ABOUT : Lora Shahine, MD, is a double board certified reproductive endocrinologist and OBGYN currently practicing at Pacific NW Fertility in Seattle, WA. Originally from North Carolina, Dr. Shahine graduated with a Bachelor of Science in biology from Georgetown University in Washington, DC, and completed her training in medical school at Wake Forest University School of Medicine, residency in obstetrics and gynecology at the University of California at San Francisco, and fellowship in reproductive endocrinology and infertility at Stanford University.

Currently pregnant with my first baby with IVF. We were told we had unexplained infertility because they had done “every” test, and they all came back perfect. Eventually we found out that they missed one test-a chromosome analysis test! It turned out that my husband has a balanced translocation, which would’ve only ever been discovered with a chromosome analysis test, and the reason they didn’t test us for it originally, was because we never had a miscarriage. Apparently they would’ve only tested us if we had a history of miscarriages, since that’s the most common scenario they see with balanced translocations. So if you have been diagnosed with unexplained infertility, please make sure you ask for a chromosome analysis test! The only way we knew to ask for the test was after our embryos came back from genetic testing and the lab that tested them told us the abnormal embryos had the same chromosome abnormality, which raised a red flag for a balanced translocation.

We are also going through an IVF cycle.. failed... We didn't go to the chromosomal analysis...I have a question please don't mind but after ur test result what changed with ur IVF treatment?

@@supriyasharma_life It made it very important that we do genetic testing of our embryos. Usually genetic testing is optional, but when you have a balanced translocation, it becomes almost mandatory because you are at increased risk of making embryos that have chromosomal abnormalities. So genetic testing of the embryos is important to catch the ones with the chromosome abnormalities.

We were just diagnosed with unexplained infertility, and YES, it feels like it would be easier to hear "x" is wrong, because right now everything looks fine and we have no idea what the issue is. :'-/ It was surprisingly hard to hear, because we were looking for an answer and it just felt like such a let down.

I love your videos Dr Shahine! I really wish I had this video before I started my journey. It would have helped me discuss options better with my REI, who I believe didn’t recommend the best low tech treatments and was only concerned with doing IVF. Now I’m so much better informed, and still hoping that I won’t have to do IVF but am mentally prepared if I do.

Perfect timing. Had one miscarriage and have been trying for one year. I am 38. I got my HSG booked (watched your vid). We have done all the other tests (husband is clear). So frustrating… but the more I read and learn, the more IVF is a possibility. ❤ good point about freezing embryo for future. Hoping for a baby soon after miscarriage in Jan 2023.

Did two HSGs and all other tests - same result. Unexplained infertility. Next step is IVF.

Hi! Thank you for a great video!! I have just started letrozole treatment. I’m a bit concerned about multiple eggs developing and the risk of getting pregnant with twins. My doctor says that there are no need for check ups every cycle now that we know from the first month that I react good on this dose. My question is if it’s, in general, common to not come in for check ups every month to see how many eggs are developing or is it normal to follow it up every cycle?

You should have at least one transvaginal ultrasound on 11th or 12th day of every cycle if using letrazole

Thanks for your video! we started trying march '23 and were able to conceive in July '23 that unfortunately led to a miscarriage. We took a break for a month and have been trying since October with no positive result. With it being March '24 now, can I still be considered in the "trying for a year" case?

I got married nine months ago. Every month we look forward to having a baby, but periods occur. I experience breast pain starting from the 15th day after my periods . That pain lasts until my periods start again. Am I ovulating? Or not?

Uncontrolled diabetes can be associated with increased risk of miscarriage and infertility but if your blood sugars are being cared for - less likely to be the cause - please review with your doctor❤️

Definition: You have a regular cycle, eggs, good sperm and one open fallopian tube and you've been trying with timed intercourse for about a year. Consider: age, health, chronic illness (including endometriosis), bad luck. Paths: natural, low tech, low tech plus shots, ivf. Evidenced based treatment plan: IUI alone, not proven. Ovulation induction alone, not proven, and vice versa. IUI in combo w/ ovulation induction has higher success rate. Best to try 3 times before IVF. Insufficient data that shots with IUI help at all, and higher risk. Treatment will likely reduce time to have a baby. Low tech also has fairly low success rates compared with ivf.
