One of the nursing homes is Special nursing home, we call it Tokubetsu Yougo Roujin Home(特別養護老人ホーㇺ). Tokubetsu Yougo Roujin Home is abbreviated as特養(Tokuyou). Most of foreign caregivers will work for 特養. Tokuyou, the special nursing homes have daily routine works and Caregivers need to follow the time schedules.
As you know, Japanese are very punctual. Look at railways system. Train always comes on time. If the train come late, even 1 min, Station employees announce to apologize. Maybe most of you feel culture different about time management.
Anyway, in order to understand Japanese way of caregiving, I want to show you a typical routine work in a special nursing home. First is morning shift, second is night shift. After this lesson, you will be able to understand the time schedule in a special nursing home.
OK, Today’s topics is Work in a special nursing home
I will show you the time flow in the morning shift.
You start morning care at 6 am, and breakfast at 7 am, 9 am, you start to do morning activities. At 11:30, you will prepare for lunch time. At 1:30 PM afternoon activities. At 3 PM, snack time and at 5PM, dinner time. OK, let me explain the detail of each schedule.
6:00AM Morning care, we call it in Japanese, 起床介助. When you are assigned on Morning shift, you will start at morning care, 起床介助. By the way, morning shift is called, 朝勤,
First of all, you visit at user’s private room, private room is 居室. The type of private room is single room, or shored room. Shared room is usually good for 2 or 4 people.
You greet the user, “Good morning おはようございます!” When you communicate with a user, like greeting or saying something, asking something to a user, we call it in Japanese声掛け.
After communicating with the user, you do excretion care 排泄介護. If the user is bedridden and can’ t move, you change the diaper on the bed. Change the diaper オムツ交換
But if the user can stand, you will transfer the user to a wheelchair. Transferring the user from the bed to the wheelchair, or from the wheelchair to the bed, we call it 移乗介護. Transferring assistance 移乗介護
Then you push the wheelchair to the toilet or use the portable toilet, depending on user’s physical condition. Assistance with toileting, トイレ介助

After excretion care, you help the user to change clothes. One of the philosophies of Japanese style of caregiving is supporting independence. Support a user to be independent. We don’t help with everything. When a user can do it, let the user do. When the user needs help, we help. Sometimes it takes more time to let a user do rather than helping, but the caregivers should wait and see, and let the user do to bring out their potential ability that they still have. Assistance with changing clothes is 更衣介護
Next, glooming care 整容介助, like oral hygiene care, face wash, brushing hair, shaving, and makeup.
When the user gets ready for the day, you guide the user to the dining room. Dining room is 食堂. Guide or lead a user to the destination is 誘導.
7:00AM Breakfast. 朝食
First you change the apron for the dining room for proper sanitation. Then you serve a breakfast. Serving a meal to users is 配膳.
This is the typical Japanese breakfast, Rice, miso soup, grilled fish, and vegetable side dish. Looks colorful and delicious.
If the user needs assistance, you do assistance with eating, 食事介助.
After a meal, most of users take medicine. Caregivers watch them to take medicine. If the user forgets, you remind the user to take medicine. Taking medicine is 服薬
You bring the user to the bathroom sink for oral hygiene care. If the user uses a denture, help to wash it. Oral hygiene care is 口腔ケア
At 8:30AM, you join the staff meeting in the morning, Staff meeting, 申し送り
The staffs assigned on night shift report what happened during nighttime. If they encountered something unusual or emergency, they should report to everybody. Care service is conducted by a team. Therefore, you should share what you find and what you feel with people involved.
This is the typical time flow from morning care to staff meeting in the morning.
9:00 AM, morning activities starts. What do you do is, bating care, watching at dining room, or helping users to go out. You will be assigned with one of the activities
Bathing,入浴
When you are assigned on bathing care, you bring the user to the bathroom. Bathroom 浴室、Lead or guide a user to the destination, is 誘導, Lead a user to the bathroom 浴室へ誘導
At the bathroom, the user takes off clothes. If the user is paralyzed, like hemiplegia, you assist her to take off her clothes. Assistance with changing clothes, 更衣介護
Japanese loves to take a bath. Usually the users can take a bath at least 2 times a week. Bathing care is 入浴介護
