Will Food Acidity Affect My ADHD Meds?

Summary:
Lots of websites proclaim that acidic meals impair the absorption of Adderall, Ritalin, and other stimulant medications. Almost no research has been conducted that shows whether taking your morning pill with a glass of orange juice will really affect how much enters your body.
However, the basic physiology of drug absorption and excretion indicate that pH is an important factor. Amphetamine and methylphenidate are weak bases, and in highly acidic environments are more electrically charged, or ionized. That makes it harder for them to pass from the inside of the stomach or intestine into the cells lining the digestive system. However, when urine is more acidic, it is easier for the ionized form of the drug to pass into the urine and be excreted. So acidification can interfere with absorption and accelerate excretion.
On the other hand, far more drug absorption occurs in the small intestine than in the stomach. Even if the pH of the stomach is much more or less acidic than normal, this gets diluted very quickly along the meters and meters of small intestine, which excretes its own digestive juices. For many individuals, the acidity of their stomach becomes largely irrelevant then, to drug absorption.
Many people don't notice much impact from food acidity and the perceived potency of their stimulant medications. Some individuals, however, are quite sensitive. Immediate release formulations are more likely to be affected by stomach pH than are most slower release formulations, which have very little release into the stomach anyway.
However, some slow release medications, like Mydayis, have layers designed to dissolve only at a certain pH.
Separating pill intake from food by an hour or more may help if you are one of the individuals sensitive to this effect. However, the impact of antacids (histamine-2 blockers, or proton pump inhibitors) usually last for many hours, and can affect absorption throughout the day.

Anecdotally, I can tell you that taking calcium carbonate with immediate release stimulant medications _definitely_ increased the strength of their effects. That being said, I drink a lot of soda and have a pretty acidic diet as well.

I appreciate you sharing your experience. To clarify for other readers, calcium carbonate is a base, or ant-acid, which can reduce stomach acidity.

As a pharmacist, this is the type of thing that captures the attention of adhd med patients. It’s a safe bet they ran out of their meds when they go to pick them up so it says a lot that they pay attention to this topic! 😂

@@olic7266 depends whether they are running out because they are using the medications up more quickly than expected, or whether they are running out just because the forgot to refill their prescription when it was due. (I think that is what is implied in the initial comment.)

Benadryl also has quite strong anticholinergic action, so there are concerns that regular long term use may predispose to dementia - in Alzheimer's it is the acetycholine neurons that are damaged and disappear early on.

I honestly never feel a difference, I drink 1 sugar free energy drink a day because I still need energy and vitamins

Very good video! Can you say something about vyvanse/Amphetamines and vitamin c? I read that vitamin c increases amphetamine elimination

Vitamin C (ascorbic acid) does acidify both the stomach and the urine, so it could enhance elimination, as discussed in the video. But individual results may vary.

How does concerta, and generally methylphenidate products change in their efficacy and duration based on the food's or drug's acidities that we have consumed near the hour of taking the medicine?

Stomach acidity is affecting absorption - how much is entering your body, and how quickly. Whatever gets absorbed in the small intestine shouldn't be any less effective. If the amount/type of acid consumes is enough to change the urine acidity, that could increase how quickly methylphenidate is cleared from your blood.

Also can you do a video with any relation celiac disease / and adhd. I feel that gluten free products may help the absorption of meds. Id also like to include, ever sense I stopped adderall I noticed a big difference in my hands and feet not turning red as much, possibly due to blood flow?

Adderall and other stimulants can increase overall blood pressure, but for some people they cause Raynaud's phenomenon - constriction of small blood vessels in hands (or feet) causing paleness. It sounds like for you there was greater blood flow to your hands and feet when on them.

hello dr.jonh , i am 37 years old male trying adderral generic this is first time try adderal i am suprising it make me to sleepy (I might sleep while sitting on the chair firist week of taking adderall generic) , lazy with headache and so depressed and want to cry and lose my self confedence , this effect is dose indepentant tried to decrease and increase and the same effect although i try to sleep good 8 to 9 hour . then try to add concerta but adderral generic cancelled the good effect of cocerta to me , before i had tried vyvanse 60 and its better than addrall at the level of concentration but no motivation and cuase bad mood to me somtimes headache and sexual and erctile disfuntion i think bc it cause severe vasoconstriction to me , what is happenening with me in your opinion ? ( i am non alcholic and dont use any drug , i did some test for blood and liver and kidney and heart and thiroid and lipid profile evry thing almost perfect just foud that i has high testosterone level with hight LH level by my nature ) . only the compination of cocentra 54 to 72 mg with retalin 30 mg improved my adhd symptoms by 50% (must open cocerta tablete and eat it , bc even if i tak 72 mg concerta without open it has almost no improvment or mood crash feeling all the day ) . do you advise me to try dextrine ? ( i had allmost all ADHD symptoms all my doctors said that iam typicall case of ADHD)

Unfortunately, I don't have a good explanation for such different effects from different formulations and medications. Individual variation certainly is common with response to medications.
