So for Forest, since they won it at 1979 and 1980, either 1978 or 1981 would have gotten them a badge in future Champions League games.

This badge is no longer used as of this year. Instead, those teams display the number of Champions League titles won on the "ordinary " Champions league badge
So the team that have won it at least 5 times or 3 in a row gets to wear the badge, however during champions league events, participating clubs can put the champions league badge only when they play champions league games not league or any other cup games thats my understanding
In the past AC Milan has the UCL Badge of Honor wherever they play, including the league. It's no longer possible since the UEFA combined those numbered badges with the logo (on the right hand side) since they allow sleeve sponsors on the left hand side.
