Which is the BEST Dental Splint?
It is finally SPLINTEMBER and we kick it off with an all-time important question - which is the best splint for your parafunctional/bruxist/TMD patient?
Surely it's a Michigan...right? Or maybe it's a Gelb appliance? Or the humble soft splint...? Did I just say that?
SURELY it's an A- Splint, B-Splint or a C-Splint?!
Well, I have an answer for you...it's called the G-Splint!
The G-Splint* is the best dental splint there is.
There are so many factors that will determine this. In this episode, Dr Jaz Gulati explores many of the factors to consider for appliance therapy in the form of dental splints.
What are you trying to achieve?
Is the patient symptomatic?
What is the 'purpose' of the splint? (Hat tip to Dr Michael Melkers)
What is the goal?
What about access, cost, airway, orthodontics and compliance with splints?
Protrusive Dental Pearl: A quick way to remove temporary crowns and onlays using a haemostat!
Tune in for the rest of Splintember where I will go deep into different splints!
*The G-Splint is just a metaphor for splint provision based on the history, exam and diagnosis for your patient!

Hi,Dr.Gulati What kind of dental splint do you use on patients with full mouth dental implants rehabilitation? A lot of my patients still have bruxism after replacement of all their teeth with dental implants.... Congrats for the work on Protrusive dental podcast

Thank you Doc! The type of restorations (in this case implants) are not as important as the GOAL or REASON for appliance. In the case of a purely protective appliance (patient is not in pain) then any splint can work. For protective appliances I tend to use dual arch passively fitting essix retainers. In the case of 2 full arches of implants opposing each other, you could also give consideration to a B splint to anteriorize the forces (and reduce them) - hope that helps!

Will you be doing a video about splints for anteriorly displaced TMJ discs? I am not a dentist, I have TMD.

Hi Keren! If a patient's main concern is the sound of popping and clicking, then sometimes an anterior repositioning splint is indicated. This guides your jaw forward to 'recapture' the disc. It is a very niche appliance for a niche issue, and not something I manage commonly as a General Dentist. I will likely mention this appliance in passing and some side effects, but that is about it. Sorry if that disappoints!

I had adult braces 20 years ago, in my late 30's. My mouth and teeth was completely altered and my bite was pulled away to the left side. Resulting in a deteriorated jaw. But my question is will a split work when I need braces to correct my bite and alignment? Which should be done first, I have no solid bite, it's in limbo. Help Splint first or braces first?

Hi Sandra - sorry to read about your pain. My approach and logic is that before any braces (irreversible procedure) are tried, to correct the bite with splints first as a 'test run' Where I work: www.tmd.london
