using one.. now? It’s all changed.
The patient shown in this film was obviously already on a ventilator, which complicates the task of turning them over enormously, as Ms Maddow indicated when mentioning all the tubes and hoses etc. connected (and actually running into the patient's lung for the ventilator and into the urethra for the catheter). Turning a patient on a ventilator (who would normally be heavily sedated in an induced coma) is not a risk-free procedure and this puts another factor into calculating the risk/benefit balance. There is also a limit to the length of time it is advisable to leave a patient in the prone position and they will need close and constant monitoring. Sedated patients on ventilators who need to be turned regularly are high dependency cases, normally in ICUs and requiring competent staff in the immediate vicinity at all times. Here in the UK our hospitals and ICU capacity is being stretched to its limits by the need for suitably trained staff as much as anything else (ICU nurses are specialists and the numbers cannot be dramatically expanded in just a few weeks or even months) and I'm sure this is also the case over in the States where you don't really have a public health-care system to speak of anyway.
This is the stuff that people need to see - the true face of covid isn't a hoax - it isn't just like the flu or something that doesn't really do much. It is something that not only makes it extremely difficult to breathe but something that takes TEAMS of people to try and help fight and even then you may not live. My grandmother died from covid and every time someone complains about wearing a mask being uncomfortable I want them to see what it's really like. Wearing a mask isn't uncomfortable - being intubated and straining to breathe is.
I've seen this done in person when my aunt was in the hospital with Influenza A and it's tough work. I was in awe of the precision and speed with which the medical personal do it. She was just 1 person in the ICU that needed it when it wasn't super busy overall, I can only imagine how tough it is to just go from room-to-room doing that on a daily basis. These workers are beyond heroes, they are putting their physical and mental health on the line just to help people and it's disturbing and disgusting how average people act so put out by simply wearing a mask to prevent spreading COVID-19.

Channel 4 TV in the U.K. went one step further last night. The cameras filmed four individuals through their six months fight for life on a Covid ward. All were healthy, three of the four made it out alive but had suffered brain damage, strokes, were unable to stand up on their own. The fourth gentleman didn’t make it & to say the final moments as he slipped away were heart breaking & even at 68 years of age I couldn’t hold back the tears. No one should have to die that way. The sad truth is that many who don’t believe how serious this Virus 🦠 is, will succumb to it. Just wear a mask, socially distance and wash your hands and everything they/you come in contact with, surely that’s not too much to ask ...................... is it ???

Okay, what is needed to reposition someone depends greatly on size etc. When I worked as a nurse aid we generally used just two aides (CNAs) This was in a long term care facility. We generally tried to reposition people who were not able to reposition themselves once every 2 hours generally to reduce the possibility of bedsores. There are items that can be used to help with repositioning in one facility we had a set of nylon sheets (about the width of a gurney) which were used in tandem so that the top one was used to reposition the resident sliding against the one underneath reducing the friction that occurs when merely using a draw sheet. If I were at a state level and could get the funding to do so I would suggest that the state offer CNA training which would increase the need for able bodies to turn people negating the need for actual nurses to be quite as involved. It is possible that part of the staff involved in the video were actually CNAs. CNAs are of the lowest level of nursing staff and it doesn't take as long to train them as it does to train a nurse so people who are fit but jobless could be given training in return for working for at least a certain amount of time in a facility or hospital. They could be trained to some degree online until they need to get the hands-on training needed to get them trained to physically do the work. When I was trained in Kansas I was trained in the facility I worked in as a ward aide then once we got much of the classwork done we were placed on the floor as a trainee paired with another aide. We did this until we could take the test and the practical exam that would allow us to do the job without the need to be paired with another aide unless you needed two for positioning etc.
This is team work. Each relies on each other. And takes a lot of upper body strength. Not a simple task. I worry for each of them. And their backs. 💪🏼♥️💪🏼 to them Most days there would be 5-6 some less, to reposition. Bedsores and wash was why.. But doing this all day to 10-15 patients 💔 it would take a great toll on the staffs body.
You should even be doing this if you are self caring for your Covid-19 at home. I also helped my pleurisy pain. Lying on my side also helped if the pain is worse in one lung.
I've slept on my stomach for decades due to the same reasoning. And it's not just about the lungs, it's also so that nasal secretions will move toward the "exit" (the nose) instead of into the sinuses where there's no exit. It's much easier to keep one's lungs and nasal passages (mostly) clear if the mucus secretions can be blown out by coughing or nose blowing than if the secretions are in the depths of the lungs and in the sinus cavities. I have to wonder how many people have needlessly died due to pneumonia or covid or other respiratory diseases as a result of doctors keeping them on their backs.
Imagine the work involved if this were an obese person. I'm overweight... I would feel really sorry for the team who had to move me. 😔 Whatever we pay these people, it isn't enough.
It looks to me like hospital beds could be designed better: Make them double-wide with a hinge down the middle so that, normally, half will provide a flat surface and the other half will hang down out of the way. Then, to switch the patient from prone to supine or from supine to prone, rotate the hanging half 90 degrees so the bed is full width, roll the patient from the old half to the new half, and rotate the old half 90 degrees so it hangs down out of the way. The advantage is that the patient could be gently rolled to the new position, instead of lifted up and turned by a large squad of nurses.
It's when people are overwhelmed with their workload that little mistakes happen, and each little mistake will lead to yet another complication. The catheter bag must NEVER be raised above the patient's waist level because urine will backflow and add yet another infection to be treated. It is basic nursing care but it still happens.

Our Daughter is a respiratory therapist too. It's brutal what our front line medical folks are going through. I spoke to her on the phone the other day, (she lives in S.C.) She expressed to me her extreme emotional fatigue. The physical fatigue aside, she works 12-hour shifts with few days off and when she does have scheduled time off it's only for one day and even then they are calling her, trying to get her to come into work because they're short-staffed. They're ALWAYS short-staffed!! But the emotional is the worst of it. Every day, she's seeing people die. Families devastated. Moronic people who are dying but insist that it's "can't be Covid-19, because that's not real! Ya'all didn't do the right tests!" Meanwhile, they're dying from Covid-19 but still in complete denial! It's simply unexplainable to so many people who haven't seen it first hand. :*( It's simply astonishing.
OK, know what I see here? 5 dedicated professionals caring for a seriously ill patient. 3 of them ladies of colour, 1 of them appears to be wearing a hijab (I may be wrong, it could just be her PPE). The other side of American culture! Respect to these selfless people. Compare and contrast with so many other video clips on YT of entitled Karens and Kens whining about having to wear a mask...

Apparently you can turn yourself, and they can not stay on their stomachs for the whole day. As Rachael said this has been done for a long time, but she showed us to let us know what a job it is. Thank you to those people who do this all day long.
It's unfortunate that there are many in the medical field that do not believe that it is a big deal bc their hospital in the middle of the foothills northern California isn't churning out bodies like a hospital in NYC...

Respiratory Therapists have always been ignored, even though they are an important part of the medical field and not taken seriously. Finally a "Respiratory" pandemic happens, catapulting us into the highest risk of getting COVID in the hospital and still ignored. All the media has done is talked about COVID and ventilators all year. A ventilator without an RT is like a race car without a driver.
Since we no longer have county hospitals and only private ones (which needs to be rectified as well as our schools) you can bet if they don’t have its because the cooperate overlords say no!
@@GreenDragon11 , seems we are the lucky ones in Europe/Germany. And just to intensify the pain, we have to pay a flat fee of 10€ per day. And there is a cap of 280€/yr. And no tax payers fund this, the ICUs are financed by car insurance money (patients from accidents), the mandatory employer's liability insurance association (workplace accidents) and regular health insurance (strokes, transplants aso) and they use a third of the beds on a regular basis. Now the number of beds is tripled and they still reserve that regular number for non-covid patients. And we now have a lot of our red-caps driving the pandemic and ending up in the ICUs. Combined with open schools, but those are shutting down since end of November and now the curve is flattening again. Beware of open schools. And all of it isn't socialism, it's community cooperation on large scale.
Turning patients onto different positions regularly is nothing knew when laying helpless in hospital beds in order to preserve their health and obviously requires 2 to upto 5 staff members.
