My ex-husband is a cosmetic surgeon. As you can imagine, I’ve had multiple procedures and surgeries over the years. This video has excellent content and the delivery is very well done. Thank you for your comments Fernando. I think we have been too focused on using bigger cannulas, not remembering that if you have a big cannula, you will inject quickly (not to mention the fact that you might hurt the patient more and cause more swelling and bruising too). I inject super slow now with a 27g cannula, and I think the risk of intra-arterial injection is lower this way. I keep the cannula moving and put little pressure on the syringe. The patient ends up very happy as there is little pain, swelling, bruising or recovery time. Thank you again for your contribution.

I am agree with you. I usually use 25 and 22, 23 G cannula. It is more safely than 27.

Yes, that's what I thought too... until I started using 27g cannulas and SLOW injection of fillers. If you have a Porshce, you'll probably drive it fast. So I elect to drive the volkswagen of fillers. The 27g!

Victorian Cosmetic Institute I’ve never had a practitioner suggest using a cannula . Must say I’ve experienced some vert bad bruising in the past . What about a filler in the lips ? Is a needle still The best for lips ? Cheers

@@OlderWomenRock I personally believe cannula can do better than needle in lips. Much less swelling and bruising. But it takes a lot more skill to do lips with a cannula. You have to weave it through the lip. Needle just slices through everything. So easy to perform with a needle but you pay the price in swelling and bruising

I have fillers on the nose and I love the result. My practicionner used a cannula and told me why but not completly, I mean she didn't tell me that it could cause blindness and now I'm so afraid, I think I won't do it again :(

8 Years ago I had filler injected under my eyes and had a BRAO stroke (branch retinal artery occlusion) in one eye that caused a blind spot. Fortunately, my blind spot actually improved over time. I was never told by the doctor that this was a possibility and never had under eye filler again until recently with a cannula.

as a patient I've had both and by far the canula is my prefered choice (off course if applied by a trustworthy competent doctor). The difference is in in the pós-application. Less pain, less discomfort

I'm very impressed by your way of conveying. Thank you so much doctor. I request you to talk about hifu. How it works and in what age one should go for hifu. Skin ages more faster after hifu or its misconception? A local aesthetician said to me that hifu works better on young people. The more you young the better it works for you. Please please doctor. I usually use cannula to inject fillers and sculptra . But sometimes 27G cannula is not easy to perform the procedure(because it is soft and the diameter is too small for some harder fillers)Beside the larger cannula, do you have any suggestion ?

I just had my lip filler done last week and my doctor used a cannula. In terms of pain I personally think the cannula hurt more compared to the needles. My recovery time is taking longer too. That’s just my experience.

cannulas are much harder to use than needles, and with time and experience, cannulas can be a lot less painful than needles.

Hi Dr Chan, what would you recommend concerning a hyaluronic acid procedure called Profhilo? It has been recommended by my dermatologist, but after seeing this, would the HA dissipate over time or would I be left looking swollen in a few years ? I understand it's 'different' to fillers, but is that the truth?

I’ve heard that the “microdot technique” to deliver dermal filler is excellent, especially with atrophic scarring. Can this be done with a cannula?

yes this can be done with a cannula. But also you can do something called 'subcision' with the cannula too, which is undermining the scar and releasing it from its attachments.

Very interesting vidéo thank you . I have never understood how it was physically possible to penetrate artery with cannula? And also thé pain must have been unbearable, the patient didn’t react? I found on google that blindness was caused by penetrating with angle 90, and with force... Are there still doctors who could do in this way?

Hi doctor could you please explain how you open a way for cannula ? First once you enter with needle vert fast then you took it off and after you make a way you enter with cannula ? I always use needle and it takes time and it's sharp as you said

Thank you ! You just need to do it carefully - undercorrect. Take more than one session to get the final result. If it is just tear troughs, personally, I would use dermal fillers as it is much simpler . If you have facial fat loss (all over) then fat transfer might be more suited. Hi Dr. Gavin, could you please clarify in one of your previous videos - do you now only use 1 ml of filler for a patient's entire face per session or 1ml all up?

it's 1mL per session on average per session. Usually done every 1 to 2 years!

Using any cannula doesn't remove the risk. There has been a case reported in New Zealand where a cannula was used and resulted in immediate permanent blindness. It can significnatly reduce the risk however.

basically don't do this. You'll need about 200mL or more per side. A much better option is autologous fat transfer (moving your own fat into these areas).

You are so informative! Asking for my sister who is in med school....are the larger cannulas safer and I know nothing is 100 percent but how unlikely is it that a cannula could puncture an artery? I ask because I’m going to be her guinea pig once she graduates😂😂😂

Hi Staci. Many practitioners say that bigger cannulas are safer because they are less 'sharp' than a finer cannula and less likely to penetrate an artery. However I see it a different way. The finer cannulas are much harder to push filler through and hence you can't inject as quickly. I believe it is the speed of injection that matters more as well as keeping the cannula moving

In the future , they might invent a cannula with micro tubes inside of it with multi holes like a garden sprinkler so the procedure can be done quicker with less trauma and a more even distribution of the product into the face

Thank you, is the cannula sharp to pierce thru the skin, do you recommend 27 gauge to fill eye troughs? whats your thoughts on filling eye troughs through the oral cavity I have had this done once

We use a needle just to pierce the skin to create an entry point. Yes, can use 27g for tear troughs. That is what I do now. I don't fill anything through the oral cavity. I have seen this done before, and I think there are too many bugs going through this way. Also it is hard to reach the area you want to.

The initial entry point is made with a needle. Cannulas are a little more painful but it depends on the operator and their skill with a cannula

They use an introducing needle to create the hole first then slide the cannula in :)

Excellent video! I was taught this by one of my old attendings. Your explanation was spot on! Question: do you use an 18 or 20 gauge to “pre-puncture” the skin for the 27 g cannula? What’s your preferred method for getting the cannula past the skin. Many thanks.

Hi Samson. I use as fine a needle as possible to make the entry point for a 27g cannula. Usually 23g is a good. 25g needle is possible too but can be hard to fine the hole.

Dice que actualmente usa cánula , en vez de aguja. Con cánula puede navegar mejor, identificar en qué área anatómica se encuentra y realizar menos punciones (por ejemplo, en labios). Usa una 27g porque al ser de menor calibre es menos traumático y la velocidad de inyección es menor (menos probabilidad de inyectar en arteria).
