I have a theory, Naoki told Kotoko that he wanted to be a doctor, but he wasn't specific (like what kind of doctor he dreamed of), then in S2 eps 1 (where we all know that in S2 Naoki and Kotoko were married), Kotoko was jealous because the woman named Mira who appeared in eps 1, kept approaching Naoki, and when Mira pretended to be sick, Naoki tried to examine her, and we also know that the examination requires physical touching, at that time Kotoko was jealous and didn't accept that Naoki had to touch another woman, then Naoki told Kotoko that Kotoko had married someone who would become a doctor, that's why Kotoko cried and left. At that time, Naoki decided to become a pediatrician, so that Kotoko would not be jealous or not accept. That's just my theory, I don't know, maybe Naoki has another reason.
I think it was kind of stated in the story as to why naoki choose paediatric. That time when his brother got sick and kotoko said something, i think that made him decide right there and then that he will become a paediatrician
I think the reason why Naoki choose pediatric, because when they were on date, there is a boy who got asthma. And Kotoko said so many children ill and Naoki has capability to make them feel better. After sending the boy to hospital.

ahhhh how I wish that there are full episode commentaries of this show. The commentaries make me feel like I am watching and fangirling with a bestfriend 🥺 I just watched my annual dose of itakiss and I saw this channel and I am surprised that I am falling even more inlove with it. Nobody among my circle understands my love for itakiss. To them, the show is immature. Everytime I watch it, there are always new things I am seeing and learning as if I am watching it again for the first time. The more I mature, the more I understand the characters and their choices. Watching your commentaries makes me feel validated with the things I love about the show. Ahhhgg kung pwede lang magkaroon ng watch party from season 1 to 2!!!!

This whole story is about the push/pull orientation of traditional and modern Japanese society. With some characters embodying both. Kotoko and Naoki’s story represent that transition.
It's sad that kotoko and naoki never got the chance to do the marriage ancestral rites before just like what he did with sahoko. You know facing each of their families sides and properly talk about their marriage. What happened was they got married quite abruptly. Both figuratively and literally. Although naoki is not completely againts the marriage. I bet he was caught off guard with the sudden set up his mother had prepared for them. That's why he expressed his dissatisfaction with how everything worked out while they were walking at the altar. I am sure as a man. Naoki don't want to skip everything and had kotoko married to him right away. He wants to make her feel secured first with their relationship as obviously kotoko is unconfident with his love for her. that's why he proposed marriage instead of making her his gf when he confessed. He wanted to take it slow. Get to know each other properly as a couple. Coz they had an open relationship althroughout season 1. You know when they act like a couple but they aren't one kind of vibe. But i think naoki was relieved of his status as her husband later in the series. Coz if he had married her after graduation he probably had a lot of rivals. Saying he's her husband was more secure than just saying he's her boyfriend. At least with his status as a husband. Those who asks for a chance with his wife would think twice and wouldn't pursue her as she's already married to him.
All this is great but there is one more reason, I believe. By agreeing to the arranged marriage, I think it was his last attempt at trying to go back to the boring life he thought he was more suited to and would feel more comfortable in. Only to realize, he was more comfortable in the chaos that Kotoko brings with her.
